Neurosurgeons performing tracheostomies- maintaining proficiency in the modern era.

OBJECTIVES: Tracheostomy is a basic surgical procedure that most surgeons, regardless of specialty, learn early in their training. With improvements in intensive care medicine, the number of neurosurgical patients requiring tracheostomy has declined. As neurosurgeons advance in their training, familiarity with airway management declines and falls under the domain of other specialties. Because neurosurgeons still manage critically ill patients, they often defer the airway management to other specialists. In many institutions, neurosurgeons no longer perform tracheostomies. The purpose of this s…

Aberrant regeneration of the oculomotor nerve: implications for neurosurgeons.

Aberrant regeneration of cranial nerve III, otherwise known as oculomotor synkinesis, is an uncommon but well-described phenomenon most frequently resulting from trauma, tumors, and aneurysms. Its appearance usually follows an oculomotor palsy, but it can also occur primarily without any preceding nerve dysfunction. It is vital that neurosurgeons recognize this disorder because it may be the only sign of an underlying cavernous tumor or PCoA aneurysm. The tumor most often implicated is a cavernous or parasellar meningioma, but any tumor that causes compression or disruption along the course of…

Do neurosurgeons subscribe to the guideline lumbosacral radicular syndrome?

BACKGROUND: This study presents a survey of the opinion of neurosurgeons on the multidisciplinary clinical guideline 'lumbosacral radicular syndrome'. The aim was to describe to what extent neurosurgeons in the Netherlands endorse the content of this guideline. The guideline was issued in 1996 by the Netherlands Institute of Quality Health Care and this is the first attempt to evaluate the implementation of this guideline. METHODS: All active neurosurgeons (n=92) in the Netherlands were invited to complete a questionnaire investigating to what extent they agree with the 26 recommendations in …

[Palliative Care for Pediatric Brain Tumors:Essential Skills for Neurosurgeons].

Pediatric brain tumors are the leading cause of cancer-related deaths in children, and cause unique suffering by degrading the neurological functions essential to a child's identity. Palliative care is no longer seen as terminal care, but rather as a crucial approach to improve quality of life (QOL), which should integrated from the time of diagnosis alongside curative therapy. Effective symptom management is fundamental, addressing increased intracranial pressure with steroids, and pain via a stepwise approach using opioids, adjuvant analgesics, seizures, and swallowing difficulties through m

Aneurysmal Subarachnoid Hemorrhage; Early Surgery; Neurosurgeons Experience; Patient Outcome.

BACKGROUND: This study examined the impact of neurosurgeons' experience on surgical timing and outcomes in aneurysmal subarachnoid hemorrhage (aSAH) and questioned the adherence to early surgery as recommended by recent guidelines. METHODS: A retrospective analysis of 196 aSAH patients treated between 2013 and 2020 was conducted. Variables included age, sex, initial Glasgow Coma Scale (GCS) scores, World Federation of Neurological Surgeons grades, Fisher's grades, rebleeding, hydrocephalus, and preoperative-postoperative neurological status. Neurosurgeons' experience was categorized by the nu…

A Humanist Neurosurgeon: A Legacy of Dr. Roy Selby.

The article is dedicated to the life and work of Dr. Roy Selby (1930-2002), an American neurosurgeon who founded neurosurgery in Malaysia. Dr. Selby stayed in Malaysia from July 1963 to May 1970. He opened the first neurosurgical department at the general hospital in Kuala Lumpur and established a training program under which Malaysian physicians and nurses were sent to neurosurgery centers in the United States and Canada. Some physicians came back and headed local neurosurgical units. On his return to the United States, Dr. Selby practiced neurosurgery until 1986, when he had to give it up du

Balancing the Scalpel and the Heart: A Neurosurgeon's Guide to Empathy.

Empathy is a basic core human characteristic, the foundation for social relationships and interpersonal attachment. In medicine, particularly in neurosurgery, empathy goes beyond the concept of clinical neutrality and emerges as a guiding philosophy to form patient-centered care. Neurosurgical patients, who are often handling complex, life-changing pathologies, benefit greatly from empathetic reassurances that diminish their anxiety and establish a more robust patient-clinician connection. However, constructs of modern healthcare that undermine empathy include the very high value placed on tim

Surgical Considerations in Endoscopic Pituitary Dissection for the Neurosurgeon.

Many surgical considerations exist for an endoscopic approach for pituitary surgery for a neurosurgeon. The neurosurgical approach to endoscopic pituitary dissection requires proper surgical planning and identification of relevant anatomic structures. With the introduction of endoscopic transsphenoidal pituitary surgery, better visualization and more intricate surgical resections are achieved. Whether performing this surgery solo or with an otolaryngologist, the neurosurgeon must consider multiple aspects for this surgical approach. This article focuses on the surgical considerations involving

Muscle and nerve biopsies: techniques for the neurologist and neurosurgeon.

OBJECTIVE: Muscle and nerve biopsies are commonly performed procedures for the diagnosis of neuromuscular disorders. Neurologists and neurosurgeons are often consulted to perform these procedures in clinical practice. We provide guidelines in the performance of muscle and nerve biopsies. METHODS: We describe the technique for performance of muscle and nerve biopsy, and review the relevant literature. RESULTS: The quadriceps muscle is the most typical biopsy site for most myopathies, whereas the sural nerve is the most common nerve biopsy site for most peripheral neuropathies. Other sites may…

W. James Gardner: pioneer neurosurgeon and inventor.

W. James Gardner, a skillful neurosurgeon and inventor, is best remembered for his cervical tongs and hydrodynamic theory of syringomyelia. A pioneer of modern neurosurgery, Gardner trained under Charles Frazier in Philadelphia, and in 1929 he moved to Ohio where he became chief of neurosurgery at the Cleveland Clinic, a position he was to hold for the next 33 years. A large surgical practice made it imperative for Gardner to develop surgical methods that were quick, effective, and advantageous for patient and surgeon. He was an early proponent of the sitting position for patients undergoing c

[Precautions for Neurosurgeons in Administering Anti-Amyloid β Antibody Therapy].

In Japan, anti-amyloid β (Aβ) monoclonal antibodies, including lecanemab and donanemab, have recently been approved as disease-modifying therapies for early-stage Alzheimer's disease (AD). These drugs, developed based on the amyloid cascade hypothesis, target toxic Aβ aggregates: lecanemab selectively binds to soluble protofibrils, while donanemab targets Aβ plaques. The Ministry of Health, Labour and Welfare (MHLW) has issued Optimal Use Guidelines that specify criteria for administration: informed consent from both patients and caregivers; cognitive assessments (MMSE and CDR); confirmation o

Immune Checkpoint Inhibitors for Brain Metastases: A Primer for Neurosurgeons.

Immune checkpoint inhibitors enhance immune recognition of tumors by interfering with the cytotoxic T-lymphocyte-associated antigen 4 (CTLA4) and programmed death 1 (PD1) pathways. In the past decade, these agents brought significant improvements to the prognostic outlook of patients with metastatic cancers. Recent data from retrospective analyses and a few prospective studies suggest that checkpoint inhibitors have activity against brain metastases from melanoma and nonsmall cell lung cancer, as single agents or in combination with radiotherapy. Some studies reported intracranial response rat

Importance of Recognizing Carpal Tunnel Syndrome for Neurosurgeons: A Review.

Idiopathic carpal tunnel syndrome (CTS) is a common complaint, reflecting entrapment neuropathy of the upper extremity. CTS produces symptoms similar to those of other conditions, such as cervical spondylosis or ischemic or neoplastic intracranial disease. Because of these overlaps, patients with CTS are often referred to a neurosurgeon. Surgical treatment of CTS was started recently in our department. Through this experience, we realized that neurosurgeons should have an increased awareness of this condition so they can knowledgeably assess patients with a differential diagnosis that includes…
