The epidemiology of malaria in Papua New Guinea.

Papua New Guinea (PNG) is a patchwork of different ecological zones, inhabited by human populations of exceptional cultural and linguistic diversity. This results in complex variations in vector ecology and malaria epidemiology. Malaria is the main cause of morbidity in many health facilities in lowland areas, but it is absent in much of the highlands. All four human malaria species occur, but endemicity varies widely, with Plasmodium falciparum locally reaching holo-endemic levels that are rarely found outside sub-Saharan Africa. The high frequency of Plasmodium vivax is an important differen…

Diarrhoea in children in Papua New Guinea.

National data for diarrhoeal disease in children can only be used as a very rough guide to morbidity and mortality, since they are based on … ntive strategies. These include breastfeeding (which has saved the lives of many thousands of Papua New Guinean children and which is once again under threat), ensuring good host defence by good nutrition, immunization and early treatment of childhood illness, and ensuring satisfactory sanitation and hygiene. Increasing fluid intake to prevent dehydration re …

Arthritis in the highlands of Papua New Guinea.

Acute polyarthritis is an important cause of morbidity in many tropical countries. Classification has often been difficult, with the term tropical polyarthritis used for those in whom a diagnosis could not be made. The implication that this is a distinct entity is probably incorrect, with likely causes being septic arthritis or post-infective reactive arthritis. This study aimed to determine the types of arthritis found in 43 patients (30 men) presenting consecutively to the Goroka Base Hospital in the Eastern Highlands of Papua New Guinea. Gonococcal arthritis was diagnosed in eight patients …

Malaria in children in Papua New Guinea.

Malaria is a major problem in children in many parts of Papua New Guinea; it has a high mortality and, if observations from other countries are applicable, significant long-term morbidity. Standard treatment regimens are the backbone of management of malaria throughout the country, but these must continue to be reviewed and updated in the face of changing patterns of parasite drug sensitivity. Diagnosis of malaria will for practical purposes remain primarily clinical for the foreseeable future. There is a need for uniform reporting of malaria statistics using the established definitions of unc…

The epidemiology of fasciolosis in Papua New Guinea.

Fasciola hepatica metacercariae were present on pasture throughout the year at the trial site. The highest infection rate in trial sheep occurred in the first year after the beginning of the wet season, in December/January, when contamination of the pasture was maintained by other sheep. Variation in rates of infection in consecutive years was linked to differences in grazing pressure and to a change from sheep to cattle as accompanying grazing stock. A minimum precipitation of 125 mm over 4 consecutive weeks appears to be necessary for infected snails to move from their normal habitats and to

Treponematosis in the eastern highlands of New Guinea.

Serological tests for treponemal disease were undertaken among the inhabitants of 10 census units in the Eastern Highlands of New Guinea. Many sera gave reactive results to some or all of the tests performed. To exclude biological false positive reactions the Treponema pallidum immobilization (TPI) test was carried out on each serum, the results being taken to indicate the presence or absence of treponemal disease in the individual. Clinically, leprosy and malaria were rare and no cases of active yaws were seen. Some middle-aged people showed clinical evidence of old yaws infections. The preva…

Obstructive sleep apnoea: a new disease for Papua New Guinea?

Obstructive sleep apnoea is a common disorder in western societies and has a strong association with obesity and alcohol use. The condition has not previously been recorded in Papua New Guinea. The clinical details of 2 patients from Papua New Guinea with obstructive sleep apnoea are described, and the principles of treatment of this condition are outlined. Sleep apnoea is likely to become an increasing problem in Papua New Guinea.

Insights into Infant Strongyloidiasis, Papua New Guinea.

The human-infecting parasite Strongyloides fuelleborni subspecies kellyi has been reported from the island of New Guinea. We analyzed fecal DNA extracts (n = 164) from 19 infants in Papua New Guinea by using Strongyloides real-time PCR and undertook metabarcoding of cox1 and 18S rRNA hypervariable regions I and IV loci. Eight infants were infected with Strongyloides spp.; 7 were infected with S. fuelleborni subsp. fuelleborni and 1 with a Strongyloides sp. previously misattributed to S. fuelleborni subsp. kellyi. Phylogenetic and haplotyping analyses indicated S. fuelleborni in Papua New Guine…

Benign breast disease in Papua New Guinea.

In this study 302 biopsy-confirmed benign breast lesions in Papua New Guinean (Melanesian) women during a ten-year period were reviewed. Inflammatory breast disease was the most frequent type. Chronic mastitis, chronic suppurative mastitis and abscesses formed 32% of benign breast lesions and 71% of the group of mastitides. 10 cases of tuberculous mastitis were recorded. Fibrocystic disease was observed at a lower age (31.9 +/- 8.2 years) than has been reported from Black United States and West Indian populations. The high frequency of mastitis is an important finding. This is most likely to b…

Childhood malignant tumours in Papua New Guinea.

Data from the Papua New Guinea Tumour Registry and the Central Pathology Department were reviewed in order to document the incidence and pattern of malignancies in children in Papua New Guinea. Altogether, 680 cases of histologically defined childhood malignancies were recorded during the 14.5 years from 1971 to 1985. The frequencies of the various tumours were compared with past data and with published data from other countries. The incidence of malignancies in Papua New Guinean children appeared to be low, 36.5/1,000,000/year, with a male:female ratio of 1.6:1. Lymphoma was the most commonly…

Grass-skirt burns in Papua New Guinea.

A retrospective survey of burn cases admitted to Alotau Hospital, Papua New Guinea, over a four-year period showed that 48% were due to grass-skirt burns. Most of these occurred in young girls and usually caused full-thickness burns of the buttocks and thigh. The commonest long-term complications were contractures of the hips and knees. The next most common cause of burns was a fall into a fire during an epileptic fit (24%). None of the patients were on anticonvulsants. The population needs to be informed of the dangers of leaving children near open fires, of the value of bundling up children …

Glaucoma in Port Moresby Papua New Guinea.

A retrospective study was carried out, of 48 patients with glaucoma, who presented over a 2 year period at Port Moresby General Hospital. No cases of chronic open angle glaucoma or acute angle closure glaucoma with shallow anterior chamber were found in the 37 Papua New Guinean patients. Trauma was the most common cause of glaucoma in this group particularly in males. Glaucoma also occurred in association with the exfoliative syndrome and secondary to anterior uveitis. All non-Papuan New Guineans retained some vision in both eyes while 23 of the Papua New Guineans had no perception of light in…
