[Traumatology of the nose].

Isolated nasal bone fractures are the most common injuries of the central midface. They can be divided into different fracture types, depending on the exposure and orientation of violence. It is essential to ensure that there is no septal hematoma or abscess, which would require an emergency surgical intervention. A closed reposition can be offered if misalignment of the nose can still be observed after the edema swelling has gone down. In case of insufficient aesthetic and functional results, rhinoplasty/septal reconstruction can be performed 6 to 12 months after the trauma at the earliest. I…

Reconstruction of the Nose.

Nasal reconstructive techniques have advanced significantly over the past 50 years. Modern techniques in nasal reconstruction are based on the nasal aesthetic subunits. In order to achieve ideal outcomes, reconstructive surgeons must consider differences in tissue qualities across the nasal aesthetic subunits and formulate reconstructive plans based on these differences. Local flaps, skin grafts, and several types of interpolated flaps comprise the most commonly used techniques for nasal reconstruction. Defects that involve structural or internal lining defects require reconstruction of signif

The Short Nose.

The causes of the short nose deformity vary greatly, from congenital malformations to acquired deformities. Despite this degree of variation, key commonalities exist, namely, a shortened nasal length, overrotation of the nasal tip, and increased nostril show. This article is designed to help the reader identify precise causes of the short nose, outline associated anatomy, and discuss reliable surgical techniques to correct this deformity.

[Twisted noses].

Correction of a twisted or crooked nose can be very complex and require the use of a broad range of surgical techniques. Patient needs are often mixed--aesthetic and functional--particularly in post-trauma cases. The quality of postoperative breathing is therefore as important as correction of the nasal deviation. Extracorporeal septoplasty or spreader grafts are very effective not only in correction of the nasal pyramid deformation, but also in resolution of functional respiratory issues. Reconstruction and proper support of the septum are necessary components for a straight nose. Aesthetic d…

The crooked nose.

Straightening a crooked nose is a challenge. Several techniques exist ranging from simple to technically complex. It is important to approach the nose systematically and to remember that perfection may be impossible to achieve. If straightening is not attainable, softening grafts or onlay grafts should be used to camouflage subtle irregularities.

The twisted nose.

To the patient seeking correction of a twisted nose, it is as important to obtain a good shape as it is to achieve normal breathing. The functional problem must be solved regardless of the extent of the resection required. The plastic surgeon may choose among many techniques, some of which have been elucidated. Our preferred approach has been described in detail. We reiterate that experience has shown that a deviated septum which requires stabilization after surgery will recur to a greater or lesser degree at a later date. A twisted nose, to be permanently corrected, should maintain its correc…

Swimmer's nose deformity.

This is the first published report of the swimmer's nose deformity. This common athletic deformity has a characteristic, asymmetric dorsolateral nasal hump that progressively develops over years during a competitive swimming career and persists after cessation of the sport. The cause is thought to be bone and soft tissue remodeling in response to repetitive trauma chronically inflicted by a swimmer's goggles during the water reentry phase of breathing.
