Oliguria and renal failure from a giant pelvic lymphoma.

Extranodal presentations of lymphoma typically manifest in the gastrointestinal tract, skin and central nervous system. Occurrences in the pelvic region are relatively rare and can pose significant diagnostic challenges. Our patient presented with progressive oliguria and eventually renal failure. He underwent bilateral percutaneous nephrostomy. After several scans, a colonoscopy and a percutaneous biopsy confirmation, he was treated with chemotherapy. This case highlights the diagnostic challenges faced in patients with large pelvic tumours and the importance of considering pelvic lymphoma as…

Transient Oliguria during Anesthesia in Cerebral Salt Wasting Syndrome.

Cerebral salt wasting syndrome is a hyponatremic and hypovolemic condition caused by intracranial disorders, such as head injury, subarachnoid hemorrhage, brain tumor, and brain operations. We report a case of a 5-year-old girl that had cerebral salt wasting syndrome with marked polyuria who showed transient oliguria during general anesthesia. The patient had undergone an operation for traumatic intracranial hemorrhage three months prior and has had marked polyuria and hyponatremia since then. After induction of anesthesia for cranioplasty, the patient had oliguria during surgery and then resu…

Oliguria, creatinine and other biomarkers of acute kidney injury.

Acute kidney injury (AKI) and fluid overload are conditions that require an early diagnosis and a prompt intervention. The recognition of these pathologic conditions is possible in the early stages if specific signs and symptoms are taken into account. Among them, oliguria represents an important sign. Reduced urine output for a certain number of hours may be an important sign of kidney dysfunction. This must be evaluated in conjunction with other factors such as hydration status and use of drugs. At the same time, traditional markers of kidney function such as urea nitrogen and creatinine mus…

Prolonged oliguria with survival in acute bilateral cortical necrosis.

Survival is uncommon in cases of acute bilateral cortical necrosis. Three cases admitted to the renal unit at Newcastle have regained useful renal function after oliguric phases of 38, 46, and 120 days.Prolonged periods of intermittent dialysis are justified in patients in whom a firm diagnosis of acute cortical necrosis is made.

Prediction of fluid responsiveness in severe preeclamptic patients with oliguria.

PURPOSE: Because severe preeclampsia (SP) may be associated with acute pulmonary oedema, fluid responsiveness needs to be accurately predicted. Passive leg raising (PLR) predicts fluid responsiveness. PLR has never been reported during pregnancy. Our first aim was to determine the percentage of SP patients with oliguria increasing their stroke volume after fluid challenge. Our second aim was to assess the accuracy of PLR to predict fluid responsiveness in those patients. METHODS: Patients with SP were prospectively included in the study. In the subgroup developing oliguria, transthoracic echo…

[Low-dose of furosemide to correct oliguria in gynecological surgery].

OBJECTIVE: To investigate the safety and efficay of low-dose furosemide in the correction of oliguria in the patients undergoing gynecologic surgery. METHODS: A total of 120 patients, aged between 20 to 50 years old, who were scheduled to receive elective gynecological open surgery under general anesthesia, were randomly divided into 3 groups: the control group, furosemide 0. 05 mg/kg (F0.5) group and furosemide 0. 1 mg/kg (F1) group (n=40). During surgery, blood volume and blood pressure was maintained in the normal range. The urine volume was recorded every 30 minutes. Oliguria was defined …

Care of the critically ill parturient: oliguria and renal failure.

The incidence of acute renal failure in pregnancy has decreased. This decrease is less marked in developing countries in which resources are more scarce. The clinical diagnosis of acute renal failure is crude due to the variability of clinical signs and the late occurrence of basic biochemical abnormalities. Obstetric and gynaecological diseases are found among the traditional pre-renal, intra-renal and post-renal causes of acute renal failure. The cornerstone of management is the identification of high-risk cases and the prevention of acute renal failure by maintaining intravascular volume. T

What is the significance of intra-operative oliguria with regard to post-operative acute kidney injury? A physiology-informed framework for intra-operative oliguria management.

BACKGROUND: Intra-operative oliguria is frequently interpreted as a surrogate for hypovolaemia or impaired renal perfusion, often prompting reflexive and potentially harmful fluid administration. However, urine output during anaesthesia is influenced by a complex interplay of neurohormonal activation, surgical stress, and positive pressure ventilation, which can decouple diuresis from actual renal blood flow. This perspective articles critically appraises the evidence linking intra-operative oliguria to acute kidney injury (AKI) and explores the physiological pitfalls of interpreting urine out…

Mild oliguria in preterm infants who later developed periventricular leukomalacia.

The aim of this study is to determine whether or not renal involvement was present during the early neonatal period in preterm infants with … iables between the two groups. The preterm infants who later developed PVL had mild but significant oliguria during the first 24h of life. This suggests that preterm infants with PVL will have renal involvement immediately after birth.

Birth asphyxia associated oliguria: relationship to outcome at 1 year.

The relationship between the degree of oliguria following severe birth asphyxia and outcome at 12 months was examined in 31 infants. All 31 infants developed encephalopathy following severe birth asphyxia and 25 had oliguria for 24 h or more following delivery. Eighteen had persistent oliguria (i.e. greater than 48 h) and the remaining seven had transient oliguria (between 24 and 48 h). Poor outcome (death or neurological abnormality at 12 months) was significantly associated with the degree of oliguria. Encephalopathy, however, was found to be more closely correlated with poor outcome rather …

Predicting oliguria following indomethacin for treatment of patent ductus arteriosus.

Fifty-two consecutive infants who received intravenous indomethacin, 0.2 mg/kg, were reviewed to determine factors associated with the occurrence of oliguria. Serum creatinine and concentration prior to indomethacin did not predict the occurrence of oliguria. A mean urine output of less than 4.5 mL/kg/hr prior to the administration of indomethacin was the only factor that we determined to be associated with a significant risk of oliguria. Two infants developed symptomatic oliguria and both had a pre-indomethacin urine output less than 1.5 mL/kg/hr.

[Oliguria and acute renal dysfunction in a six-month-old infant].

The infant (a girl aged 6 months) was admitted to the hospital because of oliguria and acute renal dysfunction. The laboratory examination results showed serious metabolic acidosis and increased blood urea nitrogen and serum creatinine levels. The patient continued to be anuric after 10 days of treatment with continuous renal replacement therapy (CRRT). she died a day later. The family history showed that the patient's sister died of acute renal failure 6 months after birth. The genomic sequencing results showed AGXT mutation in the patient and confirmed the diagnosis of primary hyperoxaluria …

Low dose dopamine in the treatment of persistent oliguria in pre-eclampsia.

Oliguria in pre-eclamptic women is most often a result of decreased intravascular volume. In a small number of patients, renal vascular spasm may be the cause of decreased urine output. Prolonged oliguria/anuria secondary to vasospasm may lead to permanent renal damage. When volume repletion is unsuccessful in restoring urine output, some authors have suggested the use of peripheral vasodilators such as hydralazine. Dopamine in low doses 2 micrograms/kg per min was used successfully to restore urine output within an hour in a pre-eclamptic patient who had been essentially anuric for 8 h. Volum…

Early hyperkalaemia in very low birthweight infants in the absence of oliguria.

We reviewed 1552 admissions to a neonatal intensive care programme; seven, all with a birth weight less than 1500 g, developed early onset, non-oliguric hyperkalaemia (potassium concentration greater than 7.0 mmol/l). Although their perinatal variables were similar to those of a normokalaemic group, hyperkalaemic infants had a higher incidence of intraventricular haemorrhage and developed increased concentrations of plasma creatinine by 7 days of age.
