Onycholysis is the detachment of the nail plate from the nail bed. If drug-induced, it can be an isolated phenomenon, but it may also accompany or follow a cutaneous phototoxicity reaction due to drug intake and exposure to ultraviolet irradiation. Photo-onycholysis is a rare photosensitivity reaction due to exposure to either a natural or artificial source of light. Many drugs are responsible for this phototoxic reaction, especially tetracyclines, psoralens, chloramphenicol, non-steroidal anti-inflammatory drugs, fluoroquinolones, and, rarely, doxycycline. Any patient given enough of a therap…

An uncommon manifestation of phototoxicity, photo-onycholysis results in the separation of the distal nail from the nail bed. Photo-onycholysis may follow the use of various medications or may occasionally be idiopathic. We present a case of photo-onycholysis in a patient treated with doxycycline for acne vulgaris.

Photo-onycholysis due to indapamide presented in a 75-year-old woman who was being treated with the diuretic for hypertension. The onset of her photo-onycholysis was delayed because of inadvertent photoprotection in the form of artificial nails and occurred in the absence of a generalized photosensitive eruption. Her nails returned to normal after withdrawal of the drug and application of opaque nail polish.

Onycholysis caused by Candida krusei is rare. A 21 years old male patient presented with grayish discolouration and elevation of all fingernails since one year. Patient was refractory to treatment with fluconazole. Potassium hydroxide preparation of subungual debris revealed fungal elements. Growth on Sabouraud dextrose agar was identified by cultural characteristics, morphotyping, microscopy and biochemical tests as Candida krusei. The isolate was resistant to fluconazole and amphotericin-B but susceptible to nystatin and clotrimazole. Patient responded well to clotrimazole and terbinafine.

Onycholysis was found in 7 workers engaged with manual skinning of cattle. In all of them the nail problems started shortly after employment. The most likely explanation for the persistent onycholysis is work related pressure anoxia of the nailbed.
