Follow-up and therapy of 136 patients with nonspecific salpingo-oophoritis evidence the efficacy of acupuncture included in a complex of therapeutic measures for the treatment of this condition, as compared to traditional therapeutic modalities. Acupuncture therapy is conductive to shortening in-hospital therapy, to full-value normalization of the body immunobiologic defense and of the autonomic nervous system, and to a reduction of the incidence of the condition recurrences.

Xanthogranulomatous salpingo-oophoritis is an extremely rare entity. The clinical features are similar to the common benign and malignant adnexal diseases, making it difficult to diagnose. Here we present a case of pelvic mass with high level of tumor markers who was operated with suspicion of adnexal tumor. Histopathology revealed it to be a case of xanthogranulomatous salpingo-oophoritis.

A 45-year-old female with oophoritis (pelvic inflammatory disease) caused by Campylobacter fetus (C. fetus) is reported. She was admitted to the hospital because of high fever and an acute abdomen. On admission, severe inflammation was observed by the laboratory findings, and abdominal X-ray and CT scan revealed ileus with marked swelling of the right ovary. A 39-year-old woman with fever of unknown origin was admitted to our hospital. Whole body scintigrams with 67Ga-citrate showed an abnormal accumulation of radioactivity in the pelvic cavity. Cystadenocarcinoma of the ovary was suspected on the basis of findings obtained by abdominal computed tomography and magnetic resonance imaging. Left oophorectomy was performed, and oophoritis was diagnosed. We would like to propose that 67Ga-citrate scintigraphy may be useful for the diagnosis of oophoritis as well as gynecologic malignant tumors.

Differential-diagnostic value of various clinical and laboratory tests was assessed in 105 patients with acute nonspecific salpingo-oophoritis. Auxiliary methods, such as cytochemical studies of neutrophil alkaline phosphatase, lymphocyte acid phosphatase, lymphocyte glycogen and lymphocyte succinate dehydrogenase, as well as the identification of auto-organic antibodies to the enosalpinx and ovarian tissue, were used to assess the severity of the condition. Their diagnostic value is stated with reference to the severity of adnexal inflammation.

There is a potential role for laparoscopy in the management of patients with suspected salpingo-oophoritis. It would be helpful in establishing the diagnosis in patients who are admitted with a clinical diagnosis of salpingo-oophoritis. Laparoscopy also would be beneficial in the long-term evaluation of fertility of patients treated for salpingo-oophoritis. Finally, this technique will delineate the microbiologic details of patients with endosalpingitis. How frequently laparoscopy should be employed will be dependent upon findings from prospective studies.

BACKGROUND: Xanthogranulomatous oophoritis (XGO) is a rare chronic inflammatory con-dition that can mimic ovarian malignancy both clinically and radiologically. It is characterized by foamy histiocytes and inflammatory infiltrates, often leading to diagnostic challenges. OBJECTIVE: To further define the immunological tissular modifications in premature ovarian failure (POF).
METHOD: The patient was followed up for premature ovarian failure and mild endometriosis associated with serum antiovarian antibodies. A laparoscopic ovarian biopsy was decided on to analyze the tissue and support the onset of immunosuppressive therapy. Immunohistochemistry was performed using monoclonal antibodies directed against T cell membrane markers, as well as activation molecules, to define the composition of the cellular infiltrate and the consequences on ovarian tissue.
