Open reduction and internal fixation of an acetabular fracture during pregnancy.

Fractures of the pelvis and acetabulum are common injuries in a mechanized society and frequently occur in the younger age groups. Commonly associated injuries such as hemorrhage, urethral tears, gastrointestinal injuries, and major vessel damage are well reported and well known to orthopaedic surgeons. Another less frequently reported complication of pelvic fractures is the potential association with pregnancy and subsequent childbirth. This problem was first described by Malgaigne in 1857 in his classic article on double vertical fractures of the pelvis, in which he described a 34-year-old w

Early hip survival after open reduction internal fixation of acetabular fracture.

PURPOSE: To estimate survival of acetabular fracture repair by tracking patients across healthcare encounters. We hypothesized that hip survival estimated this way would be lower than reported by single-surgeon or single-center series not capturing censored reoperations. METHODS: Retrospective health insurance administrative database cohort study. All claimed healthcare encounters for employer-sponsored health insurance beneficiaries aged 18-65 years without pre-existing hip pathology with a newly diagnosed acetabular fracture were identified between October 1, 2015, through December 31, 2018

Bicondylar Hoffa fracture: open reduction internal fixation using the swashbuckler approach.

Coronal plane fractures of the femoral condyle are infrequent injuries and are often missed. Unilateral bicondylar coronal plane fractures are even rarer, with only eight reported cases in scientific literature, and are often associated with other injuries. We present here a case of unilateral bicondylar Hoffa fracture that presented in our emergency department and was managed with open reduction and internal fixation by lateral parapatellar arthrotomy using the swashbuckler approach with satisfactory results.

Femoral artery intimal injury following open reduction of an acetabular fracture.

Vascular injury is a devastating complication of internal fixation of acetabular fractures. While lacerations of the large vessels may lead to massive bleeding, they are obvious and are addressed immediately during the surgery. We encountered a subtle case of intimal injury and thrombosis of the femoral artery. A 20-year-old man presented with a fractured right acetabulum and right fourth, fifth, and sixth ribs with hemopneumothorax after a high-velocity motor vehicle accident. Twelve days after the injury, open reduction and internal fixation of the acetabular fracture from an ilioinguinal ap

[Surgical techniques of open reduction and internal fixation for ankle fracture].

OBJECTIVE: To evaluate the surgical techniques of open reduction and internal fixation performed for ankle fracture retrospectively and the corresponding clinical outcomes. METHODS: From March 2001 to January 2006, 242 patients with ankle fracture were treated. A total of 85 patients with complete clinical data were analyzed. According to the Lauge-Hansen classification system, the fractures were classified into pronation-external rotation (grade II) in 12 cases and grade IV in 9 cases, supination-external rotation (grade II) in 34 cases and grade IV in 16 cases, supination-adduction (grade I

Open vs closed approach to the nasal pyramid for fracture reduction.

OBJECTIVE: To compare an open vs a closed approach to the nasal pyramid for the initial repair of nasal fractures. METHODS: Retrospective medical record review of 49 patients with acute nasal fractures treated by a single surgeon during a 5-year period. Patients underwent a closed approach to the nasal pyramid (Boise elevator only) or an open approach using rhinoplasty techniques, including rasping, osteotomies, and cartilaginous resection or augmentation. Patients were further categorized based on whether septoplasty was performed. The primary outcome measure was the revision rate (RR). RES

Open reduction and temporary rigid internal fixation of Lisfranc fracture-dislocations.

AIM: To review the results of open reduction and temporary rigid screw fixation of Lisfranc fracture-dislocations performed in Singapore General Hospital (SGH) from 1996 to 1998. METHODS: A retrospective review of II cases of Lisfranc injuries treated with temporary rigid internal screw fixation with a minimum follow up of six months was carried out. Radiological assessment was made at follow up to ascertain the anatomical reduction achieved and complications such as post-traumatic arthritis. In addition, the patients were evaluated with a functional assessment with relation to pain, interfer

Transscaphoid perilunate fracture-dislocation. Result of open reduction and pin fixation.

Sixteen patients with 17 transscaphoid perilunate fracture-dislocations were treated by open reduction and pin fixation. Open reduction was performed through a volar approach in only one wrist, a dorsal approach in nine wrists, and combined volar and dorsal approaches in seven wrists. Primary bone grafting was performed in four patients. The follow-up period was from five months to eleven years and two months, with an average of 2.69 years. Seven patients had median nerve injury and all recovered. Of the 17 wrists, the scaphoid fracture healed in 15. This injury should be treated by early open

Open Reduction Techniques for Supracondylar Humerus Fractures in Children.

Supracondylar humerus fractures are the most common elbow fractures in children. Displaced supracondylar humerus fractures that are associated with neurologic and/or vascular injuries should be treated with timely reduction via closed techniques. If closed reduction fails, reduction via open techniques is indicated. There is controversy about which surgical approach yields the best cosmetic and functional outcomes while minimizing postoperative complications. Open reduction, if indicated, has been reported to yield good outcomes in patients in whom closed reduction fails.

Open versus closed reduction: comminuted mandibular fractures.

PURPOSE: The purpose of the study was to review the literature regarding the evolution of current thoughts on management of comminuted mandibular fractures (CMFs). METHODS: An electronic search in PubMed was undertaken in May 2012. The titles and abstracts from these results were read to identify studies within the selection criteria. Eligibility criteria included studies published in English or German reporting clinical series of CMFs. RESULTS: The search strategy initially identified 409 studies. Fifteen studies were identified without repetition within the selection criteria. One case rep

Open reduction and internal fixation of radial head fractures.

BACKGROUND: We evaluated the radiologic and functional outcomes of patients with radial head fractures managed with open reduction and internal fixation. METHODS: Between 1998-2003, 15 patients (7 males, 8 females; mean age 34.1; range 18 to 49 years) with radial head fracture were treated with open reduction and internal fixation. Follow-up time was 54.6 months (42-78). Three fractures were Mason type II, 8 were III and 4 were IV. They were evaluated by anteroposterior and lateral radiographs and functionally by Broberg and Morrey criteria. RESULTS: All the fractures except in 1 patient wit

Supracondylar fractures of the elbow: open reduction, internal fixation.

This principle-based approach for fixing distal humerus fractures has many advantages. Complex fractures are able to be fixed with sufficient stability to permit immediate intensive rehabilitation. Some fractures believed to be unfixable have been fixed satisfactorily by applying the principles outlined in this article. More straightforward fractures are fixed easily using the same techniques. In the author's experience, the stability achieved with this approach is so much greater than that with traditional methods of fixing distal humerus fractures that bone graft has been required only rarel

Open reduction and internal fixation of fractures of the radial head.

BACKGROUND: The purpose of this retrospective study was to analyze the functional results following open reduction and internal fixation of fractures of the radial head and to determine which fracture patterns are most amenable to this treatment. METHODS: Fifty-six patients in whom an intra-articular fracture of the radial head had been treated with open reduction and internal fixation were evaluated at an average of forty-eight months after injury. Thirty patients had a Mason Type-2 (partial articular) fracture, and twenty-six had a Mason Type-3 (complete articular) fracture. Twenty-seven of

Open reduction and internal fixation of tibial pilon fractures.

Although it is evident that the fracture of the tibial plafond is a complex, often debilitating injury, its management is not clear. These injuries generally fall into one of two categories. The low-energy, rotational type of fracture has been shown to have excellent clinical and functional results with open reduction and internal fixation. The high-energy, compression type of fracture has had uniformly moderate results and historically high complication rates. Some authors think that bridging external fixation with or without limited internal fixation should be employed in high-energy fractur

Open reduction and internal fixation of forearm fractures in children.

We retrospectively reviewed 16 children younger than 13 years with 17 fractures of the shafts of the radius or ulna or both who had undergone an open reduction-internal fixation (ORIF). ORIF was performed when a closed reduction was deemed unacceptable in 14 radius fractures and for three unstable open fractures of the radius. The average age was 9.4 +/- 2.3 years (range, 5.0-12.5). Of the 14 fractures with an unacceptable closed reduction, soft-tissue interposition was encountered in seven. Fixation was secured by plates and screws, percutaneous Steinmann pins, or intramedullary Steinmann pin
