We evaluated whether the rate of occurrence of glaucomatous disk damage in eyes with an optic disk hemorrhage differed from the rate of occurrence in control eyes. We compared 24 patients with a disk hemorrhage to 24 control patients, who had been matched for diagnosis, disk appearance, and visual fields. Cup volume and cup-to-disk area ratio of the optic disk were measured using the stereophotogrammetric technique, and area of pallor was measured using computerized image analysis. We found that diabetes increased the probability that a patient would have a disk hemorrhage, and that a hemorrha…

INTRODUCTION: Optic disk pit (ODP) is a rare congenital abnormality of the optic nerve head and can lead to visual impairment when macular detachment occurred. Recent advances in retinal imaging brought new insights into the pathophysiology of the disease, while new therapeutic options have been also described.
METHODS: We made a comprehensive search of the literature regarding the current treatment modalities for the treatment of ODP maculopathy.
RESULTS: Although there have been reported some cases of spontaneous resolution, current management of ODP maculopathy involves several surgical a…

Optic disk hemorrhage occurs in all age groups from neonates to the elderly. Optic disk hemorrhage is best known for its association with visual field loss and progression in patients with glaucoma; however, it may occur in conjunction with other ocular or systemic conditions as well as in healthy individuals. It may also be the first sign of a sight-threatening condition. Variations in the shape, location, and size of the optic disk hemorrhage, as well as associated ocular and systemic signs or symptoms, may help determine the underlying pathology. We address the epidemiology, demographics, p…

A 13-year-old boy presented with acute loss of vision in his right eye of 2 weeks' duration. He had a high fever and was ill for several days, then improved but suffered recurrent episodes of sweating and a high fever. Ophthalmoscopy of the right eye showed optic disk edema, mild vitreous cells, and minimal exudates in the macula. Bartonella henselae titers were positive. A diagnosis of optic disk edema with a macular star secondary to cat-scratch disease was made. The patient was treated with doxycycline and made a dramatic improvement to visual acuity of 20/30 with a minimal residual relativ…

Optic disk edema with a macular star is a descriptive term encompassing a heterogeneous group of disorders. The clinical features include sudden visual loss, swelling of the optic disk, peripapillary and macular exudates that may occur in a star pattern, and cells in the vitreous. Herein we describe the clinical features, potential etiologic factors, differential diagnosis, work-up, and natural history of this entity. Although optic disk edema with a macular star is usually idiopathic, infectious causes, especially syphilis, Lyme disease, cat-scratch disease, and toxoplasmosis, should be consi…

An unusual case of melanocytoma in the optic disk was reported. At the initial examination, unilateral papilledema and irregular concentric contraction of the visual field with fair vision were observed. In the follow-up study, central retinal artery occlusion occurred and vision deteriorated to blindness. Subsequently, a darkly pigmented lesion appeared and enlarged slowly during the period of 14 years. The pathogenesis of visual disturbance caused by the melanocytoma was discussed.

A 37-year-old black man had a clinically documented case of optic disk neovascularization associated with hemoglobin SC disease. Through medical evaluation, we ruled out other disorders associated with proliferative retinopathy. Extensive areas of retinal nonperfusion were associated with vasoproliferative retinopathy at the junction of the perfused and nonperfused retina; and a delicate network of neovascularization extended from the optic disk into the vitreous gel in one eye.

PURPOSE: To report a case of malignant transformation of an optic disk melanocytoma with a second melanocytoma in the ciliary body.
METHODS: Clinical data including visual acuity, visual fields, color fundus photographs, fluorescein angiogram, and ultrasonogram and histopathologic studies of this case were reviewed.
RESULTS: The right eye of a 65-year-old white woman was diagnosed with melanocytoma of the optic nerve. Four years later, the tumor became significantly larger. The best-corrected visual acuity declined from 20/40 to counting fingers and the size of the tumor increased fourfold i…

We studied the clinical and microscopic appearances of the optic nerve head in squirrel monkeys with optic nerve degeneration produced by optic nerve transection at the orbital apex. The ophthalmoscopic development of optic disk pallor coincided with the loss of nerve fiber bundles and the rearrangement of the remaining disk astrocytes into dense parallel layers across the nerve head. No astrocytic mitoses were observed and the estimated volume of astrocytes increased only slightly from normal. Among the astrocytes in atrophic disks, many capillaries had patent lumens and ultrastructurally nor…

INTRODUCTION: The hemorrhagic complications of the optic disk drusen are rare and interesting because of their functional implications, the difficulty of the differential diagnosis and the uncertain pathogeny.
MATERIAL: and methods: Retrospective study of 6 cases with a complete general and ophthalmological examination, followed between 2 and 9 years.
RESULTS: The average age of the cases was 38.5 years (9-62 years). The visual acuity was slightly affected and reversible in the papillary hemorrhages (1 case), low and partial reversible in the extended hemorrhages (4 cases), severe affected a…

The cilioretinal arteries form a physiologic bypass between the ciliary and retinal circulation. The evaluation of photographs of 163 normal optic nerve heads revealed a significant (p less than 0.001) correlation between the number of cilioretinal arteries and the optic disk and cup size: the larger the disk and cup the more vessels. The vessels' prevalence in all optic disks was 27.0%. It increased from 0% in disks smaller than 2.00 mm2 to 50.0% in optic nerve heads larger than 4.1 mm2. No correlations existed between cilioretinal arteries and refraction, axial length, side, sex and age, acc…

We obtained quantitative measurements of capillary numbers, areas, and diameters in atrophic (pale) and normal primate optic nerve heads. The number of capillaries per square millimeter in pale optic disks was not significantly different from that in normal optic disks. Because the loss of all nerve fibers leads to a 50% decrease in nerve head substance, capillaries must atrophy to maintain a constant relationship between capillary number and tissue volume. The mean size of individual capillaries in atrophic nerve heads was smaller than normal, leading to a decrease of more than 27% in the per…
