Induced hypotension for orthognathic surgery.

Achieving satisfactory hemostasis during orthognathic surgery may be difficult because of the extensive vascularity of facial structures. Hypotensive anesthetic techniques provide clear operative fields by altering regional tissue perfusion through the use of systemic vasodilators, ganglionic blocking agents, and positioning of the patient. Thorough monitoring during surgery, careful selection of patients, and close communication between the surgeon and anesthesiologist permit safe anesthesia, can decrease operating time, and usually obviate the need for transfusions.

Evaluation of postoperative changes in condylar positions after orthognathic surgery using balanced orthognathic surgery system.

BACKGROUND: Many studies on maintaining the condyle in a normal or anatomical position during orthognathic surgery have been conducted to stabilize surgical outcomes and prevent iatrogenic temporomandibular joint complications. The aim of this study is to evaluate the changes in condylar positions after orthognathic surgery using virtual surgical planning via the balanced orthognathic surgery (BOS) system. METHODS: Postoperative changes in condylar position were retrospectively evaluated in 22 condyles of 11 patients with skeletal class III malocclusion who underwent orthognathic surgery usin…

[Orthognathic surgery is a team work!].

Orthodontic-surgical treatment of dentofacial anomalies involves a multidisciplinary approach that demands to be finely coordinated in order to get a good result. Historically, orthognathic surgery stemmed from the close collaboration between the orthodontist and the maxillo-facial surgeon; however thanks to the collaboration of many other specialists along the diagnostic and therapeutic process, orthognathic surgery nowadays reached an unprecedented level of quality, reliability, and safety. We discuss herein the role of the various specialists that may be involved in the treatment of a patie…

[Orthodontics and prosthodontics in orthognathic surgery].

Orthodontics and prosthodontics in orthognathic surgery In complex care, such as for growth disorders and congenital anomalies, multidisciplinary collaboration is essential and begins even before the actual treatment starts. Systematic evaluation of dental, skeletal and soft tissue parameters prevents the correction of one aspect from creating new discrepancies in another area. Through backwards planning, the desired fi nal outcome is fi rst determined collaboratively, after which the treatment process is mapped out step by step in reverse. The process consists of 5 phases: 1. determining desi…

Orthognathic Surgery to Enhance the Smile.

An esthetic smile is an integral feature of beauty. Improvement of the smile can be achieved by a combination of orthognathic surgery, orthodontics, and cosmetic dentistry. Preoperative evaluation serves to address a patient's surgical goals; it allows a surgeon to perform a detailed facial analysis and identify patients who are contraindicated for surgery. LeFort I and bilateral sagittal split osteotomy are performed to minimize the risk of complications. Injuries to the inferior alveolar nerve are the most common complication after orthognathic surgery, in which 90% of patients experience tr…

Applications of navigation for orthognathic surgery.

Stereotactic surgical navigation has been used in oral and maxillofacial surgery for orbital reconstruction, reduction of facial fractures, localization of foreign bodies, placement of implants, skull base surgery, tumor removal, temporomandibular joint surgery, and orthognathic surgery. The primary goals in adopting intraoperative navigation into these different surgeries were to define and localize operative anatomy, to localize implant position, and to orient the surgical wound. Navigation can optimize the functional and esthetic outcomes in patients with dentofacial deformities by identify…

[A multidisciplinary approach to orthognathic surgery].

Orthodontists are precisely aware of their therapeutic limits in dealing with maxillofacial dysmorphia. Unstable results and iatrogenic lesions caused by alveolo-dental compensations for displacements of the skeletal base have convinced them of the importance of orthognathic surgery as a complement to their orthodontic treatments. Collaboration between the orthodontist and the maxillofacial surgeon begins as of the moment a displacement of the skeletal bases is established through clinical and cephalometric diagnosis. Due to factors related to their age and dental history, it is necessary to c…

Orthognathic patients with nasal deformities: case for simultaneous orthognathic surgery and rhinoplasty.

Orthognathic surgery is a recognised way of correcting dentofacial deformities and it is common practice to treat problems that affect the chin simultaneously, while deferring or not treating nasal deformities. There is inadequate published information about the prevalence of nasal deformities in such patients, and our aim was to remedy this. We retrospectively studied 75 patients with dentofacial deformities to find out if there was an association between nasal and dentofacial abnormalities. Forty-six of the 75 patients (61%) had mild to prominent cosmetic nasal problems, of whom 27 had defor…

[Adjunctive aesthetic procedures in orthognathic surgery].

In addition to function, occlusion and airway volume, aesthetics play an important role in the planning and execution of orthognathic surgery. To improve the aesthetic outcome of orthognathic surgery, there are several procedures that can be combined with orthognathic surgery secondarily, or in the same operation. These include: lipofilling, dermal fillers, facial implants, liposuction, ear corrections, eyelid corrections, or a lip lift. Patients often appreciate it when the options for additional procedures are discussed. However, both undertreatment and overtreatment can lead to unfavorable …
