Osteomyelitis of the mandible.

Before application of any cross-sectional imaging modality, the orthopanoramic view is indispensable in assessing the status of dentition, in recognizing direct radiographic signs of osteomyelitis, or in determining the presence of predisposing conditions such as a fracture or systemic bone disease. The orthopanoramic view is the procedure of choice in follow-up examinations in patients who have osteomyelitis. In acute osteomyelitis, the higher sensitivity of MR imaging for detection of intramedullary inflammation advocates this imaging modality for confirmation of the clinical diagnosis. If s…

Osteomyelitis of the patella.

Osteomyelitis of the patella is an uncommon infection that is primarily a disease of childhood. The diagnosis frequently is delayed because of its rarity and variable presentation, which may be acute with systemic signs or insidious with mild local signs. The diagnosis should be considered in patients with persistent peripatellar pain and swelling, cellulitis, septic prepatellar bursitis, and septic arthritis of the knee that does not respond to standard treatment. Treatment is identical to the treatment of osteomyelitis at other locations. The outcome is favorable in the child regarding range…

Diagnosis of osteomyelitis.

The early diagnosis of acute hematogenous osteomyelitis depends on a high index of suspicion whenever the physician is confronted with a child experiencing acute onset of bone pain or limited motion of an extremity, regardless of the presence or absence of signs of infection such as fever, local tenderness, redness, swelling or heat (Table 4). Early diagnosis is aided greatly by the use of plain radiography to exclude other conditions and radionuclide bone scans to detect evidence of inflammation at the site of bone pain. "High-tech" procedures such as CAT and MRI should be reserved for situat…

Overview of osteomyelitis.

Osteomyelitis is becoming a more common infection. This increase has been associated with an increase in the number of orthopaedic surgical procedures and with severe bone trauma. The etiology of osteomyelitis is also changing, with more gram-negative and more polymicrobial infections due to both gram-positive and gram-negative pathogens. Underlying diseases such as diabetes mellitus, peripheral vascular and sickle cell disease are associated with a poor cure rate when treated with antibiotics. The emergence of resistant strains of bacteria during the long-term treatment necessary for osteomye…

The radiology of osteomyelitis.

In the early stages of acute osteomyelitis, bone scanning is especially helpful. It is important to correlate the bone scan with clinical findings because the scan is nonspecific. Radiograms reveal the extent of disease, but may lag behind clinical findings; however, plain film studies may reveal sequestra not seen by other means. Gallium scanning may be of value in osteomyelitis, as may be computerized tomography.

A Case of Sterile Osteomyelitis: Chronic Recurrent Multifocal Osteomyelitis (CRMO).

Chronic recurrent multifocal osteomyelitis (CRMO) is a rarely reported autoimmune inflammatory condition affecting children and young adults. The typical complaint is recurrent severe bone pain at multiple sites; often imaging suggests features of osteomyelitis or malignancy. However, the biopsy is always negative for any growth in culture and histopathology detects only inflammatory cells. This is a diagnosis of exclusion with various diagnostic criteria. We had a 20-year-old male presenting with recurring pain and swelling in the right hand and left foot for more than seven months. Imaging r…

Osteomyelitis in adolescents.

Osteomyelitis in adolescents is a serious disease with the potential for lifelong disability. Microbial invasion of the bone typically results from hematogenous seeding. Staphylococcus aureus is the most common cause of acute hematogenous osteomyelitis. The clinical presentation and management of osteomyelitis in children and adolescents is changing with the emergence of invasive community-acquired methicillin-resistant S aureus infections. Antibiotic treatment should target S aureus including community-acquired methicillin-resistant strains. In some cases, surgical debridement is indicated, e…

Osteomyelitis of the orbit.

Although rare, orbital osteomyelitis secondary to sinusitis can be devastating. Early, aggressive ophthalmologic surgical intervention, as well as otorhinolaryngologic co-management, is necessary to obtain the best outcomes. We present two cases of orbital osteomyelitis. One patient remained infected with Pseudomonas meningitis even after extensive sinus and orbital surgery, rapidly declined, and is now deceased. The other patient, after multiple sinus procedures and a medial orbitotomy, was placed on hyperbaric oxygen and is still undergoing treatment.

Osteomyelitis of the jaws.

Osteomyelitis is described as an inflammation of bone and bone marrow that may develop in the jaws following a chronic odontogenic infection or for a variety of other reasons. This situation may be acute, sub-acute or chronic, resulting in a totally different clinical picture.

[Therapy of osteomyelitis].

Exogenic post-traumatic bone infection has come strongly into focus of attention, as compared to haematogenic cases. Complex prophylaxis against such infection, therefore, is of great importance. The initial phase of bacterial osteomyelitis must be identified and must be followed by immediate action, as early treatment is absolutely essential. That is why the routes of infection, which depend on the extent of injury and on the mode of primary treatment, as well as all areas affected must be fully detected. Substantial therapeutic steps should include radical sanitation, stabilisation (especial…

Emphysematous Osteomyelitis Involving the Spine, a Case of a Rare Form of Osteomyelitis.

Gas-forming bacteria like Staphylococcus aureus (SA), along with the worrisome infections it causes, can lead to a fairly overlooked but lethal complication, called emphysematous osteomyelitis (EO). It is a condition characterized by the infection of bones complicated by the presence of gas-forming organisms. Here, we present a 50-year-old woman with insulin-dependent type 2 diabetes, who presented with cough, high-grade fevers, rigors, chills, and back pain from the last seven days. Laboratory tests revealed elevated inflammatory markers and a hyperosmolar hyperglycemic state. Sputum smear an…

Pyogenic Vertebral Osteomyelitis.

Vertebral osteomyelitis is a general term for a heterogenous group of spine infections involving the vertebral bone, intervertebral discs, and facet joints. These infections can often spread contiguously to adjacent paraspinal structures, including paravertebral muscles and the epidural space and frequently presents with nonspecific and indolent symptoms, which can delay diagnosis. Treatment can be arduous, requiring prolonged courses of antibiotics often combined with surgical debridement. This review focuses primarily on pyogenic vertebral osteomyelitis in adults, reviewing the pathophysiolo…

Tuberculous sternal osteomyelitis.

A 32-year-old female patient with systemic lupus erythematosus presented with 1 month of nocturnal subjective fevers, night sweats, poor appetite, malaise, 8-kg weight loss, and a 6-cm painful sternal mass. She had normal vital signs with a physical examination notable only for the presence of a fluctuating sternal mass. A computed tomographic scan of the thorax showed a 67 × 32 × 27-mm sternal mass associated with severe sternal osteomyelitis (Fig. 1); then a surgical drainage was performed, and abundant caseous material was removed, leaving a penrose drain (Fig. 2). Histologic examination of…
