An early report of exoscope-assisted otologic surgery.

BACKGROUND: Exoscope has been used in otoneurologic surgery in several reports. However, most ear surgeries are performed using either microscope or endoscopy today. The purpose of this study is to present our subjective and objective experience of using this instrument in our institution. METHODS: Sixteen ears with different severity and condition in 15 consecutive patients were enrolled. A questionnaire with 12 questions was sent to different participants, including surgeons, residents, medical students, and scrub nurses in the operation room. The total score and average score of each item

Introducing the High-definition 3D exoscope in ear surgery: preliminary analysis of advantages and limits compared with operative microscope.

PURPOSE: The aim of this study was to evaluate the potential of 3D exoscope (EX) in selected ear procedures assessing if this new technology could be an improvement in the field of ear surgery. METHODS: A case series of consecutive patients surgically treated with a post-auricular approach using EX was retrospectively compared with a similar previous series treated with operating microscope (OM). Patient demographics, indications for surgery, procedure type, complications, operating room setting time (ORst), operative time, adequacy of visualization, image quality, ergonomics aspects, instrum

Early Experience With Telemedicine in Patients Undergoing Otologic/Neurotologic Procedures.

OBJECTIVE: Compare outcomes for patients who were seen via telemedicine visits after otology/neurotology surgical procedures with those who … Eighteen patients were seen in telemedicine visits postoperatively after the following otologic/neurotologic surgical procedures: cochlear implant surgery (10), vestibular schwannoma resection (4), combined middle fossa and transmastoid cerebrospinal fluid (CSF) leak repair (2), middle fossa repair of superior canal dehiscence (1), and stapedectomy (1). All telehealth visits …

Underwater endoscopic ear surgery: A systematic review.

PURPOSE: To evaluate the clinical indications, surgical techniques, audiological outcomes, operative efficiency, and safety of underwater endoscopic ear surgery (UWEES). METHODS: A comprehensive literature search was conducted through 14 May 2025 using PubMed, Google Scholar, and the Cochrane Library. Keywords included "endoscopic ear surgery," "underwater ear surgery," and "endoscopic underwater ear surgery." Original English-language studies involving UWEES in human subjects (≥ 3 patients) were included. Studies using microscopes or conducted in animals were excluded. Data extraction and qu

Surgical consideration and practical difficulty in malignant otitis externa: a systematic review.

PURPOSE: This systematic review aims to explore the role of surgery in managing Malignant Otitis Externa (MOE) by analyzing surgical considerations, treatment duration, responses, and patient outcomes. METHODS: A comprehensive search was conducted across PubMed, Science Direct, Google Scholar, and Cochrane Library databases between 2003 and February 2023. Studies meeting inclusion criteria detailed MOE cases, patient medical profiles, surgical procedures undertaken, and treatment outcomes. Selection criteria emphasized surgical aspects and patient-related outcomes in MOE. RESULTS: Twenty-two

Consensus on treatment of obstructive Eustachian tube dysfunction with balloon Eustachian tuboplasty.

OBJECTIVE: There is a great variability in diagnosis of obstructive Eustachian tube dysfunction and its treatment by balloon Eustachian tuboplasty (BET). The aim of this paper was to present a consensus on indications, contraindications, methodology, complications and results after BET. MATERIAL AND METHODS: We obtained a consensus on BET, after a systematic review of the literature on BET from 1966 to November 2018, using MESH terms «Eustachian tube and (dilation or dysfunction)», including a total of 1.943 papers in Spanish, English, German and French. We selected 139 papers with a relevant

Current evidence for endolymphatic sac surgery in the treatment of Meniere's disease: a systematic review.

This paper aims to identify emerging evidence for endolymphatic sac surgery (ESS) in the treatment of Meniere's disease since the landmark study by Thomsen et al, published in 1998 (conducted from 1981 to 1989). Using the MEDLINE database (PubMed), a systematic review of the literature published from January 1990 to June 2014 was performed. We included all English-language, peer-reviewed randomised controlled trials (RCTs) and controlled studies. Single-arm cohort studies were included if the sample size was ≥ 90 with a response rate > 60%. Altogether, 11 studies fulfilled our inclusion criter

Surgical management of conductive hearing loss in children.

Patients with congenital conductive hearing loss present a surgical challenge. Surgeons must be aware of the potential altered anatomy, in particular the potential risks to the facial nerve and inner ear structures; however, meticulous attention to detail in the preoperative evaluation and in the operating room can minimize these risks. If surgery poses too high a risk to these structures, the chances of success are low, or there is a contraindication to surgery, traditional air-conduction hearing aids or bone-anchored hearing aids should be considered.

The preoperative temporal bone CT: a mnemonic for reducing surgical risk.

Otologic surgery provides safe and effective treatment for a variety of ear and skull base pathologies. However, even routine procedures are not without risk of serious surgical complications. Ear surgery requires sub-millimeter tolerances near a variety of critical structures in a complex anatomic region. As such, preoperative computed tomography (CT) can be helpful to surgeons and radiologists for identifying anatomic variants that can influence surgical technique, and which may predispose patients to surgical complications. However, the analysis and reporting of temporal bone CT is not alwa

[The patulous Eustachian tube syndrome: the current state-of-the-art and an original clinical observation. Second communication].

The choice of the treatment strategy for the patients presenting with the patulous Eustachian tube syndrome constitutes a serious challenge for an otorhinolaryngologist. Despite the considerable progress achieved during the past years in the development of modern pharmacotherapy and functional endoscopic surgery, the specialists often encounter difficulties in the management of the patients suffering from this disease. The present article (continuing the previous communication) contains information concerning the historical and modern approaches to the treatment of the patients with the patulo
