Long-term home oxygen therapy improves survival in patients with chronic obstructive pulmonary disease and persistent, severe hypoxaemia. It is uncertain that this benefit extends to patients with other chronic lung diseases. Oxygen is a treatment for hypoxaemia, not breathlessness. To confirm hypoxaemia, blood gas analysis is recommended before prescribing oxygen. There is limited and conflicting evidence that portable oxygen for exertional use is of benefit to patients with chronic obstructive pulmonary disease who do not have severe hypoxaemia. Laboratory studies show improvements in exerci…

Although part of a common "blood gas" test panel with pH and pCO2, the pO2, %O2Hb, and related parameters are independently used to detect and monitor oxygen deficits from a variety of causes. Measurement of blood gases and cooximetry may be done by laboratory analyzers, point of care testing, noninvasive pulse oximetry, and transcutaneous blood gases. The specimen type and mode of monitoring oxygenation that are chosen may be based on a combination of urgency, practicality, clinical need, and therapeutic objectives. Because oxygen concentrations in blood are extremely labile, there are severa…

The authors present a case of a 60-year-old male patient, previously diagnosed with B-cell chronic lymphocytic leukemia, who was admitted to the Emergency Room with dyspnea. The initial evaluation revealed severe anemia (Hgb = 5.0 g/dL) with hyperleukocytosis (800.000/µL), nearly all of the cells being mature lymphocytes, a normal chest X-ray and a low arterial oxygen saturation (89%; pulse oximetry). After red blood cell transfusion, Hgb values rose (9.0 g/dL) and there was a complete reversion of the dyspnea. Yet, subsequent arterial blood gas analysis, without the administration of suppleme…

While pressurized oxygen in tank form, as well as oxygen concentrators, are ubiquitous in civilian healthcare in developed countries for medical use, there are a number of settings where use of these oxygen delivery platforms is problematic. These settings include but are not limited to combat casualty care and healthcare provided in extreme rural environments in undeveloped countries. Furthermore, there are a number of settings where delivery of oxygen other than the pulmonary route to oxygenate tissues would be of value, including severe lung injury, airway obstruction, and others. This pape…

Hyperbaric oxygen is a treatment in which a patient breathes 100% oxygen intermittently while inside a treatment chamber at a pressure higher than at sea level pressure (ie, >1 atm). In certain circumstances, it represents the primary treatment modality, whereas in others it is an adjunct to surgical or pharmacologic interventions. After reviewing all the scientific evidence available to date, the Undersea and Hyperbaric Medical Society, in its latest publication, Hyperbaric Oxygen Therapy Indications (12th ed.), recommends 13 indications for hyperbaric oxygen therapy. Several of these indicat…

Oxygen therapy is one of the principal non-pharmacologic treatments for severe chronic obstructive pulmonary disease (COPD) patients. Home oxygen therapy(HOT), or long-term oxygen therapy(LTOT) for 15 hours or more per day, can improve the survival rate of severe COPD patients with beneficial effects on hemodynamic state, hematological characteristic, exercise capacity, lung mechanics, and mental state. Oxygen therapy is indicated in cases of severe chronic respiratory failure with PaO2 of 55 Torr or less, or in cases with PaO2 of 60 Torr or less in whom there is remarkable hypoxia during slee…

In critically ill patients, measurement of mixed venous oxygenation is useful as an indicator of the adequacy of tissue oxygenation and as an indirect reflection of cardiac output. The flexible fiberoptic pulmonary artery catheter oximeter permits continuous in vivo measurements of the mixed venous oxygen saturation. However, in some groups of critically ill patients, such as those with adult respiratory distress syndrome or sepsis, measurements of mixed venous oxygen may be misleading and should be interpreted with caution.
