In 9 male patients with psoriasis vulgaris a semen analysis before and during photochemotherapy with 8-methoxypsoralen and UVA (PUVA) was performed to rule out drug-induced toxic damage of spermatogenesis or impairment of fertility due to scrotal hyperthermia. Two hours after oral application of 40--60 mg 8-methoxypsoralen the patients had been irradiated in UVA high intensity treatment units. PUVA-treatments were performed four times weekly until total body clearing was achieved. For complete remission 13--26 (mean 20.5) PUVA-treatments were necessary. PUVA-bath therapy has developed into first line topical PUVA therapy in the treatment of psoriasis. Because of logistical and economic problems, bath PUVA may be difficult to administer. Recently, cream-PUVA therapy has been described as an alternative mode of topical therapy. We treated two patients with moderate plaque-type psoriasis with this new topical approach. 0,0006% 8-methoxypsoralen cream was applied for 1 hour, directly followed by increasing doses of UVA. Up to now, there are only a few data available concerning the influence of bathing time on skin phototoxicity. We compared the erythemal responses of normal skin to bath PUVA with 8-methoxypsoralen (8-MOP) after 5, 10 and 20 min immersion time. Currently, 20 min is the routinely performed immersion time in many European countries, including Germany, while in other countries bathing times are shorter. Using a 0.075% 8-methoxypsoralen solution, minimal phototoxic doses and improvement of psoriasis lesions were investigated in 15 patients suffering from chronic stationary psoriasis. The time interval between topical 8-methoxypsoralen application and long-wave ultraviolet light irradiation was increased stepwise. Report about a 44-year-old psoriatic female who developed recurrent bullous eruptions on her extremities during oral photochemotherapy. Ultrastructural analysis revealed an epidermolytic blister formation due to cytolytic alteration of infranuclear parts of basal cells. Pathogenetically, an initial subclinical phototoxic alteration of the epidermodermal coherence is assumed which may lead to a visible bulla by an additional mechanical trauma. There was no evidence for a subclinical bullous pemphigoid.

A patient with psoriasis vulgaris developed verruciform xanthoma (VX) on the scrotum during psoralen photochemotherapy (PUVA). Although it is uncertain whether VX was induced by PUVA therapy, we report the first case of VX which appeared in a patient with psoriasis during PUVA therapy. We speculate that UV light may be one of the etiologic factors triggering VX in this case.
