Unilateral papilledema in pseudotumor cerebri.

PURPOSE: To report a case of a 25-year-old girl with pseudotumor cerebri who presented with unilateral swollen optic disk. METHODS: A 25-ye … eadache improved. CONCLUSION: Although rare, unilateral swollen disk could be a sign of unilateral papilledema due to increased intracranial pressure.

Therapy-resistant papilledema in achondroplasia.

A 20-year-old achondroplastic dwarf developed severe papilledema over a short period of time. Since the age of 3 he had a ventriculoperitoneal shunt for presumed hydrocephalus. Severe ventriculomegaly was never documented, neither in the past, nor at presentation. Intracranial pressure was high, and the shunt was obliterated. Despite prompt shunt revision and subsequent bilateral optic nerve sheath decompression the patient developed postpapilledema optic atrophy with bilateral blindness. The pathophysiology of increased intracranial pressure in achondroplasia is discussed.

Dural arteriovenous malformations and papilledema.

We examined and managed two patients with posterior fossa dural arteriovenous malformations (DAVMs) and papilledema. Both DAVMs had venous drainage into the transverse, straight, and sigmoid dural venous sinuses. The mechanism of papilledema in the first case was presumed venous hypertension resulting in impaired cerebrospinal fluid absorption, as the malformation drained into the single transverse sinus. This was cured by selective arterial embolization of the causative DAVM. The second patient had venous sinus thrombosis that impaired venous drainage despite embolization. A lumboperitoneal s…

Papilledema after acute head injury.

Low grade papilledema after acute, severe head injury was identified in 15 (3.5%) of 426 patients. Papilledema was recognized immediately after head injury in 1 patient, during the 1st week in 10 patients, and in the 2nd week or after in 4 patients. Initial computed tomographic scans showed evidence of brain injury in 11 of these patients. The intracranial pressure (ICP) was monitored continuously for 3 or more days in 9 patients; it was mildly elevated (20 to 40 mm Hg) in 7 patients and moderately elevated (40 to 60 mm Hg) in 2 patients. Intracranial hypertension was controllable in each pati…

Papilledema in paroxysmal nocturnal hemoglobinuria.

A 26-year-old man with the diagnosis of paroxysmal nocturnal hemoglobinuria presented with papilledema due to increased intracranial pressure without localizing neurological signs. It is concluded that papilledema and benign intracranial hypertension could result from a cerebral vein thrombosis which can complicate the course of paroxysmal nocturnal hemoglobinuria. This has not been a well-recognized feature of the disease.

Diagnostic dilemma of papilledema and pseudopapilledema.

BACKGROUND: Papilledema is the optic disc swelling caused by increased intracranial pressure (ICP) that can damage the optic nerve and cause subsequent vision loss. Pseudopapilledema refers to optic disc elevation without peripapillary fluid that can arise from several optic disc disorders, with optic disc drusen (ODD) being the most frequent cause. Occasionally, pseudopapilledema patients are mistakenly diagnosed as papilledema, leading to the possibility of unneeded procedures. We aim to thoroughly examine the most current evidence on papilledema and pseudopapilledema causes and several meth…

[Etiologies of papilledema in Cameroonian hospitals].

INTRODUCTION: papilledema refers to the swelling of the head of the optic nerve, a major sign of many local, locoregional or systemic pathologies that may involve patients' visual or vital prognosis. This condition represents a diagnostic and therapeutic emergency frequently encountered in our facilities. Therefore, it is deemed and opportune to identify the most common etiologies observed in Cameroonian hospitals. METHOD: we conducted a documentary and descriptive study of patients with papilledema presenting to the Ophthalmology Department of the Hospital of Instruction, Application and Ref…

Papilledema and Anemia: A Rare Association.

Papilledema is the swelling of the optic disc due to the transmission of intracranial pressure through the optic nerve. It can occur as a symptom of various intracranial pathologies that elevate the intracranial pressure. Even though anemia has been described as an association with raised intracranial pressure, the exact causal relationship between the two has not been well established in the literature. We present one such unusual case wherein a 21-year-old female patient, who complained of headaches, was found to have papilledema and severe anemia. After an unyielding workup for secondary ca…

[Hydrocephalus and papilledema in caudal tumors].

Another two cases of hydrocephalus and papilledema with cauda tumours are communicated and a multifactorial pathomechanism is discussed. The following recommendations are given to avoid diagnostic and therapeutical mistakes due to an apparently cerebral symptomatology with cauda tumours and only a mild local finding: I. A thorough investigation of the history.--2. An impartial complete neurological examination omitting possible emphasis indicated by the first examiner.--3. A complex diagnostic intergration of single findings (native x-ray pictures, findings of the cerebrospinal fluid) in keepi…

Cysticercosis: an unusual cause of papilledema.

A case of cerebral cysticercosis with papilledema is presented in a 44-year-old Mexican woman who progressed to optic atrophy despite a ventriculoatrial shunt. The diagnosis was made on testing the cerebral spinal fluid for the indirect hemagglutination response to cysticercosis. The problems of the pork tape-worm as well as several references from the literature are discussed pertaining to ocular and cerebral involvement. Although this disease is not common in the United States we should think of it in cases of papilledema in patients from endemic areas and in those who have traveled in those…

[Papilledema with leukemia and similar processes].

In 11 cases of leukaemia or other lymphatic system disease meningeal affection occurred together with an oedema of optic papilla. In 6 patients (9 eyes) visual accuity fell down till 0.1 or worse. By irradiation of retrobulbar spaces in 4 of 5 cases a marked visual accuity improvement was obtained. In 2 other patients an infiltration of optic nerve was found only post mortem through histological examination. In those patients visual accuity was not diminished.

Identifying biomarkers for papilledema and pseudopapilledema.

Papilledema describes optic nerve head (ONH) swelling due to raised intracranial pressure which can lead to complications including visual loss. Differentiating it from pseudopapilledema is often challenging and may require invasive investigations. Optical Coherence Tomography (OCT) is a non-invasive imaging modality, allowing visualisation of the ONH. We identified OCT-derived biomarkers for papilledema and pseudopapilledema in the existing literature. 213 patients with confirmed papilledema, optic disc drusen (ODD), tilted optic discs (TOD) or crowded optic discs (COD) were included. OCT sca…

[Papilledema as a sign of intracranial hypertension].

Papilledema (choked disk) is a sign of intracranial hypertension (ICH) - condition that presents danger not only for patient's vision, but also for their life. Despite the fact that ICH is usually a neurosurgical pathology, sometimes an ophthalmologist is the first doctor such patients visit, most often in a primary healthcare clinic. At the same time, as practice shows, not all ophthalmologists are well aware about in this pathology; difficulties occur in differential diagnosis of papilledema against similar changes of the optic nerve head seen during ophthalmoscopic examination. This article…

Papilledema as the Sole Manifestation of Neuroborreliosis.

Lyme disease is reportable, and approximately 30,000 cases/year are notified to the local and state health departments. However, other estimates based on insurance records suggest there are close to 476,000 cases/year that are diagnosed and treated. In addition to a large burden of illness, areas where Lyme disease is common are expanding. Therefore, clinicians should be aware of uncommon presentations of this condition. We describe the case of a 5-year-old girl who presented with papilledema as an isolated manifestation of Lyme disease. Of note, her ocular symptoms were intermittent and worse…
