INTRODUCTION: Serum lipid profiles play a crucial role in diagnosing and evaluating cardiovascular diseases. However, the presence of paraprotein can lead to inaccurate dyslipidemia results on automated analyzers.
CASE REPORT: A 65-year-old woman whose combined concentrations of HDL-cholesterol (HDL-C) and LDL-cholesterol (LDL-C) consistently surpassed her total serum cholesterol levels over a period of three months presented with unusual lipid component detection. Further analysis revealed the presence of a monoclonal paraprotein, identified as an IgMλ band, with a concentration of 28.0 g/L.

BACKGROUND: To explore the reason and treating method for the negative value of uric acid in a patient patient.
METHODS: The serum of a patient with a negative uric acid detection value was diluted 5 times with "sterilized water" and with "normal saline". Additionally, the serum was treated with PEG6000 protein, and uric acid was detected.
RESULTS: Elevated IgM paraprotein in the patient resulted in abnormal uric acid response curves in serum samples or diluted serum samples. After the serum was precipitated with PEG600, the uric acid response curve was basically normal.

This brief report describes 2 cases in which paraprotein caused spurious biochemistry results. In both cases standard laboratory measurements of calcium indicated the patients were hypercalcaemic and had significantly raised Vitamin D concentrations in serum. However, use of alternative measurements of calcium and vitamin D showed that these results were spurious. A 67 years old woman with a Waldenström disease was admitted in the intensive care unit for dyspnea and fever. Antigenic targets of monoclonal gammopathy of undetermined significance (MGUS) and multiple myeloma (MM) paraproteins might play a role in the pathogenesis of these neoplasms. METHODS: We reported a case of a 71-year-old male with elevated creatinine, splenomegaly, monoclonal IgA, and MYD88 mutation. Only monoclonal plasma cells were detected first, leading to a misdiagnosis of multiple myeloma. When progressive spleen enlargement was observed, re-evaluation revealed the emergence of monoclonal lymphocytes and the diagnosis was revised to LPL.

We identified 382 consecutive patients with lymphoid neoplasms associated with serum monoclonal IgM paraprotein and classified each neoplasm according to World Health Organization criteria.
