How Does Isolated Medial Patellofemoral Ligament Reconstruction Influence Patellar Height?

BACKGROUND: Reconstruction of the medial patellofemoral ligament (MPFL) is the gold standard treatment for recurrent patellar dislocation. Patella alta has been reported in about half of patients with recurrent patellofemoral instability. HYPOTHESIS: MPFL reconstruction (MPFLr) has a beneficial role in the correction of patellar height in patients with mild patella alta (Caton-Deschamps index [CDI] between 1.20 and 1.40). STUDY DESIGN: Case series; Level of evidence, 4. METHODS: Skeletally mature patients, with no history of previous or concomitant knee surgical procedures, who underwent is

Axial patellar engagement index and patellar tilt after medial patello-femoral ligament reconstruction in children and adolescents.

BACKGROUND: The medial patello-femoral ligament (MPFL) is a major patellar stabiliser whose reconstruction in adults involves graft fixation within a femoral tunnel. In skeletally immature patients, in contrast, the graft is fixed to the soft tissues to allow normal growth. The primary objective of this prospective study was to perform computed tomography (CT) and magnetic resonance imaging (MRI) assessments of medium-term correction of patellar tilt and of the axial patellar engagement index (AEI) after a paediatric variant of MPFL reconstruction in skeletally immature patients. HYPOTHESIS:

Backfilling Patellar and Tibial Harvest Sites Using Autologous Bone Reamings and Anterior Cruciate Ligament Remnant Tissue During Bone-Patellar Tendon-Bone Anterior Cruciate Ligament Reconstruction.

The bone-patellar tendon-bone autograft remains a common and often first-choice option for anterior cruciate ligament reconstruction. Nevertheless, a subset of patients suffer from chronic anterior knee pain at the site of graft harvest. Although prior literature has indicated that backfilling of the patellar and tibial defects decreases the risk of donor site pain, sourcing and harvesting of the graft material, as well as the management of the paratenon and underlying tendon defect, remains variable. This technical note and associated video describe our preferred technique for the backfilling

Indirect forces and patella fracture after anterior cruciate ligament reconstruction with the patellar ligament. Case report.

Patella fracture is either an infrequent or underreported complication after anterior cruciate ligament (ACL) reconstruction using a portion of the patellar ligament. Patella fracture pattern has been related to the mechanism of injury. Direct fractures resulting from impaction forces delivered to the patella are typically stellate or comminuted. Indirect fractures resulting from tensile stresses applied across the extensor mechanism typically are displaced transverse disruptions. After ACL reconstruction using a portion of the patellar ligament, Y-shaped stellate fractures can occur. Fracture…

[Evaluation of results after arthroscopic reconstruction of the anterior cruciate ligament with a free graft of the patellar ligament].

From among 312 patients treated with arthroscopic anterior cruciate ligament reconstruction (one-incision, transtibial with the use of ipsilateral bone-patellar tendon-bone autograft) between 1992 and 1994 256 (82%) were reviewed with a mean follow-up of 26.4 months. An aggressive rehabilitative protocol advocated by Shelbourne has been employed. Stability of the knee has been assessed with KT-1000 arthrometer and IKDC and OAK forms served to evaluate the results. In 77% of the cases nearly normal function of the knee has been restored (87% excellent and good results in OAK form). Arthroscopic

Results of use of the central one-third of the patellar ligament to compensate for anterior cruciate ligament deficiency.

In a consecutive series of 91 patients with 92 injured knees in which the anterior cruciate ligament was irrevocably destroyed, 79 were males; 12 were females. After a 7-year interval one male patient had surgery on his other knee. Twenty-five of the knees demonstrated isolated injuries to the anterior cruciate ligament, but the incidence of this lesion may be even greater than recorded here. The anterior cruciate ligament was repaired by a patella ligament reconstruction along with other surgical procedures when indicated. There is no acceptable alternative treatment for surgical repair, or s

The equine patellar ligaments and the infrapatellar fat pad - a microanatomical study.

BACKGROUND: Interpretation of patellar ligament (PL) ultrasonography may be difficult, as hypoechoic or heterogenous echogenicity are common findings. Verifying suspected disease of equine PLs by histopathology is also problematic as descriptions of normal PL vascularity and histology are scarce. The current study describes the PL and infrapatellar fat pad (IFP) vascular pattern from computed tomography scans of barium perfused normal equine specimens (n = 8; age 10 days to 18 years), as well as routine histology to serve as a reference for future investigations into PL pathology and IFP disea…

[Rupture of the patellar ligament one month after treatment with fluoroquinolone].

Musculoskeletal disorders including arthralgia and myalgia are adverse effect common to all fluoroquinolones. We report a case of spontaneous rupture of the patellar ligament in a 37-year-old man participating in leisure sports which occurred one month after a three-week course of Ciprofloxacine(R). Several cases of tendon ruptures have been reported in the literature, including the patellar tendon. Unusual features in our case were the one-month delay before tendon rupture and the absence of inaugural signs. We reviewed the pathophysiological mechanisms leading to fluoroquinolone-related tend…

Classification of lesions of the medial patello-femoral ligament in patellar dislocation.

The remnants of the medial patello-femoral ligament (MPFL) of 67 knees, 18 with acute patellar dislocation and 49 with chronic patellar dislocation, were studied. The MPFL injuries of the acute cases were categorised into 2 groups: an avulsion tear type and a substantial tear type. The chronic cases were put into 3 groups: those with loose femoral attachment (9 knees), those with scar tissue formation or abnormal scar branch formation (29 knees), and those with no evidence or continuity of the ligament (absent type) (11 knees). It is concluded that incompetence of the medial patello-femoral li

[Enthesopathy of the patellar ligament in weight lifters: a new therapeutic procedure.].

The insertion tendinitis of the proximal part of the patellar tendon frequently occurs in weight-lifters. In a prospective non-randomized study soft-laserlight (helium/neon-laser, wave length: 632.8 nm, power: 5 mW) was applicated on standardized acupuncture points on 21 knee joints of 15 performance athletes. Pain reduction during sports activity was obtained in 85.7% after the end of therapy and in 81 % in a three months follow-up. Local tenderness improved in 57.1 %. Key words: patellar tendinitis, jumper's knee, soft laser, laser acupuncture, weight-lifting.

[Treatment of old or recurrent ruptures of the patellar ligament by contralateral autograft].

Patellar ligament ruptures are difficult to treat, and require transfer of good quality tissues for an effective reconstruction. In this study the contralateral patellar ligament was chosen in preference to other types of graft. A graft was taken from the opposite knee (length 16 cm, width 8 mm) composed of a block of tibial bone, middle third of patellar ligament, block of patella, and quadriceps tendon. The bone blocks were embedded in slots made on the anterior surface of the patella and the tibia. The method of fixation allowed exact positioning as regards the patella height, and was suffi…

Association Between Preoperative Patellar Jumping Sign and Graft Failure After Medial Patellofemoral Ligament Reconstruction for Recurrent Patellar Dislocation.

BACKGROUND: Two recent studies have shown that patients with jumping sign exhibit more pronounced lower limb torsional abnormalities and trochlear bump in recurrent patellar dislocation (RPD), suggesting that medial patellofemoral ligament (MPFL) reconstruction may not fully correct the patellofemoral relationship in patients with a jumping sign. PURPOSE: To determine whether there was an association between the preoperative jumping sign and MPFL graft failure. STUDY DESIGN: Case-control study; Level of evidence, 3. METHODS: Patients who underwent primary surgery and follow-up evaluation du

[Late patellar tendon rupture 10 years after anterior cruciate ligament reconstruction using a bone-patellar tendon-bone graft].

Autologous transplantation of the central third of the patella tendon with a bone-patellar tendon-bone (BTB) graft is one of the most commonly used techniques for anterior cruciate ligament (ACL) reconstruction. Frequently chosen alternative sources include semitendinosus and gracilis tendon autografts. The differences of opinion regarding graft sources mainly result from comparison of outcome and complications. Although higher donor site morbidity and postoperative extensor mechanism complications are postulated for bone-patellar tendon-bone grafts, patellar tendon ruptures following anterior

Morphology of insertion sites on patellar side of medial patellofemoral ligament.

PURPOSE: The purpose of this study was to clarify the insertion sites on the patellar side of the medial patellofemoral ligament (MPFL). METHODS: A total of 35 nonpaired human cadaveric knees were used in this study. After identification of the MPFL, the insertion sites on the patellar side of the MPFL were marked. Three-dimensional images were created, and the location and morphology of these insertion sites were analysed. RESULTS: The morphology of the insertion sites on the patellar side of the MPFL was consistent. The proximal fibres of the MPFL were inserted to the deep fascia of the va

The patellar insertion of the medial patellofemoral ligament in children: a cadaveric study.

BACKGROUND: The purpose of this study was to evaluate the medial patellofemoral ligament (MPFL) patellar insertion in skeletally immature anatomic specimens. METHODS: Nine pediatric cadaveric knee specimens were examined through gross dissection. Metallic markers were placed at the MPFL patellar insertion footprint. Computed tomographic scans for each specimen were analyzed. The MPFL insertion footprint width, patellar height, and patellar width were measured. The distance from the MPFL insertion footprint center to the midline of the patella was assessed. The proportion of the patella that t

[Fractures of the tibial tuberosity associated with avulsion of the patellar ligament in adolescents].

PURPOSE OF THE STUDY: Avulsion fracture of the tibial tuberosity is considered as a relatively uncommon adolescent injury. The avulsion of the patellar ligament may occur as part of the injury. Two cases of sixteen year old boys with this kind of injury have been reported. Fractures are described using Ogden et al. classification : type I is an avulsion fracture of the most distal portion of the ossification center of the tubercle ; type II, an upward angulation of the lip fragment ; type III, a fracture of the lip fragment with propagation of the fracture-line into the knee joint. Each type i…

Prediction of Contralateral Patellar Instability After Ipsilateral Medial Patellofemoral Ligament Reconstruction.

BACKGROUND: About 10% to 16% of patients with patellar instability have bilateral involvement. The risk factors for future contralateral patellar instability in patients who have ipsilateral patellar instability are not known. Knowledge related to the possibility of future contralateral patellar instability would be helpful, as previous studies have shown suboptimal outcomes and increased complication rates in patients with bilateral patellar instability. PURPOSE: The purpose of the current study was to identify the risk factors for future contralateral patellar instability and develop a pred
