Delayed neuronal death following perinatal asphyxia in rat.

The consequences of perinatal asphyxia on the rat brain were studied 80 min to 8 days after birth with hematoxylin-eosin and in situ DNA double-strand-breaks labeling histochemistry. Asphyxia was induced by immersing fetus-containing uterus horns, removed from ready-to-deliver Sprague-Dawley rats, in a water bath at 37 degrees C for various time periods (0-22 min). Spontaneous- and cesarean-delivered pups were used as controls. Perinatal asphyxia led to a decrease in the rate of survival, depending upon the length of the insult. No gross morphological changes could be seen in the brain of eith

Infections, preterm delivery, and perinatal death in midgestation.

Amniotic fluid infection results in considerable pregnancy wastage in the second and early third trimesters. In our study of 1119 perinatal autopsies amniotic sac infection was associated with death in 133 fetuses and neonates in midgestation. These cases accounted for almost 12% of all perinatal autopsies and for 20% of all perinatal autopsies in midgestation at Women and Infants Hospital during the past decade. Gram-positive organisms, especially coagulase negative staphylococcus, group B streptococcus and Streptococcus viridans, were the most common pathogens isolated.

Abruptio placentae and perinatal death: a prospective study.

Abruptio placentae caused 3.96 perinatal deaths per 1,000 births in a large prospective study. Intrapartum but not prepartum maternal hypertension was observed in the fatal cases. Decidual necrosis at the placental margin and large placental infarcts were the most characteristic placental abnormalities. The decidual necrosis was correlated with maternal cigarette smoking and low pregnancy weight gains in the abruption placentae cases. The fetuses and neonates who died had a pattern of growth retardation characteristic of antenatal undernutrition, indicating that poor maternal nutrition during …

The importance of placental evaluation following perinatal death.

Although uncommon, abnormalities of the placenta are important to recognise owing to the potential for both maternal and fetal morbidity and mortality. The placenta is often overlooked in the routine evaluation of a normal gestation, receiving attention only when an abnormality is detected. During the formal scan to confirm a fetal death, the information gathering process to elucidate a possible cause of death begins, yet, even in this instance, the placenta is seldom examined. We aim to draw attention to the importance of placental assessment by presenting a case of a stillbirth where the ans

PERINATAL DEATH. A CLINICOPATHOLOGICAL ANALYSIS OF 99 CASES.

The findings at autopsy in 99 cases of perinatal deaths in a ten-year period and on pathologic examination of the accompanying placentas in 39 cases were analyzed clinicopathologically. Also reviewed were the pathologic diagnoses of 225 placentas that were examined for causes other than neonatal death. Cardiovascular and pulmonary problems, particularly hyaline membrane disease, atelectasis and cardiac anomalies were most common. Asphyxia, heart failure and premature placental separation were the most frequent causes of death. In slightly more than half of the cases a reasonable final diagnosi…

Perinatal mortality audits and reporting of perinatal deaths: systematic review of outcomes and barriers.

BACKGROUND: Perinatal deaths are a devastating experience for all families and healthcare professionals involved. Audit of perinatal mortality (PNM) is essential to better understand the factors associated with perinatal death, to identify key deficiencies in healthcare provision and should be utilised to improve the quality of perinatal care. However, barriers exist to successful audit implementation and few countries have implemented national perinatal audit programs. CONTENT: We searched the PubMed, EMBASE and EBSCO host, including Medline, Academic Search Complete and CINAHL Plus database…

Are some perinatal deaths in immigrant groups linked to suboptimal perinatal care services?

OBJECTIVE: To test the hypothesis that suboptimal factors in perinatal care services resulting in perinatal deaths were more common among immigrant mothers from the Horn of Africa, when compared with Swedish mothers. DESIGN: A perinatal audit, comparing cases of perinatal deaths among children of African immigrants residing in Sweden, with a stratified sample of cases among native Swedish women. POPULATION AND SETTING: Sixty-three cases of perinatal deaths among immigrant east African women delivered in Swedish hospitals in 1990-1996, and 126 cases of perinatal deaths among native Swedish wo…

[Perinatal deaths due to the amniotic fluid embolism].

Evaluation of medical management in cases of deaths of women in childbirth often poses significant problems. Based on the available casuistry of the Forensic Medicine Department of the Medical University of Warsaw, the authors attempted an assessment of medical management based on the analysis of medical records from the period 1999-2007 along with the results of autopsies and histopathological examinations in selected cases of deaths, emphasizing the principles of management and therapeutic possibilities in amniotic fluid embolism.

[Perinatal deaths in the county of Ostfold 1989-97].

All perinatal deaths in the county of Ostfold during the nine year period 1989-1997 were assessed by perinatal audit. Each death was allocated to one of two groups: satisfactory or suboptimal perinatal care. The classification of the causes of death includes fetal, obstetrical and neonatal conditions. The audit is supplemented with some epidemiological data. It was concluded that the standard of care was suboptimal in a fourth of the evaluated cases (52 out of 212). Misdiagnosed or ignored growth retardation and inadequate reaction to information of reduced fetal movements, were the two most f…

Impact of improved perinatal care on the causes of death.

A total of 440 perinatal deaths occurring in a maternity hospital over a 6 year period have been analysed clinically and pathologically. The decline in mortality could be attributed to a reduction in asphyxial deaths, lethal malformations, and macerated stillbirths. The establishment of a neonatal intensive care unit seemed to have been more successful in combating birth asphyxia than respiratory distress syndrome. Although the greatest reduction in perinatal mortality was in babies with birthweights between 1 and 1.5 kg, there was no decline in the deaths from hyaline membrane disease or intr…

Anticipation, Accompaniment, and a Good Death in Perinatal Care.

The ethics of perinatal care, and the experiences of families who receive such care, remains a nascent area of inquiry. It can be hard to see how existing "good death" constructs apply to the experiences of fetal patients and their families. In this paper, we explore two themes raised by a case at our fetal health center: anticipation and accompaniment. In this case, a mother presented to our fetal health center; her unborn son, our fetal patient, was diagnosed with life-threatening hypoplastic left heart syndrome and endocardial fibroelastosis. The parents were told that their son's life expe

Perinatal deaths and insufficient antenatal care in Latvia.

OBJECTIVE: A national audit on perinatal deaths was performed to assess the quality of antenatal care, and to suggest measures for improved antenatal care. MATERIAL AND METHODS: Medical records of all the perinatal deaths in Latvia in the years 1995-1996 have been studied. Non-attenders and attenders of antenatal care were characterized by socio-economic and medical variables: maternal age, parity, history of perinatal outcome, health status and behavioral hazards during the index pregnancy, length of gestation and birth weight. The Nordic-Baltic perinatal death classification was used. RESU…

Perinatal death, the family, and the role of the health professional.

In order to substantially assist a grieving family at the time of perinatal death, the health care team should have sufficient knowledge of the grief reaction, including the duration, maternal/paternal differences, and perinatal psychodynamics. Immediate support includes bereavement counseling. Predelivery care should be provided, if appropriate. The health care team should be prepared for common questions and know about local cultural traditions. Follow-up care after discharge, including anticipatory guidance, is important. The issues of repeat pregnancies and holiday behavior should be addre…

Causes of perinatal death at a Saudi maternity hospital.

The time, cause and avoidability of perinatal deaths were analyzed in infants who were delivered in the main Maternity hospital in Riyadh and who represented a large proportion of all births in the city. The crude perinatal mortality rate was 39.8 per 1000 births. Of all deaths 53 percent occurred either intrapartum or within 24 hours of birth. The mortality rate in this time period was 9 times higher, and the intrapartum mortality rate 16 times higher than the corresponding Swedish rates. The causes of death were classified according to Wigglesworth. Of all deaths, 37 percent were due to asph…

Postdate pregnancies: a review of 46 perinatal deaths.

A review of 46 perinatal deaths was conducted using a 1982 to 1985 regional perinatal network database of 6701 delivered postdate (greater than or equal to 42 weeks gestation) infants. Perinatal mortality (6.9 per 1000 births) increased as gestational age advanced beyond 42 weeks, and was higher in young teenagers or mothers with some additional antenatal complications, such as hypertension. Meconium staining of the amniotic fluid, low Apgar scores, low birthweight, congenital malformations, and neonatal complications, including pneumothorax, meconium aspiration, and seizures, were associated …
