The main objective of this study is to analyze results obtained with hydroxyapatite bone cement (HABC) ossiculoplasties. This is a retrospective study of a case series. This study was conducted in an academic hospital and tertiary referral center. A total of 127 ossiculoplasties using HABC were evaluated. Ears were divided into three groups according to procedure: group 1 involved reinforcement of the incudostapedial joint with cement and reconstruction of an incus long process defect with cement. BACKGROUND: The treatment of major facial clefts has evolved greatly over the past 40 years. Early in the authors' experience, soft tissues were treated by Z-plasty and local flap rotation, resulting in a patchwork effect and noticeable scars. Bony deficits were treated with bone grafts that often failed to restore normal facial contour. In 1985, the authors developed a treatment philosophy to resolve these issues, and its tenets have formed the foundation of treatment since then.

In this Perspective, we summarize some of the most contentious issues surrounding diagnosis and treatment of myeloma. We outline how a fundamental clash of philosophies, cure versus control, may be at the heart of many of the controversies. From the very definition of the disease to risk stratification to the validity of current clinical trial endpoints, we highlight the major areas of debate and provide alternative viewpoints that have implications for trial design and interpretation, as well as clinical practice.

Interaction with local communities is commonly known as academia's "third mission," yet academia-community rifts are still common, running deeper in marginalized communities. Severe periodontitis is a highly prevalent dental disease. With the advent of implant dentistry, teeth are often extracted and replaced. Periodontal surgery, where indicated, could also result in increased trauma to the patient. This literature review discusses different treatment modalities for periodontitis and proposes a treatment approach emphasizing maximum preservation of teeth while minimizing morbidity to the patient. Osteopathic manipulation has been used to treat a wide range of diseases in largely outpatient settings. The authors describe the emergent use of osteopathic manipulative treatment to improve respiratory mechanics in a critically ill patient with acute respiratory failure. High-velocity mobilization of cervical and thoracic dysfunctions resulted in a decreased work of breathing, improved arterial oxygenation, resolution of tachycardia, and an overall improvement in the patient's clinical condition.

Managing the adult dysplastic hip with osteoarthritis is challenging. Adult dysplastic hips have altered bony and soft tissue anatomy. Management strategy depends upon the severity of the dysplasia as assessed using various prevalent classification systems. Differing surgical philosophies have been put forward to address this complex scenario. We describe the management of arthritis after adult developmental dysplasia or the hip using principles and techniques developed and described at our centre.

Noninferiority studies in surgery are, by their very nature, reductionist. They use multiple variables to generate a yes or no answer about the new device being tested. A binary outcome is appropriate for a regulatory agency such as the Food and Drug Administration, but the clinical situation is more nuanced. It is critical to understand the underlying philosophies and choices that go into trial design when a surgeon is recommending a new device.
