UNLABELLED: Plasmapheresis is an effective treatment modality in several immunological kidney diseases. It is also indicated in certain neurological and hematological abnormalities, and some other diseases.
AIMS: In this study the indications and outcomes of the plasma exchange treatments performed in the Plasmapheresis Unit of the authors during the last 12 years are summarized, and the findings are compared to those published in the literature. The procedure, mechanisms of action and adverse effects are also briefly discussed.

This review describes the three ways of plasmapheresis in patients with myasthenia gravis: plasma exchange (PE), double filtration plasmapheresis (DFP) and immunoadsorption (IA). DFP or IA was used for selective removal of antibodies to acetylcholine receptor without replacement of albumin. Plasmapheresis is indicated in patients with myasthenic crisis or before thymectomy. Comparative studies between plasmapheresis and high-dose of intravenous immunoglobulin (IVIG) showed similar transient improvement on myasthenic symptoms up to 4 to 8 weeks. In 1986 an epidemic of HIV infection among paid plasma donors was identified in Mexico; paid donors were iatrogenically infected in a plasmapheresis center. These paid donors sold both plasma and blood: they provided one-third of blood consumed in 1986. This led to infection of blood recipients, mainly women of childbearing age. Blood transfusion is the leading cause of AIDS in women in Mexico. A term baby developed severe hyperbilirubinaemia in association with group-B streptococcal sepsis. Haemodynamic instability deterred us from performing exchange transfusion, and so plasmapheresis was used to lower the bilirubin level. The procedure was very effective and well tolerated.

Four cases with pemphigus 3 p. vulgaris and one p. foliaceus were treated with plasmapheresis. The volume of plasma exchange was 500-800 ml once a week for 6 weeks. In one patient the procedure was repeated four times, within 8 months. All patients were treated with prednisone and azathioprine or cyclophosphamide at the same time, but in lower dosages. The results were satisfactory and the remission after the treatment lasted for 11 months to 2 years.
