Does Pharmacological Therapy Still Play a Role in Preventing Sudden Death in Surgically Treated Tetralogy of Fallot?

Tetralogy of Fallot (TOF) is the most common cyanotic congenital heart disease, with a familial recurrence risk of 3%. Despite performing an optimal surgical repair, TOF patients may feature a poor medium and long-term survival rate: atrial re-entrant tachycardia will develop in more than 30% of patients and high-grade ventricular arrhythmias will be seen in about 10% of patients. These life-threatening arrhythmias and consequent sudden death continue to represent serious complications following TOF repair. Radiofrequency ablation and implantable cardioverter defibrillator are today the most e

Theorizing that Psychedelic Assisted Therapy May Play a Role in the Treatment of Trauma-Induced Personality Disorders.

Borderline personality disorder (BPD) and post-traumatic stress disorder (PTSD) share overlapping neurobiological mechanisms particularly reward deficiency and stress-like anti-reward processes. And so, BPD may be reclassified as a "traumatic personality stress disorder" (TPSD) with ensuing common therapeutic strategies that may stabilize dopaminergic reward function such as psychedelic-assisted therapy. Integrated therapeutic strategies may be further supported by genetic studies aimed at assessing similarities between the two therapeutic entities. In this perspective we theorize that psyched

Combinatorial therapy with immunosuppressive, immunomodulatory and tear substitute eyedrops ("Triple Play") in Recalcitrant Immunological Ocular Surface Diseases.

PURPOSE: The current paradigm for therapy of recalcitrant ocular surface diseases (OSD) consists of a sequential, step-up treatment approach. A combinatorial topical therapy (anti-inflammatory/immunosuppressive [steroid] with immunomodulatory [pooled human immune globulin] and tear substitute [serum]) that simultaneously targets several immunological pathways may be more efficacious. This report evaluates if the combinatorial therapy resulted in clinical benefit in patients with recalcitrant OSD. METHODS: We performed a retrospective case study of patients receiving topical, preservative-free

Modeling ADHD-type arousal with unilateral frontal cortex damage in rats and beneficial effects of play therapy.

It has been recently shown that human adolescents with Attention Deficit/Hyperactivity Disorder (ADHD) have frontal lobe deficits, especially on the right sides of their brains (). ADHD is commonly treated with psychostimulants which may have adverse consequences. Hence, less invasive therapies need to be developed. In the present work, we tested the ability of right frontal lesions to induce hyperactivity in rats. We also evaluated the effects of chronic play therapy during early adolescence to reduce both hyperactivity and the elevated playfulness later in development. Play therapy was able …

Doctor of physical therapy students' perceptions of artificial intelligence chatbots versus peer role play for therapeutic interviewing practice.

INTRODUCTION: Therapeutic interviewing supports rapport building, diagnostic accuracy, and person-centered care. Peer role play (PRP) is used in Doctor of Physical Therapy (DPT) education for rehearsing patient-provider interactions; however, there are limitations regarding patient portrayal. Artificial intelligence chatbots (AI) may offer an alternative. This study examined DPT students' perceptions and quality of information using AI for therapeutic interviewing compared with PRP. METHODS: This retrospective mixed methods cohort study analyzed data from two simulated interviewing activities

Play therapy as a tool to foster autonomy, initiative and socio-emotional learning in children with down syndrome.

INTRODUCTION: In India, where the right to education is a fundamental right, it still remains an unfulfilled entitlement for disabled children due to their limited psychological skills. Imparting psychological skills to disabled children requires strategic planning and technique. Play can be a useful and effective tool to provide psychological skills to children. AIM: The current research aims to explore the impact of play therapy on autonomy, initiation, and social-emotional learning among children with Down syndrome. MATERIAL AND METHODS: The study uses the mixed research approach and adop…

Other-initiations of repair by children with developmental language disorder in speech-language therapy and non-institutional play.

This study explores the use of other-initiations of repair by children with developmental language disorder (DLD). The data are children's video-recorded language assessment sessions, speech-language therapy sessions and two kinds of non-institutional play sessions, parent-child and peer play. The videotapes were transcribed following CA conventions focusing on speech and relevant embodied actions. Other-initiations of repair were identified, collected and analyzed. The analysis focuses on open requests and candidate understandings which were the two most common types of other-initiation of re

Two-year follow-up of 90 children with autism spectrum disorder receiving intensive developmental play therapy (3i method).

BACKGROUND: The Intensive, Interactive, and Individual (3i) intervention approach aims to decrease the severity of autism spectrum disorder (ASD) using intensive developmental play therapy (3i). We performed a retrospective study of 90 children who were enrolled for 2 years in the 3i approach to assess changes and predictors of changes in ASD severity at follow-up (FU). METHODS: The ASD severity of all patients (N = 119) who began 3i intervention between 2013 and 2018 was systematically measured using the childhood autism rating scale (CARS) and autism diagnosis interview-revised (ADI-R). Anorexia nervosa and depression in a 5-year-old girl: Treatment with focused family play therapy and medication.

OBJECTIVE: The current case presentation examines the treatment and recovery of a 5-year-old girl diagnosed with comorbid anorexia nervosa and major depressive disorder. Researchers have shown that children as young as 3-years-old may be dissatisfied with their bodies and have concerns about their weight. This case presentation of a 5-year-old child demonstrates that the age range defining early onset of eating disorders can span into early childhood, which requires treatment tailored to the developmental level of these young children. METHOD: A new method "focused family play therapy" was ap…

Do apelin and ghrelin play a role in multiple sclerosis? The analysis of patients treated with immunomodulatory therapies.

INTRODUCTION: Multiple sclerosis (MS) is an autoimmune disorder with a complex and multifactorial etiology. Increasing evidence suggests that peptide hormones may significantly influence the pathogenesis of MS by affecting inflammatory processes and metabolic dysregulation. The aim of the study was to evaluate the levels of apelin and ghrelin, which are two peptide hormones, in patients with relapsing-remitting MS (RRMS) receiving natalizumab and fingolimod compared to healthy controls. METHODS: We enrolled forty-nine patients with RRMS and thirty-eight healthy individuals. The participants w

Playing with Gladys: A case study integrating drama therapy with behavioural interventions for the treatment of selective mutism.

This case study examines an integrative approach combining drama therapy and the behavioural skill "shaping", as offered to Gladys, a 5-year-old girl diagnosed with selective mutism. This study found that shaping, when implemented in the context of play, with play as the primary reinforcer, elicited from Gladys vocalization and eventually speech within a very short time. Her vocalizations allowed her to enter dramatic play, which in turn propelled spontaneous speech. This article looks at how the three elements of dramatherapy - the playspace, role-playing and dramatic projection - brought abo

Play amid pressure: Multidisciplinary perspectives on the role of child life therapy in an Australian tertiary hospital emergency department.

BACKGROUND: Child life therapists (CLT) use play and education to empower children and reduce their stress and anxiety in hospital. There is growing research on their effect on patient care; however, experiences of the multidisciplinary team (MDT) who work alongside them have not been explored. OBJECTIVES: To explore the MDT's perceptions on the role and impact of CLT in a children's emergency department (CED), during routine procedures such as cannulation, sedation and laceration repair. METHODS: A qualitative descriptive study was conducted using in person semi-structured interviews with 1

[The snowflake family - Integrative play therapy for traumatized child with complex developmental disorder induced by family environment. Case study].

In the case of a little preschool boy, I would like to show how traumatic environmental influences affect a child's development, coping strategies, as well as his symptomatic behavior, emotion and mood regulation, and social functioning. This little boy has also suffered many damaging environmental effects during his own life, losses, stressful situations, conflicts. In addition, the losses, traumas and injuries suffered by his parents also shaped his development, the development of his difficulties and symptoms. The range of early fractures was compounded by the trauma at Ákos, the loss of hi

Impact of therapeutic play and music therapy on dental anxiety and pain perception in pediatric patients: a clinical study.

Therapeutic play and music therapy are promising behavioral techniques that could effectively reduce dental anxiety and pain perception. This study aimed to evaluate the effects of therapeutic play and music therapy on dental anxiety and pain levels in pediatric patients undergoing tooth extraction. 126 children aged 6–8 years who required maxillary molar extraction were divided into three groups (n = 42): control (Tell-Show-Do), music therapy, and therapeutic play groups. Vital signs (eg, heart rate, blood pressure, oxygen saturation, and body temperature) were recorded, while the Modified Ch
