An unusual cause of popliteal cyst.

We describe a surgical treatment for popliteal cyst in a 41-year-old woman affected by diffuse pigmented villonodular synovitis (PVNS) of the knee with extra-articular spreading. The treatment consists of an arthroscopic synoviectomy followed immediately by an open removal of the cyst. At 28-month follow-up, the patient is asymptomatic and no evidence of recurrence of PVNS has been seen. We believe that this double surgical approach may reduce the probability of recurrence of PVNS.

Arthroscopic management of popliteal cysts.

BACKGROUND: Management of popliteal cyst is controversial. Owing to high failure rates in open procedures, recent trend is towards arthroscopic decompression and simultaneous management of intraarticular pathology. We retrospectively analysed clinical results of symptomatic popliteal cysts after arthroscopic management at 24 month followup. MATERIALS AND METHODS: Retrospective analysis of hospital database for patients presenting with pathology suggestive of a popliteal cyst from June 2007 to December 2012 was done. Twelve cases of popliteal cyst not responding to NSAIDS and with Rauschning a…

Diagnosis and treatment of popliteal cysts.

The authors reviewed 122 patients affected with popliteal cysts and treated between 1975 and 1985 with or without surgery; follow-up ranged from 1 to 7 years (average 3 years and 2 months). The patients were subdivided into 3 age groups, and observed for the clinical, anatomo-pathological and therapeutic differences among them. Cysts affecting children and adults were considered to be acute, or chronic bursitis, and those affecting elderly patients true cysts deriving from the joint cavity. The diagnostic effectiveness of arthrography and sonography is emphasized.

Adventitial cysts of the popliteal artery.

Adventitial cysts of the popliteal artery are rare. Their pathogenesis is debated. In one of the two cases reported here, there was a communication between the arterial cyst and the articulation of the knee. This supports the embryonic theory that the articular synovium and adventitial cysts have the same mesenchymal origins. Treatment is surgical, with simple evacuation of the contents of the cyst in uncomplicated cases, excision and reconstruction in the others.

Arthroscopic Decompression of a Popliteal Cyst.

BACKGROUND: Popliteal (Baker) cysts are enlarged gastrocnemius-semimembranosus bursae leading to swelling in the popliteal fossa. Surgical decompression and capsulectomy is the definitive treatment for symptomatic cysts with arthroscopic or open decompression. Arthroscopic decompression is minimally invasive, entails lower risks, and allows for earlier and more aggressive rehabilitation compared with open excision. INDICATIONS: Indications for popliteal cyst decompression include pain and mechanical discomfort refractory to conservative treatment. Further indications are neurovascular comprom…

[Adventitial cyst of the popliteal artery].

Vascular cystic adventitious degeneration (CAD) is a rare disease of unclear etiology that affects mainly the popliteal artery (up to 85-90% of cases). Clinical manifestations are similar to aneurysm of the popliteal artery. CAD occurs mainly in males of the 4th and 5th decades of life (range 10-77 years). These patients usually have no risk factors of cardiovascular diseases. Unilateral symptoms prevail. We report a review and a 46-year-old patient with CAD of the popliteal artery.

Adventitial Cyst of the Popliteal Artery.

Teaching Point: This clinical case shows the importance of also examining the arteries when investigating suspected deep venous thrombosis.

Osteochondrolipoma Presenting as a Popliteal Cyst.

Here, we describe a popliteal mass that was initially misdiagnosed as a simple popliteal cyst, which finally turned out to be osteochondrolipoma. A 63-year-old housewife presented with sustained knee pain in association with a palpable mass on the popliteal fossa. The mass was in the posteromedial area and soft, non-tender, non-movable in the posteromedial area. Using plain radiography, the mass appeared as a round, soft tissue density lesion containing bony fragments. We performed an ultrasound-guided needle biopsy in conjunction with magnetic resonance imaging, followed by an open excisional…

Coccidiomycosis presenting as a popliteal cyst.

Coccidiomycosis is a fungal infection that primarily causes pulmonary disease. Extrapulmonary dissemination can occur to the musculoskeletal system with the knee joint most frequently involved. This case report describes a patient with coccidiomycosis whose initial presentation was of a popliteal cyst. The need for aggressive surgical and antibiotic treatment to eradicate this infection is discussed. Coccidiomycosis should be considered in a differential diagnosis of patients with popliteal cysts without other obvious etiologies.

[Adventicial cyst of the popliteal artery].

In a 42-year-old man with intermittent calf claudication, cystic adventitial disease of the popliteal artery was demonstrated by 3D CT. Cystic mass was seen to be compressing the arterial lumen. The diagnosis of cystic adventitial was confirmed by surgery. Compared with color doppler sonography, MR Imaging, 3D CT provides additional useful anatomic information concerning disease of the popliteal artery, usually gained only with sonography or arteriography.
