Delivery Routes of Topical Eyedrops Reaching the Posterior Segment of the Eye-Recent Advances.

Current topical eyedrop treatments for posterior segment diseases face significant challenges, including rapid tear clearance and ocular barrier limitations, which hinder effective drug delivery to target tissues in the posterior segment. To date, no eyedrop formulation has been clinically approved for the treatment of ocular fundus diseases. Eyedrops reach the posterior segment via two primary routes: the corneal route and the conjunctival routes, the latter comprising the conjunctival-vasculature route and the conjunctival-scleral route. This review summarizes recent advances in topical eyed

Laser-Activated Drug Implant for Controlled Release to the Posterior Segment of the Eye.

To treat chronic posterior eye diseases, frequent intravitreal injections or sustained-release drug implants are the current standard of care. Sustained-release drug implants often involve burst release of the drugs and the dosage from the implants cannot be controlled after implantation, which may lead to local side effects. The present study attempts to develop a dosage-controllable drug delivery implant that consists of a nanoporous biodegradable PLGA capsule and light-activated liposomes. Controllable drug release from the implant was achieved using a pulsed near-infrared (NIR) laser both

Optical Coherence Tomography Findings in Ocular Syphilis Involving the Posterior Segment of the Eye.

PURPOSE: To analyse posterior segment optical coherence tomography (OCT) findings in ocular syphilis. METHODS: Medical records of 54 patients presenting consecutively with syphilitic uveitis were reviewed. Vitreous, retina and choroid (one eye/patient) were assessed by spectral-domain OCT on presentation (54 eyes) and after treatment (31 eyes). Improvement in signs and associations between presenting signs and final best corrected visual acuity (BCVA) were determined by McNemar's and Fisher's exact tests. RESULTS: Early inner retinal OCT findings included hyperreflective dots (n = 49, 91%),

Histopathological study of anterior segments of the eye with the intraocular lens in the posterior chamber.

We conducted a histopathological study of the anterior segments of the eye of a female who died 2 years after insertion of an intraocular lens (IOL), which had one end of the supporting loop inserted in the lens capsule bag and the other in the ciliary sulcus. No changes were noted in the iris and ciliary body located within the capsule. However, in the part in the ciliary sulcus, the iris root was bent towards the chamber angle and the trabecular meshwork was depressed. Moreover, the ciliary body exhibited hyaloid degeneration. A secondary cataract had formed in the periphery of the IOL. Thes

Dual-functionalized mesoporous silica nanoparticles for topical axitinib delivery to the posterior eye segment.

Topical drug delivery to the posterior eye segment remains a significant challenge due to ocular anatomical barriers, particularly in diseases such as wet age-related macular degeneration (AMD), where treatment typically relies on frequent intravitreal (IVT) injections of anti-angiogenic agents. In this study, we present a non-invasive eye drop formulation of axitinib (AXT), a potent vascular endothelial growth factor receptor (VEGFR) inhibitor, encapsulated within 25-nm dual-functionalized mesoporous silica nanoparticles (AXT@dual-MSNs) engineered for efficient retinal delivery. The nanoparti…

[Irrigation system "Meturakol" in the treatment of dystrophic processes of the posterior eye segment].

An original method for treating degenerative processes in the eye by means of Meturakol irrigation system is described. Studies were performed in 95 patients aged 60-70 years. Use of Meturakol irrigation system ruled out pain associated with frequent administration of drugs, ensured high concentrations of drugs in the posterior segment of the eye, and improved the efficiency of drug therapy in patients with degenerative diseases of the eyes. Good remote results are due to potent drug supply to ocular tissues and to the use of hemostatic sponge Meturakol, containing methyluracil. The method is

A Cationic Block Co-Polymer for Gene Delivery in the Posterior Segment of the Eye.

Diseases of the back of the eye such as neovascular age-related macular degeneration (nAMD) are vision threatening and treatment is burdensome for patients, often requiring ocular injections every other month. Injection risks and logistics lower patient compliance; however, even patients receiving optimal treatment can deteriorate. Gene silencing has shown great potential as a therapeutic alternative but is often limited by instability of the genetic payload, reducing efficacy. In the current work, a cationic block co-polymer was developed and investigated as a delivery system for an antisense

Topical and systemic absorption in delivery of dexamethasone to the anterior and posterior segments of the eye.

PURPOSE: This study aimed to: (1) determine the relative efficiencies of topical and systemic absorption of drugs delivered by eyedrops to the anterior and posterior segments of the eye; (2) establish whether dexamethasone-cyclodextrin eyedrops deliver significant levels of drug to the retina and vitreous in the rabbit eye, and (3) compare systemic absorption following topical application to the eye versus intranasal or intravenous delivery. METHODS: In order to distinguish between topical and systemic absorption in the eye, we applied 0.5% dexamethasone-cyclodextrin eyedrops to one (study) e

[Possibilities and limitations of the surgery of the eye's posterior segment under the outpatient conditions].

The goal of this article was to analyze possibilities of the vitreoretinal surgery under the outpatient conditions and to set its limitations. During the period January 1st-September 30th, 2004, there were performed 95 operations of 78 eyes in 77 patients. Number of men and women was practically equal; the age ranged 17-86 years (average 62.6 years). We operated on mostly the retinal detachment. These as well as other procedures, including also the extreme surgery with relaxing retinectomy, extraction of the subretinal tractions and membranes, silicone oil implantation or extraction, or operat

[Indications for transvitreal removal of foreign bodies from posterior eye segments].

Results of treatment of 40 patients (43 eyes) with foreign bodies in the posterior segment of the eye are discussed. Special attention is paid to correct assessment of the risk and efficacy of removal of a fragment in each case. Only 67.5% of patients presented with obvious indications for removal of a foreign body through the vitreous. Fragments were removed in 83.3% of these patients. Complications occurred in 6.6%. The main causes of failure of sparing removal of a foreign body from the posterior segment of the eye were detachment of the retina and hemophthalmia.

Nonlinear distortion correction for posterior eye segment optical coherence tomography with application to tree shrews.

We propose an empirical distortion correction approach for optical coherence tomography (OCT) devices that use a fan-scanning pattern to image the posterior eye segment. Two types of reference markers were used to empirically estimate the distortion correction approach in tree shrew eyes: retinal curvature from MRI images and implanted glass beads of known diameter. Performance was tested by correcting distorted images of the optic nerve head. In small animal eyes, our purposed method effectively reduced nonlinear distortions compared to a linear scaling method. No commercial posterior segment…

Delivery of Topically Applied Calpain Inhibitory Peptide to the Posterior Segment of the Rat Eye.

We developed an inhibitory peptide that specifically acts against mitochondrial μ-calpain (Tat-μCL, 23 amino acid, 2857.37 Da) and protects photoreceptors in retinal dystrophic rats. In the present study, we topically administered Tat-μCL to the eyes of Sprague-Dawley rats for 7 days to determine both the delivery route of the peptide to the posterior segment of the eye and the kinetics after topical application in adult rats. Distribution of the peptide was determined by immunohistochemical analysis, and enzyme-linked immune-absorbent assay was used to quantify the accumulation in the retina.

Transcriptomic Analysis of the Ocular Posterior Segment Completes a Cell Atlas of the Human Eye.

Although the visual system extends through the brain, most vision loss originates from defects in the eye. Its central element is the neural retina, which senses light, processes visual signals, and transmits them to the rest of the brain through the optic nerve (ON). Surrounding the retina are numerous other structures, conventionally divided into anterior and posterior segments. Here we used high-throughput single nucleus RNA sequencing (snRNA-seq) to classify and characterize cells in the extraretinal components of the posterior segment: ON, optic nerve head (ONH), peripheral sclera, peripa
