To study the electrophysiological effects of oral propafenone on accessory pathways and determine the potential for catecholamine-mediated reversal of these effects, comprehensive electrophysiology studies (EPS) were conducted in 11 patients with manifest (n = 9) or concealed (n = 2) pre-excitation syndrome. EPS were performed at baseline (in the drug-free state), after oral propafenone loading, and with isoproterenol infusion during propafenone therapy. The study group included 10 men and 1 woman with a mean age of 39 +/- 13 years, who presented with symptoms of palpitations (n = 6), presynco

A 34-year-old woman with mitral valve prolapse, frequent ventricular dysrhythmias, and chest pain was studied. Surface electrocardiograms demonstrated short PR and prolonged Q-T intervals. A-V nodal conduction times during atrial pacing were characteristic of A-V nodal bypass tract function. The case is thus interpreted to be one of overlap between three syndromes known to predispose to cardiac dysrhythmias, i.e., mitral valve prolapse, Lown-Ganong-Levine syndrome, and prolonged Q-T interval syndrome.

The sequence of retrograde atrial activation of premature ventricular beats was studied in 12 patients without pre-excitation syndrome. Forward and retrograde AV nodal conduction times were within normal limits. The septal low right atrial (LRA2) deflection was inscribed 25 to 55 ms (ave: 37.9 ms) before the low left atrial (LLA2) electrogram and 35 to 75 ms (ave: 53.7 ms) ahead of the high right atrial (HRA2) deflection. LLA2 preceded HRA2 in 8 patients, more or less coincided with HRA2 in 3 cases, and was inscribed 10 ms ahead in 1 patient. In all cases a decrease in the coupling (St1-St2) i

A male in his mid 50s, chronic smoker and hypertensive for 10 years presented with history suggestive of typical angina, electrocardiographic evidence of pre-excitation and serial elevation of cardiac biomarkers. Serial electrocardiograms showed subtle changes (axis shift, horizontal ST-segment changes) that could be presumptive of an anterior wall myocardial infarction. Speckle tracking echocardiography revealed territorial reduction of longitudinal strain corresponding to the left anterior descending artery with coronary angiography corroborating the same and underwent successful revasculari

Pre-excitation syndrome is common in families with Leber's hereditary optic neuropathy (LHON). 24 Finnish families with LHON were screened for the 11778 and the 3460 mitochondrial DNA mutations. 5 of 30 individuals with LHON and the 11778 mutation had the Wolff-Parkinson-White pre-excitation syndrome. None of 10 with the 3460 mutation or of 11 with "other" mutations had this syndrome. Overall, 5 of 51 LHON patients and 9 of 112 symptom-free maternal relatives had Wolff-Parkinson-White syndrome (9%). In paternal relatives, the frequency was 1.6%. Mitochondrial DNA causal for LHON may contribute

Various parameters relating to the radio-frequency ablation of accessory pathways were studied in 53 patients (27 males, 26 females: mean age 38.5 [14-64] years) with a history of paroxysmal tachycardia (over 1 month to 50 years), shown to be caused by an accessory pathway (Wolff-Parkinson-White syndrome). In all patients the following values were obtained: (1) number of procedures necessary to achieve permanent blockage of the accessory pathway (1-4); (2) duration of each procedure (45-420 min); (3) duration of fluoroscopy (5-102 min); (4) number of necessary radio-frequency applications (1-4

Ablation of accessory pathway (AP) of any location was performed either with a right or a left approach (patent foramen ovale, transpeptal or a retrograde transvalvular aortic catheterism in 121 patients. The best ablation site was localized: 1) by the recording of a potential likely with the Kent bundle activation; 2) the earliest site of retrograde atrial activation during orthodromic reciprocating tachycardia (80 +/- 35 ms); 3) first ventricular potentials recorded ahead or synchronous with the delta wave in standard leads; 4) disappearance of preexcitation due to the pressure of the cathet

Length-based Fourier analysis (LFA) was applied to tomographic gated blood-pool study, and phase and percent-shortening diagrams were generated. In 22 patients with pre-excitation syndrome and ten control subjects, the most basal short-axis section was used for tomographic analysis. When the initial abnormal phase was considered as the location of accessory conduction pathway (ACP), correct diagnosis for the localization of ACP was given in 19 of 22 patients. In ten control subjects, no specific segments of initial phase were noted, although six patients had initial phase in the septal or para

The scanty available data in histology of accessory AV pathways, namely James', Mahaim's and Kent's fibres, together with Paladino's bundle, defects in the fibrous annuli and "Ring tissue", were considered for their bearing on old and new pathophysiological questions about ventricular pre-excitation and complicating tachy-arrhythmias. The histological findings in nine hearts allowed for a realistic and synthetic schematization of all the main types of anomalous AV bypasses, which have been emphasized in discussing the subject. The inadequacy of basic knowledge concerning the anatomoclinical si

INTRODUCTION: Pregnant women with pre-excitation syndrome are more likely to develop supraventricular tachycardia (SVT) during pregnancy and delivery, leading to an increased risk of adverse events.
METHOD: This was a retrospective study of 309 pregnancies in 280 women (29 women had two pregnancies in this series) with pre-excitation syndrome who delivered at West China Second University Hospital from June 2011 to October 2021. All the 309 pregnant women with pre-excitation syndrome were divided into SVT and non-SVT groups to analyze the cardiac and obstetric complications.
RESULTS: Among th

Objective: To investigate the characteristics of pregnancy with pre-excitation syndrome and its influence on pregnancy outcomes. Methods: A retrospective analysis was made on the clinical data of 62 cases of pregnancy complicated with pre-excitation syndrome in Beijing Anzhen Hospital from Jan. 2008 to Dec. 2008. According to whether there was a supraventricular tachycardia (SVT) in pregnancy, they were divided into two groups. There were 16 pregnant women in the SVT seizure group during pregnancy, and 46 pregnant women in no SVT seizure group, with a multi-disciplinary comprehensive diagnosis

Most accessory pathways in pre-excitation syndrome are capable of antegrade conduction from atrium to ventricle, and identified by characteristic ECG abnormalities such as a delta wave (ventricular pre-excitation) and a shortened PR interval. Therefore, the traditional diagnosis method is to detect the initial changes of the QRS complex. Here we report a patient initially mistaken as having atrioventricular re-entrant tachycardia (AVRT) and concealed pre-excitation syndrome. A diagnosis of incomplete latent pre-excitation syndrome was established on the basis of differences between the ECGs dur
