Presbycusis and its management.

Age-related hearing loss, or presbycusis, is becoming a public health issue. With the large number of ageing baby boomers, primary care providers can expect to see an increase in older adults suffering from chronic health problems such as presbycusis. It is the primary care provider's responsibility to perform prompt auditory screenings and to recognize the early clinical signs and symptoms of hearing loss. Early interventions, such as hearing aids or surgery, could help to improve the client's quality of life. Primary care providers should also teach older adults proper auditory hygiene, and …

Audiometric shape and presbycusis.

The aim of this study was to describe the prevalence of specific audiogram configurations in a healthy, otologically screened population between 55 and 65 years old. The audiograms of 1147 subjects (549 males and 598 females between 55 and 65 years old) were collected through population registries and classified according to the configuration of hearing loss. Gender and noise/solvent-exposure effects on the prevalence of the different audiogram shapes were determined statistically. In our population 'Flat' audiograms were most dominantly represented (37%) followed by 'High frequency Gently slo

Cochlear pathology in presbycusis.

A survey of the temporal bone collection at the Massachusetts Eye and Ear Infirmary reveals 21 cases that meet the criterion for the clinical diagnosis of presbycusis. It is evident that the previously advanced concept of four predominant pathologic types of presbycusis is valid, these being sensory, neural, strial, and cochlear conductive. An abrupt high-tone loss signals sensory presbycusis, a flat threshold pattern is indicative of strial presbycusis, and loss of word discrimination is characteristic of neural presbycusis. When the increments of threshold loss present a gradually decreasing…

Audiologic findings in presbycusis.

Presbycusic Ss (N: 64) were studied using tests for cochlear functions (SISI test, Metz test, delta F threshold, remote masking), for retroc … try yielded pathologic values in more than half of the Ss. Tonal test patterns demonstrated that in presbycusis there is a frequent alteration of the monaural and binaural analytic processes of intensity, frequency and phase; a constant increase of stiffness in the hydrodynamic system of acoustic energy transmission; and, sometimes, an increase of resistance …

Temporal integration and presbycusis.

On 8 presbycusic Ss with similar sensorineural hearing losses, temporal integration (TI) was estimated by comparing HTLs for signal durations of 500 and 20 msec, at frequencies .5-6 kc/s. Normal-hearing young adult controls were also tested. Degree of group hearing loss for the presbycusic Ss was related to group TI data, but did not account for the amount of decrease in TI with these older Ss. Slightly reduced TI (6 db) was found in Ss' area of normal hearing even though the test frequency and adjacent frequencies were within a normal-hearing range. Reduced TI occurred most consistently in Ss

[Rehabilitation possibilities in presbycusis].

Although several types of hearing loss in aged people can be distinguished, the word presbyacusis has grown to common use. It is not primarily the decrease of sensitivity for sounds, but rather the decrease of the ability for discrimination and the increase of being troubled by noisy circumstances, that impair understanding of speech. Conversation with aged people have to be practised slowly and in a well pronounced way in favourite circumstances for listening. The hearing impairment in aged people can grow to a problem, especially by the social circumstances, so rehabilitation may not be rest

Critical bandwidth in presbycusis.

Twenty patients with presbycusis and a rather flat audiometric pattern had critical band estimation performed by the method of loudness summation, using noise bands centred around 1 kHz. The pooled data indicated a normal critical band both in patients with a hearing loss less than 50 dB HL and in patients with a hearing loss greater than or equal to 50 dB HL. The normal loudness difference between broad band and narrow band noise was reduced at all levels except the highest, giving rise to very flat CB functions. This reduction was more pronounced at the highest test levels in recruiting ears…

Histopathological observations of presbycusis.

Temporal bone histopathology of 17 aged patients who had spontaneous and gradually progressive bilateral sensorineural hearing losses associated with aging was studied. Six cases in the present material showed the gradually sloping audiometric curve; nine cases, abrupt high tone hearing loss; and two cases, the flat audiometric curve. The most prominent histopathological change in the inner ear was a decrease in the population of the spiral ganglion cells. However, diffuse senile atrophy was also often seen in the organ of Corti and the stria vascularis. A positive correlation between the degr

[Presbycusis - Age Related Hearing Loss].

Presbycusis or age related hearing loss can be defined as a progressive, bilateral and symmetrical sensorineural hearing loss due to age related degeneration of inner ear structures. It can be considered a multifactorial complex disorder with environmental and genetic factors. The molecular, electrophysiological and histological damage at different levels of the inner ear cause a progressive hearing loss, which usually affects the high frequencies of hearing. The resulting poor speech recognition has a negative impact on cognitive, emotional and social function in older adults. Recent investig…

Central presbycusis: an emerging view.

Age-related dysfunction of the central auditory system (central presbycusis) is common but rarely looked for by those who provide aural rehabilitation. Patients who complain of difficulty hearing in noise--the key symptom of central presbycusis--are generally disadvantaged with conventional rehabilitation. This symptom should be documented with commercially available speech-in-noise tests, which use materials that are uncomplicated to administer. Those patients who perform poorly on such tests should have a customized rehabilitation program aimed at optimizing their remaining communication abi…

Hearing loss with aging: presbycusis.

Older adults with presbycusis have varied presentations. Early identification requires sensitivity to communication problems described by patients and their families. Early amplification may lead to better success in hearing aid use and may prevent social isolation.
