Relieving pressure.

Mrs Smith has been resident on a continuing care ward for approximately six years. She was originally admitted after a left cerebral vascular accident that resulted in a right-sided hemiplegia. She is dysphasic but appears to understand much of what is said to her, and she is continent of urine and faeces, using facial expressions and body gestures to communicate her needs. In recent years her weight has increased from 63 kg to 83 kg and she is now chairbound, following a recent leg fracture.

Gradient pressure.

Using a properly timed sequence of early gradient pressure greatly facilitates the burn patient's rehabilitation in our program. Hypersensitivity is avoided or alleviated, skin breakdown is minimized, and early hypertrophic scar development and contracture is controlled. This allows commercial garments to be fitted at an early stage with the assurance that the skin is ready to accept the increased shear force and pressure of the garment. Early use of pressure can give the patient a head start in the rehabilitation process.

Optimizing Initial Shunt Pressure in Idiopathic Normal Pressure Hydrocephalus.

BACKGROUND: Although setting a low initial opening pressure in a programmable valve with a gravitational unit seems to be the gold standard treatment for idiopathic normal pressure hydrocephalus (iNPH) patients, there are no studies in the literature describing the optimal initial low-pressure setting. Here, we sought to describe the optimal initial opening pressure setting for iNPH in terms of both efficacy and safety. MATERIALS AND METHODS: Patients with probable iNPH underwent shunt surgery with a programmable valve with a gravitational unit, using either an opening pressure of 3 or 6.5 cm…

Blood Pressure-Lowering Medications, Sodium Reduction, and Blood Pressure.

BACKGROUND: Both blood pressure-lowering medication and sodium reduction are effective in hypertension control, but whether the effect of sodium reduction differ across blood pressure-lowering medications is unclear. This study aims to evaluate the dose-response effect of sodium intake reduction on blood pressure in treated hypertensive individuals and the impact of different classes of blood pressure-lowering drugs. METHODS: We searched multiple databases and reference lists up to July 9, 2024. Randomized controlled trials with a duration of ≥2 weeks comparing the effect of different levels …

Alternating pressure (active) air surfaces for preventing pressure ulcers.

BACKGROUND: Pressure ulcers (also known as pressure injuries, pressure sores, decubitus ulcers and bed sores) are localised injuries to the skin or underlying soft tissue, or both, caused by unrelieved pressure, shear or friction. Alternating pressure (active) air surfaces are widely used with the aim of preventing pressure ulcers. OBJECTIVES: To assess the effects of alternating pressure (active) air surfaces (beds, mattresses or overlays) compared with any support surface on the incidence of pressure ulcers in any population in any setting. SEARCH METHODS: In November 2019, we searched the…

Lower body negative pressure to safely reduce intracranial pressure.

KEY POINTS: During long-term missions, some astronauts experience structural and functional changes of the eyes and brain which resemble signs/symptoms experienced by patients with intracranial hypertension. Weightlessness prevents the normal cerebral volume and pressure 'unloading' associated with upright postures on Earth, which may be part of the cerebral and ocular pathophysiology. By placing the lower body in a negative pressure device (LBNP) that pulls fluid away from cranial compartments, we simulated effects of gravity and significantly lowered pressure within the brain parenchyma and …

Does pressure cause liver cirrhosis? The sinusoidal pressure hypothesis.

Independent of their etiology, all chronic liver diseases ultimately lead to liver cirrhosis, which is a major health problem worldwide. The underlying molecular mechanisms are still poorly understood and no efficient treatment strategies are available. This paper introduces the sinusoidal pressure hypothesis (SPH), which identifies an elevated sinusoidal pressure (SP) as cause of fibrosis. SPH has been mainly derived from recent studies on liver stiffness. So far, pressure changes have been exclusively seen as a consequence of cirrhosis. According to the SPH, however, an elevated SP is the ma…

Blood pressure modifies retinal susceptibility to intraocular pressure elevation.

Primary open angle glaucoma affects more than 67 million people. Elevated intraocular pressure (IOP) is a risk factor for glaucoma and may reduce nutrient availability by decreasing ocular perfusion pressure (OPP). An interaction between arterial blood pressure and IOP determines OPP; but the exact contribution that these factors have for retinal function is not fully understood. Here we sought to determine how acute modifications of arterial pressure will affect the susceptibility of neuronal function and blood flow to IOP challenge. Anaesthetized (ketamine:xylazine) Long-Evan rats with low (…

Toe pressure measurements compared to ankle artery pressure measurements.

The Trans-Atlantic Inter-Society Consensus (TASC)-recommended absolute toe pressure is < 30-50 mm Hg for definition of chronic critical limb ischemia (CLI). Toe pressures can be measured by different techniques. The authors analyzed the clinical use of the Doppler technique and an automatic device with optical sensors and estimated their value in documentation of chronic critical limb ischemia compared to ankle artery pressures. Three different investigations were performed: (1) In 16 healthy subjects the digital artery pressures were measured by using 3 different optical sensors (transmission…

Blood pressure and high blood pressure. Aspects of risk.

This report deals with three aspects of risk related to blood pressure and high blood pressure. The first aspect of risk concerns distributions of systolic blood pressure (SBP) and diastolic blood pressure (DBP) in the adult population and their relation to long-term risk of morbidity and mortality. By middle age, only a minority (about 20%) of Americans have optimal SBP and DBP levels, less than 120 mm Hg and less than 80 mm Hg, respectively. For the majority with higher levels, risks of major clinical events, including death from cardiovascular diseases and from all causes, are markedly incr…

Focal ischaemic normal pressure glaucoma versus high pressure glaucoma.

In a total group of 130 patients with Normal Pressure Glaucoma (NPG) twenty-six were classified as Focal Ischaemic NPG (FINPG). This subgroup has a typical defect at the disc with a comparable visual field defect in the corresponding half of the visual field. Visual field defects are more often seen in the upper than the lower half of the visual field. The defects in the upper half are on the average larger (stage 1.6) than those in the lower half (stage 0.9). Abnormalities of the chamber angle were observed in 12% of these patients, the same percentage as in the normal population. Hypertensio…

Intracranial pressure: cerebrospinal fluid dynamics and pressure-volume relations.

Continuous measurement of the intracranial pressure (ICP) is routine in todays evaluation of various intracranial diseases and increased ICP is a common therapeutical problem in neurosurgical patients. Still, very little is known about the patho-physiological and biomechanic events that lead to increased ICP. ICP is governed by 1) the resistance to absorption of cerebrospinal fluid (Rout), 2) the production rate of CSF (If) (taken together Rout and If are referred to as the "CSF dynamics"), and 3) the pressure in the Sagittal Sinus (Pss) in accordance with the equation: ICP = If X Rout + Pss. …

Gravitational effects on intraocular pressure and ocular perfusion pressure.

Changes in the gravitational vector by postural changes or weightlessness induce fluid shifts, impacting ocular hemodynamics and regional pressures. This investigation explores the impact of changes in the direction of the gravitational vector on intraocular pressure (IOP), mean arterial pressure at eye level (MAPeye), and ocular perfusion pressure (OPP), which is critical for ocular health. Thirteen subjects underwent 360° of tilt (including both prone and supine positions) at 15° increments. At each angle, steady-state IOP and MAPeye were measured, and OPP calculated as MAPeye - IOP. Experim…

Analysis of Intracranial Pressure Pulse-Pressure Relationship: Experimental Validation.

The slope of linear relationship between the amplitude of pulsations in intracranial pressure (ICP) versus mean ICP has recently been suggested as a useful guide for selecting patients for shunt surgery in normal pressure hydrocephalus (NPH). To better understand how the pathophysiology of cerebral circulation influences this parameter, we aimed to study the relationship between mean pressure and pulsation amplitude in a wide range of conditions affecting cerebrovascular tone and ICP in experimental conditions. We retrospectively analysed experimental material collected previously. Three physio…

[Perioperative blood pressure management : What is the optimal pressure?].

BACKGROUND: Measurement of blood pressure is part of standard monitoring procedures in anesthesia, in addition to the other vital parameters of heart frequency and peripheral oxygen saturation. In recent years the relevance of the duration and extent of perioperative episodes of hypotension for the occurrence of postoperative complications or even increased mortality have become the focus of scientific investigations. OBJECTIVE: The aim of this review is to briefly recapitulate the physiological aspects of blood pressure and to describe the pathophysiology and risk factors of perioperative hy…
