Oxidative agents can cause acute haemolytic anaemia in persons with G6PD deficiency. Clinically significant methemoglobinemia can develop as a result of medications. Although dapsone and primaquine are known to produce methemoglobinemia in susceptible individuals, methemoglobinemia has been reported only rarely in the human immunodeficiency virus (HIV) population. A series of 2-substituted primaquine analogues has been synthesized and evaluated against Plasmodium berghei in the mouse and Leishmania donovani in the hamster. Three members (3a,d,e) of the series were evaluated against Plasmodium cynomolgi in the rhesus monkey. One analogue (3d) was evaluated against Trypanosoma rhodesiense in the mouse, and two (3b,e) were evaluated against Schistosoma mansoni in the mouse. This paper reports on 2 hemolytic serious reactions in 2 Arabian patients because of the use of primaquine as presumptive antimalarial treatment given in Cuba to all travellers from countries where malaria is endemic. Taking into account the non-existence of imported malaria cases in travellers from the Arab-world since more then 15 years and the frequency of individuals presenting with glucose-6-phosphate dehydrogenase deficiency it is recommended not to use this therapeutic procedure, and to maintain the rest of surveillance measures to all travellers from such region.

We determined CYP2D6*4 and CYP2D6* genotypes and metabolizer phenotypes in 96 patients with suspected malaria in Venezuela and found intermediate or poor metabolizer phenotypes in ≈25% of cases. Nine of 44 malaria patients had Plasmodium vivax recurrence. Public health authorities should evaluate the benefits of increasing total doses of primaquine for treatment.
