Serotonin Syndrome After Prolonged Remifentanil and Propofol Infusion for Craniotomy: A Case Report.

Serotonin syndrome (SS) is a life-threatening condition caused by serotonergic medications. We describe a unique case of SS likely caused by prolonged exposure to propofol and remifentanil alone. A young male presented for vestibular schwannoma resection. Several hours into the case, the patient demonstrated hyperthermia and hemodynamic instability, followed by clonus, rigidity, shivering, and tachycardia after emergence. SS was diagnosed using Hunter's criteria and improved with supportive measures. While the patient endorsed a history of methamphetamine use, his urine drug screen was negativ

Failed Emergence After Pediatric Epilepsy Surgery: Is Propofol-Related Infusion Syndrome to Blame?

Propofol infusion syndrome was first reported in the literature by Bray in 1998. He described a series of fatal outcomes after a presenting constellation of symptoms observed in pediatric patients who had received prolonged propofol infusions. Profound metabolic acidosis and bradycardia are the disease's hallmark features, which can further develop expeditiously to rhabdomyolysis, renal failure, and heart failure. It has been subsequently theorized that a triggering mechanism or a precipitating factor sets up the progressive physiologic spiral which can ensue. The name of the disease was expan…

Propofol-related infusion syndrome in critically ill pediatric patients: coincidence, association, or causation?

Over the past two decades numerous reports have described the development of a propofol-related infusion syndrome (PRIS) in critically ill adult and pediatric patients who received continuous infusion propofol for anesthesia or sedation. The syndrome is generally characterized by progressive metabolic acidosis, hemodynamic instability and bradyarrhythmias that are refractory to aggressive pharmacological treatments. PRIS may occur with or without the presence of hepatomegaly, rhabdomyolysis or lipemia. To date, the medical literature contains accounts of 20 deaths in critically ill pediatric p

Veno-arterial ECMO for the management of propofol infusion syndrome following cardiac surgery.

Propofol infusion syndrome (PRIS) has been described in patients requiring prolonged sedation with this drug, as may occur in some cases after cardiac surgery. Its treatment is based on discontinuation of the drug and supportive management of organ dysfunction until resolution. In the postoperative period of cardiac surgery, diagnosing PRIS poses a challenge, since it must be differentiated from other, more common complications such as postcardiotomy shock. In these patients, PRIS may fatally worsen the prognosis. Veno-arterial ECMO therapy may help maintain cardiac output. Both the diagnosis …

Propofol infusion syndrome: a structured literature review and analysis of published case reports.

Propofol infusion syndrome is a rare, potentially fatal condition first described in children in the 1990s and later reported in adults. We provide a narrative review of what is currently known about propofol infusion syndrome, including a structured analysis of all published case reports; child and adult cases were analysed separately as propofol is no longer used for long-term sedation in children. The review contains an update on current knowledge of the pathophysiology of this condition along with recommendations for its diagnosis, prevention, and management. We reviewed 108 publications d…

The propofol infusion 'syndrome' in intensive care unit: from pathophysiology to prophylaxis and treatment.

Propofol is a short-acting intravenous anesthetic agent widely used for sedation in anesthesia and intensive care. However, during the last 15 years there have been quite a lot of publications reporting unexplained deaths among pediatric and adult critically ill patients. These cases shared common symptoms and signs unrelated with initial admission diagnosis and were under long-term propofol infusion at high doses. A new syndrome called 'propofol infusion syndrome' was defined, including cardiovascular instability, metabolic acidosis, hyperkalaemia and rhabdomyolysis, with no evidence for othe…

Diagnosed by Reversibility: Unmasking Propofol-Related Infusion Syndrome in a Critically-Ill Elderly Male.

Propofol-related infusion syndrome (PRIS) is an uncommon complication resulting from prolonged propofol use. Common clinical presentations include metabolic acidosis, cardiac arrhythmias, and renal complications. The mortality rate is high if it is not recognized in time. There is no antidote to propofol. Initial treatment involves discontinuing ongoing propofol use and providing supportive measures. The reversal of clinical and laboratory features upon discontinuation of propofol provides a basis for retrospective diagnosis or PRIS. In severe cases, ultrafiltration may be utilized.

Case Report: Sustained mitochondrial damage in cardiomyocytes in patients with severe propofol infusion syndrome.

Introduction: Propofol infusion syndrome (PRIS) is rare but a potentially lethal adverse event. The pathophysiologic mechanism is still unknown. Patient concerns: A 22-year-old man was admitted for the treatment of Guillain-Barré syndrome. On day six, he required mechanical ventilation due to progressive muscle weakness; propofol (3.5 mg/kg/hour) was administered for five days for sedation. On day 13, he had hypotension with abnormal electrocardiogram findings, acute kidney injury, hyperkalemia and severe rhabdomyolysis. Diagnosis and interventions: The patient was transferred to our intensive…

Propofol infusion syndrome: a structured review of experimental studies and 153 published case reports.

INTRODUCTION: Propofol infusion syndrome (PRIS) is a rare, but potentially lethal adverse effect of a commonly used drug. We aimed to review and correlate experimental and clinical data about this syndrome. METHODS: We searched for all case reports published between 1990 and 2014 and for all experimental studies on PRIS pathophysiology. We analysed the relationship between signs of PRIS and the rate and duration of propofol infusion causing PRIS. By multivariate logistic regression we looked at the risk factors for mortality. RESULTS: Knowledge about PRIS keeps evolving. Compared to earlier …

Minimal Propofol Dosing Triggers Propofol-Related Infusion Syndrome (PRIS) in a Geriatric Burn Patient: A Brief Report of Complex Burn Pharmacokinetics.

Background Propofol is an intravenous hypnotic agent widely used for anesthesia and sedation in critically ill, mechanically ventilated patients due to its rapid onset and offset, which facilitate ventilator weaning and extubation. Despite these benefits, propofol's lipid emulsion formulation poses an infection risk and is associated with propofol-related infusion syndrome (PRIS). PRIS is characterized by acute bradycardia that is resistant to treatment and may progress to asystole. Objectives We report the case of a 76-year-old woman who sustained full-thickness burns covering 30% of her tota

Propofol-related infusion syndrome happened in a non-critically ill perioperative young patient: A case report.

Propofol is a commonly used sedative for the induction of clinical anesthesia and sedation maintenance in intensive care unit patients. However, in a minority of patients, prolonged infusion of propofol may cause metabolic acidosis, electrocardiographic abnormalities, arrhythmias, and rhabdomyolysis, a condition known as propofol-related infusion syndrome. If not promptly identified and managed, propofol-related infusion syndrome can be fatal. Propofol-related infusion syndrome typically occurs in critically ill patients in the intensive care unit and is less common in clinical surgical anesth

The risk of propofol infusion syndrome on epilepsy patients:Insights from FAERS data and published case reports.

OBJECTIVE: PRIS has been documented in epilepsy patients treated with propofol. But the clinical features associated with the occurrence of PRIS in patients with epilepsy remain incompletely elucidated. This study aimed to investigate the relationship between epilepsy, antiepileptic drugs, and PRIS and draw conclusions about its clinical features. METHODS: Extracted PRIS reports documented in the FAERS (January 2013 to December 2023). Epilepsy patients and non-epilepsy patients were distinguished based on the indication information. We performed multivariate analyses for demography, epilepsy

Propofol infusion syndrome: a lethal condition in critically injured patients eliminated by a simple screening protocol.

UNLABELLED: Propofol infusion syndrome (PIS) is defined by arrhythmia, rhabdomyolysis, lactic acidosis, and unrecognized leads to death. We sought to determine the incidence of PIS in trauma patients and evaluate the efficacy of a prospective screening protocol in this patient population. MATERIALS AND METHODS: In Phase I of the before-and-after study (1st January, 2005-31st December, 2005), trauma patients who received propofol were evaluated. Records were reviewed for demographics, injury severity, propofol time, dose, and rates, laboratory values, and adverse events. Patients were identifi…

Propofol infusion syndrome complicated with mitochondrial myopathy, encephalopathy, lactic acidosis, and stroke-like episodes: a case report.

BACKGROUND: Propofol infusion syndrome (PRIS) is a rare but lethal complication of propofol use. It has been suggested that the pathological mechanism of PRIS involves mitochondrial disorder caused by propofol. CASE PRESENTATION: A 24-year-old woman who had been diagnosed with mitochondrial myopathy, encephalopathy, lactic acidosis, and stroke-like episodes was admitted to our hospital with impaired consciousness and myoclonus. To control the non-convulsive status epilepticus, propofol was administered. Arterial blood gas revealed metabolic acidosis, and creatinine kinase was elevated. The pa…
