In the last two decades there has been a remarkable expansion in research on psychotherapy in two areas: (a) assessing the effectiveness of psychotherapy compared to wait-list, no-treatment patient groups and groups treated with pharmacotherapy; (b) investigating curative factors in the therapeutic process: the extent of the psychotherapist's experience and kinds of interventions and errors in handling techniques. The first section of the review deals with the first of these areas and the second with the other. The following are the main research findings: 1. The effectiveness of psychotherapy…

This case report details the treatment of a longtime psychodynamic psychotherapy patient, with a particular focus on a ketamine-assisted psychotherapy (KAP) session, and how the progress achieved during this session compares with the literature on KAP. The patient is a 54-year-old woman with a history of multiple traumas, including sexual assault and life-threatening physical injuries, as well as a recent diagnosis of primary lateral sclerosis (PLS). For most of her life, she relied on extreme idealization of important people in her life as her primary defense mechanism, helping her to maintai…

Psychotherapy of obsessive-compulsive disorder. Obsessive-compulsive disorder (OCD) benefits from the large efficiency of cognitive behavior therapy (CBT). The core technique of CBT consists in repetitive exercices of exposure with response prevention. For the patient, six months to one year daily exercises during one hour per day is the standard ambulatory format. Therapist time devoted to the patient is around 15 to 20 hours, divided into half an hour to one hour sessions. CBT is as much efficient as serotonin reuptake inhibitor (SRI), the major recommended drug treatment of OCD. Significant…

Medea, the title character in Euripides' play, murdered her two sons in response to her husband Jason's abandonment. If her behavior can be understood, it is best understood in the context of shame. In an evolutionary context, shame is the affective response to the loss of one's place in the group. This response is related to the neurobiology of pain-not the acute pain experienced through the post-central gyrus, but the chronic, lingering pain that is experienced through the insular and cingulate cortices where homeostasis is regulated "from above." Shame is thus a fall in self-esteem, but sha

The purpose of the present study was to investigate the long-term effects of outpatient client-centered psychotherapy limited to 12 sessions on patients with adjustment disorder (ICD-10 F43.2). The patients in the follow-up study (n = 25) had been diagnosed to be suffering from an adjustment disorder in response to one of the following stressful events: the loss of an important person or a severe negative experience at work or university with lasting negative consequences. Two years after completion of treatment they were examined in regard to their levels of anxiety, depression and satisfacti…

The purpose of this paper is to describe and illuminate the phenomenon of scapegoating in group psychotherapy. Specifically, the role of projective identification - on both individual and group-wide bases - in the evolution of the deviant is delineated. Individual, interpersonal, and whole-group interventions are presented along with the technique of functional subgrouping, a relatively new and particularly potent group intervention. Several case vignettes are detailed for illustration.

Interpersonal psychotherapy (IPT), a time-limited treatment for major depression, was developed, defined in a manual, and tested in randomized clinical trials by the late Gerald L Klerman, MD, and collaborators. It has subsequently been modified for different age groups (adolescents-elderly), types of mood disorders (dysthymia, bipolar disorder, antepartum and postpartum patients), and non-mood disorders (bulimia, drug abuse, borderline personality disorder, social phobia, somatization, medically ill patients). It has been used as a long-term treatment, in a group format, over the telephone, a…

Patients with functional dyspepsia were assigned at random to cognitive psychotherapy (10 sessions of 50 min duration, n = 50) or to a control group (no treatment, n = 50). Before treatment all patients were assessed on psychological, somatic and lifestyle factors. If allocated to the therapy group all patients were also asked to define the main problems they wanted to discuss in therapy ('target complaints'). The patients were evaluated at the end of therapy (after 4 months) and at 1 yr follow-up. Outcome measures were dyspeptic symptoms, scores on 'target complaints' and psychological parame…

Previous research suggests that causal attributions for life-events are involved in the process of recovery from depression. The study assessed depressive symptoms and attributions before and after treatment in 40 clients. Each client received two forms of therapy in a cross-over design: Exploratory (relationship-oriented, interpersonal) therapy, and Prescriptive (cognitive-behavioural) therapy. During the course of treatment, attributions became significantly more unstable, specific and controllable. There was a positive relation between attributional change and change in depression symptoms,

One hundred ten nonpsychotic opiate addicts were randomly assigned to receive paraprofessional drug counseling alone or counseling plus professional psychotherapy. The outcomes of patients who received psychotherapy were examined in terms of their DSM-III diagnoses. Four groups were compared: those with opiate dependence alone (N = 16); opiate dependence plus depression (N = 16); opiate dependence plus depression plus antisocial personality disorder (N = 17); and opiate dependence plus antisocial personality disorder (N = 13). Those with opiate dependence plus antisocial personality disorder a…
