The authors report on their initial experience with pylorus-preserving pancreatoduodenectomy. In the last three years 11 patients with neoplastic disease of the pancreatic head and ampullary region underwent the above mentioned procedure. The surgical technique carefully preserved the blood supply and innervation to the antro-pyloric region and duodenum was transected 2 cm distal to the pylorus. Postoperative mortality was 9%. An instrument designed for this purpose should be safe, versatile and easy to use. A modified curved, 6 inch clamp facilitates doing a pyloromyotomy by implying the same action as other more commonly used instruments. The natural spreader for the pylorus is easy to make and it is also relatively inexpensive.

Double pylorus is a rare abnormality. Over a 4-year period, we have found two cases of double pylorus in 2,500 endoscopic examinations, an incidence of 0.08%. In one patient, the development of double pylorus from a prepyloric ulcer was followed by serial endoscopies. Most cases of double pylorus are acquired from peptic ulcer disease.

The control mechanisms of pyloric pressure responses have not been elucidated clearly. The purposes of this study were twofold: 1) to determine the dose-related pressure responses of the pylorus to exogenous glucagon, secretin, and cholecystokinin, and 2) to correlate changes in pyloric pressure with serum concentrations of these hormones. The entity of multiple pyloric channels may be congenital but the majority are acquired lesions representing fistulae secondary to peptic ulcer disease in the pyloroduodenal region. The fistula usually joins the lesser curvature of the stomach with the superior aspect of the duodenal bulb. The multichannelled pylorus produces a characteristic radiological appearance on barium mean examination with a double or split barium column in the region of the pylorus. Four cases are described and illustrated.

Doubling of the pyloric channel is one of the rare anomalies of the intestinal tract. There have been 23 cases of double pylorus that we have been able to find in the literature. As the etiologic aspects of double pylorus the majorities of the cases were thought to be acquired lesions from ulcer disease except three cases. We present a case of double pylorus which seems to be acquired lesion, with a review of the literature.
