OBJECTIVE: To evaluate the efficacy of surgery for the patients with pyriform sinus carcinoma and analyze the prognostic factors related to the cancer.
METHODS: Between December 1995 and December 2002, 62 patients with pyriform sinus cancer were treated in Zhejiang Tumor Hospital. There were 13 patients staged T1, 17 T2, 12 T3, 20 T4. Four patients received preoperative radiation and 40 patients had post-operative radiation. Among 62 patients, 33 patients were treated by partial laryngectomy, 29 patients were treated by total laryngectomy.

Thirty-five specimens from total laryngopharyngectomy clinically diagnosed as squamous cell carcinoma of the pyriform sinus were studied. Patients receiving chemotherapy or radiotherapy were excluded. Pathological examinations revealed 26 (74.3%) precancerous lesions and early carcinoma of the pyriform sinus. The treatment of 102 patients with squamous cell carcinoma of the pyriform sinus seen at the University of Virginia Medical Center from 1958 through 1977 is reviewed. Eight-seven cases form the basis for this report. Surgery alone, radiation alone and combination of the two were the treatment modalities utilized for the patients in this study. One hundred four patients with epidermoid carcinoma of the pyriform fossa were reviewed retrospectively. Survival, local recurrence and metastatic rates were all compiled for the purpose of comparing the efficacy of combined therapy with radiation therapy and surgery alone in treating such lesions. In our patient population, cancer of the hypopharynx arose 19 times as often in the pyriform sinus as in the postcricoid space (152:8). Most of the growths were far advanced when first seen (90% T3), and enlarged cervical nodes were present in 66% of the patients. Thirteen patients with pyriform sinus fistula treated surgically at our department were clinically evaluated. Twelve (92%) fistulae occurred in the left pyriform sinus, and one (8%) in the right. There were a mean of 4 infectious episodes before the final diagnosis. OBJECTIVE: To assess the effectiveness of reconstruction with pyriform sinus mucosa in partial laryngectomy and resumed laryngeal function.
METHOD: A total of 198 cases of laryngeal cancer who had undergone partial laryngectomy from 1980 to 1995 in our hospital were retrospectively reviewed. Hypopharyngeal perforation secondary to tracheal intubation is a rare and serious complication. Radiographic evaluation is helpful when the clinical diagnosis is not obvious. In one such case CT demonstrated the site of perforation and its extension into the superior mediastinum, facilitating the subsequent surgical approach.

Hemicricolaryngopharyngectomy was performed for anterior pyriform sinus carcinomas with preservation of voice and swallowing. The surgical specimen includes the homolateral half of the hypopharynx, larynx, and cricoid ring. The pharynx is reconstructed with a pectoralis myocutaneous flap, a deltopectoral flap, skin or dermal graft.
