Histologic pictures have been conflicting in previous cases of quinidine-associated hepatitis. We report a case of reversible granulomatous hepatitis from quinidine hypersensitivity, with granuloma induction occurring within three days after readministration of quinidine. Fever, urticaria, and mild thrombocytopenia were associated clinical findings. Both light and electron microscopic study results are reported.

A total of 36 plasma quinidine concentrations from nine hospitalized pediatric patients with cardiac arrhythmias were examined retrospectively to determine factors significantly affecting quinidine dosing. We randomized 61 patients with paroxysmal atrial fibrillation (AF) ( < 48 hours from onset) to either sotalol or quinidine treatment. Conversion of rhythm was recorded by Holter monitoring. The starting 80 mg dose of sotalol was repeated at 2, 6, and 10 hours if AF persisted (heart rate > 80 beats/min), and if systolic blood was > or = 120 mm Hg. To determine whether digoxin-quinidine interaction alters the inotropic effect of the glycoside, the response of peak isometric force and maximal rate of force development (dF/dt) in isolated feline right ventricular papillary muscles to digoxin and quinidine alone, and in various combinations, was examined. Using His bundle recording techniques, we examined direct and autonomically mediated conduction system effects of quinidine in five cardiac transplant recipients who have anatomically denervated hearts. We made control conduction interval and refractory period measurements, and then infused 10 mg/kg quinidine gluconate over a 20-min period.
