History and physical examination is the cornerstone in the appropriate diagnosis and treatment of any patient. A comprehensive physical examination is necessary to aid in determining distributions of symptoms and to lead one to the site of pathology. The aim of this article is to aid the clinician in distinguishing radiculopathy from other causes of neck and low back pain. Physical examination of the patient with suspected radiculopathy needs to be thorough and complete to make the most accurate diagnosis. Thorough knowledge of the evidence-based literature is beneficial in maximizing patient …

Electromyography (EMG) studies are useful in the anatomical localization of nerve injuries and, in most cases, isolating lesions to a single nerve root level. Their utility is important in identifying specific nerve-root-level injuries where surgical or interventional procedures may be warranted. In this case report, an individual presented with right upper extremity radicular symptoms consistent with a clinical diagnosis of cervical radiculopathy. EMG studies revealed that the lesion could be more specifically isolated to the T1 nerve root and, furthermore, provided evidence that the abductor…

This paper is a review of current information on cervical radiculopathy. The focus is on the natural history of the process, the accuracy of diagnostic tests to determine etiology, the differential diagnosis involved, and the surgical tactics available. The most important conclusions are the criteria for patient selection for surgery and a statistical demonstration of the adequacy of both the anterior and the posterior approaches for decompression of the cervical nerve root.

Orthopaedic surgeons frequently treat patients who report pain that radiates from the back into the lower extremity. Although the most common etiology is either a herniated disk or spinal stenosis, a myriad of pathologies can mimic the symptoms of radiculopathy, resulting in differences in the clinical presentation and the workup. Therefore, the clinician must be able to distinguish the signs and symptoms of lumbar radiculopathy from pathologies that may have a similar presentation. Being cognizant of these other possible conditions enables the physician to consider a breadth of alternative di…

The evaluation of patients with suspected lumbar radiculopathy is one of the most common reasons patients are referred for electrodiagnostic testing. The utility of this study depends on the expertise of the physician who plans, performs, and completes the study. This article reviews the strengths and weaknesses of electrodiagnosis to make this diagnosis, as well as the clinical reasoning of appropriate study planning. The current use of electrodiagnostic testing to determine prognosis and treatment outcomes is also discussed.

OBJECTIVE: To investigate radiculopathy after multilevel laminectomy in patients with cervical compression myelopathy and underlying mechanism.
METHODS: We retrospectively reviewed 287 consecutive patients with cervical compression myelopathy treated by multilevel cervical laminectomy and identified 37 patients (12.9%) with postoperative radiculopathy. There were 27 men and 10 women, with an average age of 56 years at time of surgery. The diagnosis was either cervical spondylosis (25 patients) or ossification of the posterior longitudinal ligament (12 patients).
RESULTS: Radiculopathy was ob…

Cervical monoradiculopathy occurs most commonly at C5-6 and C6-7. It is due mostly to acute herniation of nuclear material. The syndrome often responds to conservative prescription but when surgery is indicated, the results are good. The neurologic examination is the most specific and sensitive clinical test. Acute radiculopathies due to herniated nucleus are, in our hands, best approached via a posterior muscle-splitting incision. Chronic radiculopathies due to osteophyte formation may be approached either anteriorly or posteriorly. Reflex discogenic pain requiring anterior fusion exist but a…

With recent advancements and growing literature on the treatment of cervical spine disease in athlete populations, there are new paradigms for treatment, either operatively or nonoperatively, to optimize return to play (RTP) and outcomes while minimizing complications. Although conservative management is effective in many cases, high-level athletes with persistent symptoms may benefit from early surgical intervention. Anterior cervical discectomy and fusion and cervical disc arthroplasty (CDA) have shown high RTP rates, with CDA offering potential advantages in motion preservation and lower ad

A man in his 70s presented to hospital in early summer with a 5-week history of progressive lower back and right thigh pain, sensory deficit and right leg weakness. There had been limited response to analgesics in the community. Primary investigations on admission revealed no cause for his symptoms. Five days into admission, history emerged of a possible tick bite with subsequent rash sustained 3 months earlier, raising the possibility of neuroborreliosis leading to radiculopathy. Cerebrospinal fluid demonstrated a lymphocytic pleocytosis. An elevated Borrelia burgdorferi antibody index confir…

This article discusses the electrodiagnostic assessment of radiculopathy. Relevant anatomy initially is reviewed followed by discussion surrounding the approach to nerve conduction studies and needle electrode examination when it comes to radiculopathy evaluation. Pitfalls of the electrodiagnosis versus clinical diagnosis of radiculopathy and the definitions of acute versus chronic, and active versus inactive, are reviewed.

Electrodiagnostic studies play an important role in the evaluation of radiculopathy. This article reviews the use of standard nerve conduction studies, late responses, evoked potentials, and needle electrode examination in the work-up of lumbosacral and cervical radiculopathy.

This case report describes an 84-year-old man who presented with 3 weeks of gradually worsening right arm weakness associated with a painful vesicular rash across his arm. This occurred 3 days after his first dose of the ChAdOx1 nCoV-19/AZD1222 (University of Oxford, AstraZeneca and the Serum Institute of India) vaccine. The diagnosis was complicated by the presence of right C5-C6 foraminal stenosis compressing on the C6 nerve root sheath on non-contrast MRI, leading to an initial diagnosis of cervical radiculopathy. However, a positive varicella zoster virus-PCR test and findings of abnormal …

Musculoskeletal mimics of cervical radiculopathy will be explored in this AANEM monograph. A review of the literature and textbooks was conducted. Musculoskeletal cervical radiculopathy mimics include: (1) head/neck pain, such as neck tension, cervicogenic headache, and temporomandibular disorder; (2) referred pain from shoulder pathology, such as rotator cuff tears/impingement; (3) elbow region abnormalities, including medial/lateral epicondylitis; (4) wrist/hand conditions, such as DeQuervain's tenosynovitis and intersection syndrome; (5) muscle connective tissue disorders, including myofasc…
