[Pulmonary vein isolation using radiofrequency ablation].

Catheter ablation represents the primary treatment for most arrhythmias. The effectiveness of catheter ablation for the treatment of atrial fibrillation is superior to drug therapy. Therefore, catheter ablation has been established as an increasingly common procedure in clinical routine. In this context, the electrical isolation of the pulmonary veins (PVI) constitutes the cornerstone of the interventional therapy of paroxysmal and persistent atrial fibrillation. This article describes the procedure of pulmonary vein isolation utilizing radiofrequency point-by-point ablation. It shall be a pra

Tracheal necrosis following thyroid radiofrequency ablation.

INTRODUCTION: Radiofrequency ablation for benign thyroid nodules has a low rate of complications in experienced hands for selected indications, but tracheal necrosis is a major complication. CASE REPORT: A 60 year-old female patient underwent percutaneous radiofrequency ablation of an unesthetic benign isthmic thyroid nodule. The procedure was performed with a cooled electrode, using the "moving shot" technique on a trans-isthmic approach, under general anesthesia. Postoperative course was complicated by dysphonia and cervical pain implicating a third-degree skin burn of the medial cervical r…

Laparoscopic partial splenectomy using radiofrequency ablation.

INTRODUCTION: Epidermoid cysts are believed to be congenital in origin and often present in the pediatric population. Because of the concerns of compromised immunologic function after total splenectomy and increasing demand for minimally invasive approaches, interest has increased in performing the partial splenectomy in this patient population by laparoscopic techniques. Nonetheless, concerns for adequate hemostasis have limited its widespread adoption. Because radiofrequency ablation for the partial splenectomy has been done in a laparoscopic porcine model with good results, we used this tec…

Radiofrequency ablation of colorectal liver metastases.

BACKGROUND: Untreated patients with colorectal liver metastases rarely survive 3 years, and the 3-year survival rate for patients treated with chemotherapy is 3%. The best survival rates are for the small subgroup that has operable disease, i.e., 39% at 5 years. Radiofrequency ablation (RFA) offers a new opportunity to destroy liver metastases in patients who are not surgical candidates because of disease distribution or comorbidity. METHODS: Acceptance criteria were a maximum of four or five liver lesions with a maximum diameter of 4 or 5 cm and no evidence of active extrahepatic disease. Ne…

[Recent advances in radiofrequency catheter ablation].

Radiofrequency catheter ablation has been established as a first-line treatment of various paroxysmal tachycardias, and its developments are still ongoing. As recent advances of radiofrequency catheter ablation, we can point out the following issues: 1) transaortic approach for idiopathic ventricular tachycardia(VT) of LBBB-form with inferior axis, 2) new approach guided by mid-diastolic potential for verapamil-sensitive VT, 3) pulmonary vein(PV) isolation technique guided by PV ostial circular electrogram mapping for paroxysmal atrial fibrillation, 4) new ablation strategies for macro-reentry

Radiofrequency catheter ablation: indications and complications.

Radiofrequency catheter ablation was first described in pediatric patients in the early 1990s. Since then, multiple advances in the technology and understanding of radiofrequency ablation have allowed this technique to blossom into one of the most powerful therapeutic tools available to the pediatric electrophysiologist. This treatment has, in the majority of cases, replaced arrhythmia surgery as the definitive cure for most arrhythmias. Ablation therapy is commonly implemented as an elective procedure to treat paroxysmal reentrant supraventricular tachycardia. There are several advantages to …

Radiofrequency catheter ablation for atrial fibrillation.

Until recently, catheter-based radiofrequency ablation for atrial fibrillation was limited to palliative approaches of either atrioventricular node ablation or modification. It is now recognized that at least a proportion of patients with paroxysmal atrial fibrillation may be suitable for curative ablation of an underlying single arrhythmogenic focus. With the intense interest in this area, a catheter-based cure involving endocardial linear lesion creation for patients with chronic or paroxysmal atrial fibrillation may not be far in the future.

Radiofrequency catheter ablation for supraventricular tachycardia.

Radiofrequency catheter-mediated ablation is a recently developed technique of achieving cure of certain rhythm disorders, notably supraventricular tachycardia. In less than a decade, it has evolved from a theoretic concept to first line therapy for many patients, including those in the pediatric age group. The most common types of arrhythmias amenable to catheter ablation include supraventricular tachycardia due either to an accessory pathway or A-V nodal reentry and atrial fibrillation with a rapid ventricular response refractory to medical therapy. Before the development of the technique, t

[Radiofrequency ablation of idiopathic ventricular tachycardia].

Ablation by radiofrequency has recently come into use to treat cardiac arrhythmias, mostly supraventricular tachycardia. We describe 2 males aged 14 and 24, respectively, with idiopathic ventricular tachycardia. They underwent endocardial mapping and the exit points of the tachycardia were identified. In 1 of them tachycardia originated in the left ventricle, and the other in the outflow tract of the right ventricle. Radiofrequency ablation of the arrhythmia was successful in both.

Aortic leaflet perforation during radiofrequency ablation.

A 15-year-old girl underwent successful radiofrequency ablation of an accessory pathway. Following ablation, a new III/VI diastolic murmur was noted. Echocardiography revealed a perforated aortic leaflet, with a small amount of adherent valvular tissue and trivial aortic insufficiency by color Doppler. The patient remains asymptomatic. We are not aware of any similar complication from electrophysiological study, catheter ablation, coronary angiography, or percutaneous transluminal coronary angioplasty. We speculate that the current state of catheter technology contributed significantly to this…
