1. We have confirmed Ogata's results in experimental rat-bite fever caused by the bite of rats. 2. In our experiments with guinea pigs, swelling and congestion of the bitten parts, swelling of the subcutaneous lymph nodes, fever, and loss of weight were the typical symptoms. The similarity in the cases of rat-bite fever recorded in the literature establishes it as a definite clinical entity. The same symptomatology occurs in cases from Asia, Europe, and America. The greater frequency of the disease in Japan than elsewhere is probably due to the housing conditions and habits of the people resulting in the more frequent occurrence of rat-bites. It does not seem necessary to consider that cases occurring in Europe and America are due to the bites of rats that have been imported from Japan. We present the first ever report of streptobacillary rat-bite fever in New Zealand. The patient was a young man who was admitted with systemic sepsis. He presented with a high fever, hypotension, and tender axillary lymphadenopathy. He had been bitten by a rat a week earlier. Blood cultures grew Streptobacillus moniliformis, thus confirming the diagnosis. The literature on rat-bite fever is also reviewed.

We report a case of Streptobacillus moniliformis polyarthritis mimicking a rheumatoid arthritis, in a pet shop employee. In culture of fluid joint growth a curious Gram-negative bacillus was identified by polymerase chain reaction as Streptobacillus moniliformis. The outcome was good after surgical debridment and rifampin-clindamycin combination during 4 weeks.

Rat-bite fever (RBF) is a systemic bacterial illness caused by Streptobacillus moniliformis that can be acquired through the bite or scratch of a rodent or the ingestion of food or water contaminated with rat feces. Cases are rarely reported in the United States and the true incidence of disease is unknown. On August 7, 1996, a physician in a rural community reported a case of infection with S. moniliformis bacteremia in a 15-year-old boy to the New Mexico Department of Health.
