Conversion disorder presenting as hemiplegia and hemianesthesia with loss of neurologic reflexes: a case report.

Conversion disorders can present with a variety of sensorimotor signs and symptoms. We present the case of a young woman who presented with sudden onset hemiplegia and hemianesthesia including unilateral loss of deep tendon reflexes, Babinski reflex, and loss of rectal tone with bladder incontinence. The loss of rectal tone, loss of deep tendon reflexes, and a flaccid Babinski reflex are unusual signs in conversion disorders. A thorough medical workup including x-rays, CT scans, and magnetic resonance imaging revealed no organic causes for the neurologic deficits. Before the onset of symptoms,

An evaluation of the finger flexion, Hoffman's and plantar reflexes as markers of cervical spinal cord compression - A comparative clinical study.

OBJECTIVES: This study aimed at determining the frequency of abnormal finger flexion, Hoffman's and extensor plantar (Babinski) response in healthy adults and to determine the sensitivity and specificity of these tests as markers of spinal cord compression in symptomatic patients. METHODS: Patients attending the neurosurgery clinic with neck related complaints formed the case group. The control group consisted of consenting patient attenders and volunteers drawn from the students and faculty of our institute. All subjects underwent examination of the finger flexion, Hoffman's and plantar refl

The crossed extensor hallucis response in neurological disorders; an electromyographic analysis.

The electromyographic activity in several leg muscles during the elicitation of "the crossed up-going toe sign" (CUT) was studied in patients and compared with that obtained in connection with the Babinski sign and in normal subjects. The results indicate that the patterns of activation and the responsible pathways are different for the CUT and Babinski signs. The difference between the normal subjects and the CUT positive patient is a matter of quantitative difference of coactivation of distal leg muscles during the test procedure rather than a change of reflex patterns.

Renewing the fire: Joseph Babinski.

Joseph Babinski (1857-1932), a French neurologist of Polish origin, was a physician of the Paris hospitals and member of the Academy of Medicine. Two main periods must be distinguished in his scientific activity. Early in his career, Babinski, working as an anatomo-pathologist and his tologist under the direction of Victor Cornil, Alfred Vulpian and Louis Ranvier, reported on several observations of general pathology and neuropathology. After his chief residency under Charcot (from 1885 to 1887), Babinski abandoned his previous interests and became increasingly involved with hysteria. He thus

Hyperactive pectoralis reflex as an indicator of upper cervical spinal cord compression. Report of 15 cases.

Myelopathy from cervical spondylosis is often accompanied by hyperreflexia of the upper-extremity deep tendon reflexes (DTRs). Reflexes such as the pectoralis jerk and the deltoid jerk may only be apparent in the context of hyperreflexia. Although the nerve roots involved in the reflex arcs are well described, levels of cervical spinal cord compression that lead to the hyperreflexia are not as clear. This is of particular significance for patients with multilevel cervical spondylosis in determining the levels responsible for their symptoms. The authors examined 15 consecutive patients who pres

[Combination big-toe stretch reflex for the detection of latent pyramidal signs].

The stretch-reflex of the extensor hallucis muscle will only become apparent when in case of an upper motor neuron lesion the extensor hallucis response is stimulated simultaneously. This extrinsic-intrinsic combinative reflex may be useful to elicit a latent extensor response or to prove a doubtful one.

Adolph Seeligmüller (1837-1912) and the first graphic illustration of the "toe phenomenon" (later called "Babiński sign") in the medical literature.

The "toe phenomenon" refers to the extension (dorsiflexion) of the great toe, which occurs instead of the normal flexion following stimulation of the foot sole. Its clinical significance was not fully appreciated until Joseph Jules François Félix Babiński (1857-1932) described it in 1896. In 1881, Ernst Strümpell (1853-1925) had described a continuous (tonic) extension of the big toe, a finding that years later the French neurologist Jean-Athanase Sicard (1872-1929) recognized as an equivalent of the "toe phenomenon", also indicating pyramidal tract dysfunction. Previously, this phenomenon had

[Babinski and Chaddock signs without apparent pyramidal disfunction].

PURPOSE: The main objective of this study was to verify, in one hundred in-patients from the Serviço de Clínica Médica do Hospital Universitário Clementino Fraga Filho da Universidade Federal do Rio de Janeiro who did not have a history of clinical symptoms of pyramidal disfunction, the presence of the Babinski and Chaddock signs. As a secondary objective, we looked for a prevalence of one of the signs over the other, and the influence of the head position regarding the obtained responses. METHOD: The patients were examined while supine with their heads in three different positions. RESULTS:

The Utility of a Prediction Model Using Neurological Examination Findings for Diagnosing Degenerative Cervical Myelopathy.

BACKGROUND: The diagnostic accuracy of neurological examination findings for identifying degenerative cervical myelopathy (DCM) is not apparent, given the paucity of studies with appropriate control groups. In order to address this knowledge gap, we conducted a community cervical spine screening project and examined subjects without DCM or evidence of myelopathy on cervical magnetic resonance imaging (MRI). METHODS: This study included a total of 229 patients diagnosed with DCM, based on MRI evidence of spinal cord compression and improvement after surgery, and 807 controls without DCM (40 to

HTLV-I associated myelopathy in the northern region of Brazil (Belém-Pará): serological and clinical features of three cases.

Three patients (males, black, ages 37, 40 and 57) attended a university clinic with a progressive paraparesis of obscure origin. One patient who referred disease duration of more than 16 years, showed diminished deep reflexes, bilateral Babinski's sign, diminished sensation of vibration, abnormal bladder function and back pain. The other two patients (with one and six years of disease duration) complained of weakness in one leg, increased deep reflexes and back pain. Babinski's sign and bladder disturbance were also present in the patient with six years of disease. Blood samples tested by an e

[Regarding the clinical diagnosis of the monotopical spinal forms of multiple sclerosis. The value of the fan sign in the adult].

INTRODUCTION: We wish to discuss the value of the clinical history and examination in orientation of the diagnosis of probable multiple sclerosis (MS). CLINICAL CASE: We report the two year study of a woman who over the previous ten years had had three episodes of paraesthesia, with pins and needles in her left leg and other parts of the left side of her body, although never affecting head or neck. She also complained of tiring more than usual. In an outpatient clinic she was found to have a syndrome affecting the upper segments of the spinal cord, mainly involving the right side and resembli

[Crossed plantar extension. A clinical and physiological study (author's transl)].

A 44 year-old man was examined for a slowly progressive left hemiplegia predominating on the lower limb. A left sign of Babinski was present. On the right side the plantar response was flexor. Stimulation of the right sole elicited a left extensor plantar response. CT Scan showed calcification sin the right paracentral lobule. The variations of the contralateral soleus H reflex when conditioned by a nociceptive stimulus applied on the right little toe were studied. No modification was seen. This contrasts with the facilitation observed in normal subjects and in patients with a lesion of the ce
