Participation of older newly-diagnosed cancer patients in an observational prospective pilot study: an example of recruitment and retention.

BackgroundThere have been few prospective observational studies which recruited older newly-diagnosed cancer patients, and of these only some have reported information on the number needed to screen to recruit their study sample, and the number and reasons for refusal and drop-out. This paper reports on strategies to recruit older newly-diagnosed cancer patients prior to treatment into an observational prospective pilot study and to retain them during a six-month period.MethodsMedical charts of all patients in the Segal Cancer Centre aged 65 and over were screened and evaluated for inclusion.

Ethical issues in the care of adolescent and young adult oncology patients.

The diagnosis and treatment of cancer leads to short-term and long-term challenges for every patient. This is especially true for adolescents and young adults (AYAs) with cancer who strive to gain independence, autonomy, confidence, and social status while developing into adulthood. In this article, we review prominent ethical issues in AYA oncology that are related to autonomy, shared decision-making, care refusal or abandonment, end-of-life care, truth telling, and fertility preservation. Clinicians should recognize that AYA patients develop at their own pace; the onus lies with clinicians t

Ethics for the pediatrician: obstetric conflict: when fetal and maternal interests are at odds.

Obsessive-compulsive disorder (OCD) is considered one of the most debilitating disorders worldwide. Data suggests that trauma may be a significant source of not only posttraumatic stress disorder (PTSD), but also OCD. An epidemiologic study of the general population found the risk for OCD to be increased 10-fold in persons with PTSD. The National Vietnam Veterans Readjustment Study found the prevalence of OCD to be 5.2% in war-zone veterans and other studies highlighted PTSD-OCD comorbidity rates ranging from 41% to 70%. However, the frequency of diagnosed OCD is low among veterans (0.6%), and

Higher refusal rates for organ donation among older potential donors in the Netherlands: impact of the donor register and relatives.

BACKGROUND: The availability of donor organs is considerably reduced by relatives refusing donation after death. There is no previous large-scale evaluation of the influence of the Donor Register (DR) consultation and the potential donor's age on this refusal in The Netherlands. METHODS: This study examines 2101 potential organ donors identified in intensive care units between 2005 and 2008 and analyzes the association of DR consultation and subsequent refusal by relatives and the relationship with the potential donor's age. RESULTS: Of the 1864 potential donor cases where the DR was consult

A Systematic Review of Pharmacologic Treatments for School Refusal Behavior.

OBJECTIVE: School refusal is an important pediatric problem with significant negative short- and long-term outcomes. Specific psychosocial treatments appear effective in reducing school refusal, but many children do not respond to these treatments. Although systematic reviews have examined the efficacy of psychological interventions for school refusal, no systematic reviews on pharmacological interventions exist. METHODS: We conducted a comprehensive literature search of MEDLINE, PsycINFO, Scopus, and Embase for randomized controlled trials (RCTs) or quasi-experimental pharmacologic trials in

Heterosexual anal intercourse among Black and Latino adolescents and young adults: a poorly understood high-risk behavior.

The purpose of this study was to gain a better understanding of the context of heterosexual anal intercourse (HAI) among adolescents. Black and Latino youth were recruited at an urban college and an inner-city adolescent clinic. Participants completed a sexual behavioral questionnaire and the Sexual Relationship Power Scale (SRPS). A total of 61 young people, all of whom were sexually experienced, completed the survey (53 females; 8 males). Of these respondents, 20% reported engaging in HAI (N = 12), and 50% reported HAI refusal. The Relationship Control subscale scores of the SRPS were signif

Informed consent to breech birth in New Zealand.

The authors note significant room for improvement in facilitating informed consent in the management of breech presentation. New Zealand maternity care providers, including midwives, general practitioners and specialist obstetricians, have legal duties to provide full and unbiased information about risks and benefits of all relevant treatment options. In the case of breech presentation, such options include the interventions of external cephalic version or planned caesarean section, as well as the option to decline intervention and proceed with a planned vaginal breech birth. Information shoul

Conscience clauses offer little protection. Most are deficient, and many have been met with hostile judicial interpretations.

Although the federal government and most states have enacted laws protecting healthcare providers' rights to refuse to provide or participat … ights of conscience can be expected from three sources. First, some medical schools have considered refusal to participate as a negative factor in the admission process. Second, greater financial pressures on the American healthcare system may cause nonmonetary factors to be sacrificed to the exigencies of the moment. Third, all healthcare reform plans propose an increa …
