[Pediatric cardiopulmonary resuscitation].

Our aim is to summarize the new European Resuscitation Council (ERC) 2021 guidelines on paediatric life support. In children, exhaustion of compensatory mechanisms in respiratory or circulatory failure leads to cardiac arrest. Recognition and treatment of children in critical condition are the most important element of its prevention. With the ABCDE approach, life-threatening problems can be identified and treated using simple interventions (bag-mask ventilation, intraosseous access, fluid bolus, etc.). Important new recommendations: 4-hand ventilation during bag-mask ventilation, target satur…

Damage control resuscitation.

The United States Navy originally utilized the concept of damage control to describe the process of prioritizing the critical repairs needed to return a ship safely to shore during a maritime emergency. To pursue a completed repair would detract from the goal of saving the ship. This concept of damage control management in crisis is well suited to the care of the critically ill trauma patient, and has evolved into the standard of care. Damage control resuscitation is not one technique, but, rather, a group of strategies which address the lethal triad of coagulopathy, acidosis, and hypothermia.…

Pediatric Burn Resuscitation.

Children have unique physiologic, physical, psychological, and social needs compared with adults. Although adhering to the basic tenets of burn resuscitation, resuscitation of the burned child should be modified based on the child's age, physiology, and response to injury. This article outlines the unique characteristics of burned children and describes the fundamental principles of pediatric burn resuscitation in terms of airway, circulatory, neurologic, and cutaneous injury management.

Endpoints of resuscitation.

Despite the multiple causes of the shock state, all causes possess the common abnormality of oxygen supply not meeting tissue metabolic demands. Compensatory mechanisms may mask the severity of hypoxemia and hypoperfusion, since catecholamines and extracellular fluid shifts initially compensate for the physiologic derangements associated with patients in shock. Despite the achievement of normal physiologic parameters after resuscitation, significant metabolic acidosis may continue to be present in the tissues, as evidenced by increased lactate levels and metabolic acidosis. This review discuss…

[Basic cardiopulmonary resuscitation].

Since the introduction of the modern techniques for cardiopulmonary resuscitation in the 1960s, professionals have discussed the need to standardize its application and teaching both among health care professionals and the general public. The "ILCOR", Committee to Coordinate Resuscitation Techniques, in an effort to simplify resuscitation techniques, set out some recommendations in August 2000 which were adopted by the leading organizations such as AHA and ERC, in charge of diffusing Vital Suport techniques. In these, different levels of attention have been incorporated depending on the qualif…

Cardiopulmonary cerebral resuscitation.

Cardiopulmonary arrest (CPA) is defined as the abrupt and unexpected cessation of spontaneous and effective ventilation and circulation. CPA can be the natural ending of a normal and long life; however, when CPA is the result of a reversible problem in an animal that has a treatable medical condition, rapid recognition and treatment may make the difference between a happy ending and premature death. Cardiopulmonary resuscitation provides artificial ventilation and circulation until advanced cardiac life support can be provided and spontaneous cardiopulmonary function is restored. The term card…

Exclusion from resuscitation.

Resuscitation was attempted for 156 (19.9%) of the 783 cases in St Thomas's Hospital, aged 16 or over, who suffered first cardiac or respiratory arrest of the current admission during the year to 13 April 1987. Seventeen (11%) of the 156 were discharged alive from hospital after resuscitation. All seventeen had been active outside the home before admission, and all except one remained so after discharge. The factors which were associated with exclusion from resuscitation attempts are analysed. Most accorded with known prognostic factors, but resuscitation was attempted for some persons who see…

Resuscitation of the newborn.

The simple mnemonic ABCDE summarizes the five key principles of neonatal resuscitation: 1. Airway 2. Breathing 3. Circulation 4. Drugs 5. Evaluation of concurrent and causative problems and maintenance of a neutral thermal Environment.

Resuscitation of the newborn.

Resuscitation of the newborn is not different from resuscitation of any other patient, but in newborns, resuscitation requires special skills and considerations because of the size of the patient. If properly done, it can save many lives. If done incorrectly, however, it can save many lives. If done incorrectly, however, it can result in death or permanent neurologic damage to the infant. Because it is difficult to predict the status of the infant before birth, any delivery room must have experienced personnel and adequate equipment necessary for the resuscitation of the newborn.

Principles of Resuscitation.

Shock from all causes carries a high mortality. Rapid and intentional intervention to resuscitate can reduce mortality and organ injury. Approaches to fluid resuscitation, vasopressor use as well as commonly assessed laboratory values are reviewed in this paper.

Endpoints in resuscitation.

PURPOSE OF REVIEW: Shock occurs because of a failure to deliver adequate oxygen to meet the metabolic demands of the body resulting in metabolic acidosis, inflammation, and coagulopathy. Resuscitation is the process of treating shock in an attempt to restore normal physiology. Various hemodynamic, metabolic, and regional endpoints have been described to evaluate the degree of shock and guide resuscitation efforts. We will briefly describe these endpoints, and propose damage control resuscitation as an additional endpoint. RECENT FINDINGS: Serum lactate, base deficit, and pH are well establish…

Hypovolemic shock resuscitation.

Several changes in the way patients with hemorrhagic shock are resuscitated have occurred over the past decades, including permissive hypotension, minimal crystalloid resuscitation, earlier blood transfusion, and higher plasma and platelet-to-red cell ratios. Hemostatic adjuncts, such as tranexamic acid and prothrombin complex, and the use of new methods of assessing coagulopathy are also being incorporated into resuscitation of the bleeding patient. These ideas have been incorporated by many trauma centers into institutional massive transfusion protocols, and adoption of these protocols has r…
