Retinal artery occlusion in migraine.

Retinal artery occlusion is a rare complication of migraine. The following case report highlights the importance of early diagnosis in this condition. The long term management of patients with this and other forms of complicated migraine is discussed.

Partial central retinal artery occlusion.

The clinical picture of multiple circumpapillary cotton-wool spots and mild ischaemic swelling of the inner retina is attributed to partial occlusion of the central retinal artery. The pathophysiological basis of the fundus signs is discussed and the role of interrupted axoplasmic transport in the development of the cotton-wool spots is emphasized.

[Retinal arterial macroaneurysms].

Nineteen eyes of 17 patients were examined for complications associated with a retinal macroarterial aneurysm. 13 of 17 patients were feminine and over 70 years of age and 15 had longstanding systemic hypertension. Visual loss associated with retinal macroarterial aneurysms was related to macular hemorrhage, exudate, or serous retinal detachment. Twelve eyes had retinal hemorrhages associated with the macroarterial aneurysm. Hemorrhages were subretinal and/or preretinal of which some diffused into the vitreous. Seven eyes were treated by direct argon laser photocoagulation of the aneurysm givi

Sequential bilateral retinal artery occlusion.

An 86 year old woman experienced a sequential bilateral loss of vision over a period of less than 24 hours. Clinical findings and complementary studies suggested a bilateral atherogenic embolic event. Initially, she presented a superior branch retinal artery occlusion in her right eye followed by a central retinal artery occlusion with cilioretinal artery sparing in her left eye. Some conservative maneuvers performed did not improve visual acuity in the left eye. Supra-aortic Doppler ultrasonography revealed mild right internal carotid artery stenosis and moderate left internal carotid artery …

[Occlusion of the retinal artery].

The occlusion of the retinal artery usually leads to sudden and, in most cases, continual visual impairment of the affected eye. This is an uncommon illness with a variable progression. To date it has not been possible to develop a generally recommended therapy using prospective, randomised studies, as is the case for circulatory impairment in other organs. Due to this uncertainty, the treatment of a retinal arterial occlusion is regarded as an ophthalmological emergency and a challenge for the attending practitioner. In this overview, the actual state of our knowledge on the pathogenesis of o…

Retinal arterial macroaneurysms.

Retinal arterial macroaneurysms represent a distinct clinical entity. Macroaneurysms are seen in the elderly with a marked female predominance and a strong association with hypertension and arteriosclerotic vascular changes. The classic appearance provides an easy diagnosis; however, variable presentations, such as subretinal hemorrhage, macular exudate, and epiretinal membranes can make the diagnosis difficult. The differential diagnosis of retinal arterial macroaneurysms include retinal telangiectasia, angiomatosis retinae, venous macroaneurysms, background diabetic retinopathy, and cavernou

Macroaneurysms of the retinal arteries.

Six patients with spontaneous retinal and/or vitreous hemorrhages were found, on evaluation by fluorescein angiography, to have macroaneurysms involving the major retinal arteries. All showed evidence of other vascular disease. With absorption of the hemorrhages, five patients experienced appreciable improvement in vision. The aneurysms appeared to diminish in size as a result of fibrous changes within the arterial walls. In one patient, however, a second vitreous hemorrhage occurred several months later. The possibility of repeated hemorrhage suggests that more consideration should be given t

Intra-retinal Arterial Cannulation using a Microneedle for Central Retinal Artery Occlusion.

Central retinal artery occlusion (CRAO) is a severe disease, often causing blindness. We evaluated the efficacy and safety of a surgical procedure for the treatment of acute CRAO in which retinal arterial cannulation with tissue plasminogen activator (tPA) is performed. The surgical procedure consisted of vitrectomy followed by cannulation of the central retinal artery and injection of tPA (200 μg) using a 47-gauge microneedle. Thirteen CRAO patients were treated within 48 hours of the onset of symptoms. The central retinal artery of all 13 eyes was successfully cannulated. The mean interval b…

Central retinal artery occlusion or retinal stroke: a neurosonologist's perspective.

In central retinal artery occlusion (CRAO) or retinal stroke, which is usually a vision-threatening condition, timely diagnosis is imperative to improve the chances of retinal preservation and to establish adequate secondary prevention measures. Even though retinal strokes have been traditionally assigned to the field of ophthalmology, while considering reperfusion therapy as the only way to avoid permanent vision loss, we suggest prompt evaluation of CRAO causes (primarily related to cardiovascular risk factors) performed by a well-organized interdisciplinary team (ophthalmologist and neurolo…

Periarteriolar-Sparing Retinal Edema in Acute Central Retinal Artery Occlusion.

A 56-year-old man was referred to our hospital about 48 h after sudden onset of painless central visual loss in his right eye (OD) on a winter morning. He had a more than 25-year history of systemic hypertension and smoking. Funduscopic observation showed striated retinal whitening in the macular region, faint cotton-wool patches around the optic disc, and segmental narrowing of the retinal arteries near the optic disc edge. Based on the pattern of onset and fundus findings, he was diagnosed with vasospastic acute central retinal artery occlusion OD, and a venous drip injection of prostaglandi…

Central Retinal Artery Occlusion With Triple Cilioretinal Artery Sparing.

We report a rare case of central retinal artery occlusion (CRAO) with triple cilioretinal artery sparing in a 76-year-old male with hypertension who presented with sudden diminution of vision in the left eye (OS) for one day. Optical coherence tomography angiography (OCTA) demonstrated the presence of three cilioretinal arteries and the absence of flow signals in the rest of the macula. Primary ophthalmic treatment was instituted immediately in the form of ocular massage, and acetazolamide 500 mg per oral (PO) stat was given. Systemic investigations revealed a significant blockage in coronary …

Carotid artery disease in patients with retinal artery occlusion.

Embolization from the carotid bifurcation has been proposed as the most common cause of central retinal artery occlusion (CRAO) and branch retinal artery occlusion (BRAO). The purpose of this study was to evaluate carotid artery disease in patients with CRAO and BRAO. Using carotid ultrasonography, 17 patients (13 males, 4 females, mean age 68.7 +/- 7.1 years) with CRAO and BRAO were examined for stenotic findings and plaque morphology (homogeneous or heterogeneous) within 7 days after onset. The internal carotid artery (ICA) ipsilateral to the affected side showed a significantly higher incid…

Visual outcome in central retinal and branch retinal artery occlusion.

PURPOSE: To study retrospectively the presenting visual acuity and the visual outcome in patients with central retinal artery occlusion (CRAO) and in patients with branch retinal artery occlusion (BRAO). METHODS: We studied the visual acuity and outcome in 23 patients (23 eyes) with CRAO and in 30 patients (30 eyes) with BRAO that met the inclusion criteria: a funduscopic appearance of retinal whitening, a delay in arterial dye filling in a fluorescein angiogram, the first examination in our hospital within 7 days of onset, and a minimum follow-up period of 90 days. RESULTS: Both presenting …
