Risk-Adjustment of Perioperative Mortality Rate Measurement in a Low-Income Country.

BACKGROUND: The health care systems in low-income countries have extremely limited capacity to treat surgical diseases. The perioperative mortality rate has been suggested as a key quality metric to guide the expansion of care, but there is little information on how to risk-adjust this outcome measure. METHODS: We did a 42-month observational cohort study of surgical operations at a Ugandan secondary referral hospital. We examined factors associated with in-hospital 30-day perioperative mortality outcomes. The aim of the study was to suggest a suitable indicator metric for comparative health

Improving Risk Adjustment in the Assessment of Congenital Heart Center Surgical Quality.

BACKGROUND: Understanding center performance in congenital heart surgery remains challenging, with limitations to standard regression approaches. Modern causal inference methods may improve estimation of expected mortality through better balancing case mix but have not been studied. METHODS: Benchmark operations across 115 centers (2016-2022) from The Society of Thoracic Surgeons Congenital Database were included. The standard approach uses mixed-effects logistic regression to estimate centers' expected operative mortality inclusive of all patients, even those treated at the center of interes

Risk Adjustment Is Necessary in Value-based Outcomes Models for Infected TKA.

BACKGROUND: The Medicare Access and CHIP Reauthorization Act of 2015 provides the framework to link reimbursement for providers based on outcome metrics. Concerns exist that the lack of risk adjustment for patients undergoing revision TKA for an infection may cause problems with access to care. QUESTIONS/PURPOSES: (1) After controlling for confounding variables, do patients undergoing revision TKA for infection have higher 30-day readmission, reoperation, and mortality rates than those undergoing revision TKA for aseptic causes? (2) Compared with patients undergoing revision TKA who are belie…

Adherence and psychological adjustment among women at high risk for breast cancer.

Increasingly, women with a positive family history of breast cancer are being targeted for cancer prevention and control efforts. Two findings have been demonstrated consistently across studies of this high risk population. First, these women often have high levels of psychological distress, as well as persistent and intrusive worries about developing breast cancer. Second, despite their increased risk for developing breast cancer, a substantial proportion of these women do not adhere to recommended breast cancer screening guidelines. There is growing evidence that psychological distress is an

Level of education and multiple sclerosis risk after adjustment for known risk factors: The EnvIMS study.

BACKGROUND: Several recent studies have found a higher risk of multiple sclerosis (MS) among people with a low level of education. This has been suggested to reflect an effect of smoking and lower vitamin D status in the social class associated with lower levels of education. OBJECTIVE: The objective of this paper is to investigate the association between level of education and MS risk adjusting for the known risk factors smoking, infectious mononucleosis, indicators of vitamin D levels and body size. METHODS: Within the case-control study on Environmental Factors In MS (EnvIMS), 953 MS pati

Quantifying the impact of time-varying baseline risk adjustment in the self-controlled risk interval design.

PURPOSE: The self-controlled risk interval design is commonly used to assess the association between an acute exposure and an adverse event of interest, implicitly adjusting for fixed, non-time-varying covariates. Explicit adjustment needs to be made for time-varying covariates, for example, age in young children. It can be performed via either a fixed or random adjustment. The random-adjustment approach can provide valid point and interval estimates but requires access to individual-level data for an unexposed baseline sample. The fixed-adjustment approach does not have this requirement and w

Risk-adjusted training and evaluation for breast cancer detection.

Breast cancer detection, and broadly medical object detection, revolves around discovering and rating lesions. One of the most common ways of measuring performance is FROC (Free-response Receiver Operating Characteristic), which calculates sensitivity at predefined thresholds of false positives per case. However, depending on the clinical context, not all lesions might be of equivocal impact on the long-term outcome of a patient. Some lesions missed e.g. in screening might be detected in the subsequent screening round without impacting the clinical prognosis, whilst missing others might signif

Resilience in the aftermath of trauma: Classes of adjustment in at-risk youth.

OBJECTIVE: Youth who have been exposed to potentially traumatic events are at risk of developing chronic and severe adjustment problems. However, some youth exposed to potentially traumatic events exhibit better-than-expected adaptation. Person-centered analyses, based on multiple criteria for adaptive behavior and adversity, are well-suited to empirically identifying different patterns of adaptation among high-risk youth. METHOD: Data from 381 youth from three at-risk subgroups in Portugal were collected using self-reported measures of trauma history, trauma-related symptomatology, coping sk

Risk-adjusted outcomes of inpatient medicare medical admissions.

It is important that actual outcomes of care and not surrogate markers, such as process measures, be used to evaluate the quality of inpatient care. Because of the heterogenous composition of patients, risk-adjustment is essential for the objective evaluation of outcomes following inpatient care. Comparative evaluation of risk-adjusted outcomes can be used to identify suboptimal performance and can provide direction for care improvement initiatives. We studied the risk-adjusted outcomes of 6 medical conditions during the inpatient and 90-day post-discharge period to identify the opportunities f

Rhinos and risk assessments: Adjusting risk assessment methodologies to account for 'unforeseeable' events.

It is wrong to describe the COVID-19 pandemic as a 'black swan' (ie a catastrophic event that no one saw coming). Far more apt would be to call it a 'grey rhino' - something that has long been predicted but overlooked by leadership the world over. This paper argues that it is time to stop relying on outdated risk formulas and adjust risk assessment methodologies to account for these grey rhinos. Simply put, it is time to accept that the potential impact of an event is more important than its likelihood.

Is adjustment disorder genetically correlated with depression, anxiety, or risk-tolerant personality trait?

Adjustment disorder has three main subtypes: adjustment disorder with depressed mood, adjustment disorder with anxiety, and adjustment disorder with disturbance of conduct. The disorder is moderately heritable and has lifetime comorbidities with major depressive disorder (MDD), anxiety disorders, or risk-tolerant personality. However, it remains unclear whether the degrees of genetic correlations between adjustment disorder and other psychiatric disorders and intermediate phenotypes are similar or different to those between MDD, anxiety disorders or risk-tolerant personality and these other ps

Parental Empathy, Aggressive Parenting, and Child Adjustment in a Noncustodial High-Risk Sample.

This study examined the relations between parental empathy, parenting physical aggression, parental psychological control, and child adjustment in a sample of parents who had their children removed from their custody because of child abuse or neglect. Twenty parents between 24 and 40 years of age (M = 31.15, SD = 4.85; 85% female) with a child aged between 1.5 and 16 years (M = 6.5, SD = 3.88; 70% boys) participated in the study. Our sample was comprised of relatively racially diverse and low-income parents, with 40% from racial minority groups and 70% below the poverty line. Parents were recr

Heterogeneous responses to risk-adjustment reform: evidence from dementia diagnosis in Medicare Advantage.

INTRODUCTION: In 2020, Alzheimer's disease and related dementias (ADRD) diagnoses were reintroduced into the Medicare Advantage (MA) risk-adjustment model creating financial incentives that may increase diagnostic coding and plan payments. This effect may vary across plans depending on provider integration, administrative capacity, and geographic context, with implications for payments and potential "upcoding." METHODS: Using 100% Medicare data from 2016-2021, we estimated event study and difference-in-differences models comparing adjusted trends in incident ADRD diagnoses in MA relative to t

Using risk adjustment to improve the interpretation of global inpatient pediatric antibiotic prescribing.

OBJECTIVES: Assessment of regional pediatric last-resort antibiotic utilization patterns is hampered by potential confounding from populatio … k model. The crude regional pCA exposure rates ranged from 10.3% (Africa) to 67.4% (Latin America). Risk adjustment substantially reduced the regional variation, the adjusted rates ranging from 17.1% (Africa) to 42.8% (Latin America). CONCLUSIONS: Greater comparability of pCA exposure rates can be achieved by using a few easily collected variables to produce ris …

Risk and resistance factors associated with paternal adjustment to obstetrical brachial plexus injuries.

This study aimed to identify risk and resistance factors associated with the psychological adjustment of fathers of children with obstetrical brachial plexus injuries (OBPI). Participants were 34 fathers of children with OBPI recruited from an Australian OBPI clinic. Measures completed were OBPI severity, disability related stress, family functioning and social support. Together the risk and resistance factors of severity and family functioning accounted for 28% of the total variance in paternal psychological adjustment. Family functioning explained 12% of the variance in psychological adjustm

Effortful control, social competence, and adjustment problems in children at risk for psychopathology.

This study explored the factor structure and developmental trajectory of effortful control (EC), its relations with child adjustment, and the moderating role of age and gender in 75 4- to 6-year-old children at risk for psychopathology. Confirmatory factor analyses revealed two subcomponents of effortful control: Suppress/Initiate (the ability to inhibit a dominant response while initiating a new response) and Motor Control (inhibiting fine and gross motor activity). EC performance improved with age, and both subcomponents were associated with greater social competence at all ages. Association
