Weight-loss dynamics with tirzepatide versus semaglutide.

Glucagon-like peptide-1 receptor agonists (GLP-1RAs) are widely used for obesity and type 2 diabetes, yet substantial variability in weight-loss response remains poorly understood. We conducted a retrospective cohort study using de-identified electronic health records and performed 1:1 propensity matching on age, sex, type 2 diabetes status, baseline BMI and weight, index year, and follow-up duration. The matched cohorts included 10,339 tirzepatide-treated and 10,339 semaglutide-treated patients. Patients were categorized by maximum weight loss over 2 years into five response groups. Adverse e…

Semaglutide and Nonarteritic Anterior Ischemic Optic Neuropathy.

IMPORTANCE: Semaglutide, a glucagonlike peptide-1 receptor agonist (GLP-1RA), has recently been implicated in cases of nonarteritic anterior ischemic optic neuropathy (NAION), raising safety concerns in the treatment of type 2 diabetes (T2D). OBJECTIVE: To investigate the potential association between semaglutide and NAION in the Observational Health Data Sciences and Informatics (OHDSI) network. DESIGN, SETTING, AND PARTICIPANTS: This was a retrospective study across 14 databases (6 administrative claims and 8 electronic health records). Included were adults with T2D taking semaglutide, oth…

Semaglutide-induced Wernicke encephalopathy: a comprehensive analysis.

Although GLP-1 receptors analogues (RAs) benefits-risks profile has been largely documented in diabetes, higher dosages recently approved in obesity still require further assessment. We describe here the case of a 49-year-old female patient treated with semaglutide for obesity, who presented with Wernicke encephalopathy in a context of iterative vomiting and reduced food intake. Eighteen other cases of Wernicke encephalopathy were reported in literature and in the WHO global safety database (VigiBase). A context of nausea/vomiting or reduced food intake is described in 68% of cases, with weigh…

Semaglutide and health risk: Development and validation of a LC-HRMS method for testing semaglutide in whole blood and application to real cases.

The use of semaglutide, also known by its trade name Ozempic®, has been increasing worldwide in recent years due to its benefits in treating type II diabetes. Thanks to its effects on appetite regulation, in many countries it is also used to treat obesity. However, due to its promotion by social media and celebrities as a weight-loss treatment, semaglutide is misused by a non-diabetic and non-obese population and by a young public, which is the main target of these media. Following the alert by the ANSM (Agence nationale de sécurité du médicament) in France and the FDA (Food and Drug Administr…

Butyrate enhances the obesity reduction effect of Semaglutide.

BACKGROUND: Obesity represents a global public health burden with continuously increasing prevalence over recent decades. Combination regimens pairing semaglutide (Sema) with additional therapeutic agents have emerged as a major focus for anti-obesity research. METHODS: In diet-induced obese (DIO) mice, we evaluated whether oxytocin (Oxt, 3 and 5 mg·kg-1), long-acting oxytocin (la‑Oxt), caffeine, or butyrate could potentiate the anti-obesity efficacy of Sema. We further compared hypothalamic neuronal activity and plasma metabolomic profiles following Sema treatment against dietary restriction…

Negative effects of semaglutide on bone in obese mice.

GLP1 receptor agonists, such as semaglutide, are used in the management of obesity. While causing weight loss, these drugs reduce both fat and lean mass. The impact of GLP1 receptor agonists on bone health has not been explored. Femur mass, stiffness, and strength are lower in obese male C57BL/6J mice treated with semaglutide, and these effects are more pronounced than in mice that are calorically restricted to the same extent. Decrements in bone are paralleled by lower circulating levels of markers of osteoblast activity. In an additional cohort, femur weight, strength, and stiffness are the …

Semaglutide for Refractory Reactive Hypoglycaemia: A Case Report.

Reactive hypoglycaemia is characterised by symptomatic hypoglycaemia occurring several hours after food intake and can significantly impair quality of life. Management is often challenging, particularly in patients who fail dietary modification or cannot tolerate conventional pharmacological therapies. We present the case of a 50-year-old woman with longstanding, debilitating reactive hypoglycaemia who experienced marked symptomatic and biochemical improvement following treatment with semaglutide, a glucagon-like peptide-1 (GLP-1) receptor agonist. The patient presented with recurrent episodes…

Semaglutide: Double-edged Sword with Risks and Benefits.

Type 2 Diabetes Mellitus therapy has evolved over the years to now include a new class of therapeutics, semaglutide. This article reviews the mechanism of action and formulation of semaglutide therapy, potential benefits, contraindications, adverse effects, and drug interactions. Oral and subcutaneous semaglutide therapies have shown effectiveness in improving glycemic control, weight loss, and reducing cardiovascular risks associated with diabetes mellitus. Semaglutide has also shown potential in being used as a therapeutic strategy in Alzheimer's disease due to its anti-neuroinflammatory eff…

Tendency of Semaglutide to Induce Gastroparesis: A Case Report.

Semaglutide, an agonist of the glucagon-like peptide-1 receptor, is frequently used in the treatment of diabetes mellitus type 2, although, lately, weight loss has additionally become a reason for its use. However, if a patient is already experiencing bloating, nausea, abdominal pain, and discomfort in the abdomen, it is not recommended to use it due to concern about aggravating these symptoms. Although it is often well tolerated, there are occasions when it can have several gastrointestinal side effects. Therefore, we report a case of a patient who started taking semaglutide and later develop…

Predictors of treatment response to semaglutide for weight management.

The emergence of weekly, injectable glucagon-like peptide-1 (GLP-1) receptor agonists has revolutionized the obesity management landscape. Numerous studies have shown high efficacy as well as great variability in weight loss, highlighting the need for this mini review, summarizing current evidence about predictors of semaglutide-related weight loss. The first well-known determinant of semaglutide-induced weight loss is the presence of type 2 DM which is associated with significantly lower body weight reduction. The second predictor is sex, with females exhibiting a greater weight loss than mal…

Postmenopausal bleeding associated with semaglutide: a diagnostic challenge.

Semaglutide, a glucagon-like peptide-1 (GLP-1) receptor agonist, is widely used for obesity and type 2 diabetes management, but its gynecological adverse effects have not been systematically described. We report a 58-year-old postmenopausal woman with new-onset vaginal bleeding six weeks after starting semaglutide for weight reduction, with onset following dose escalation to 0.5 mg weekly. Hemoglobin fell from 12.9 g/dL (Système International [SI]: 129 g/L) to 10.8 g/dL (SI: 108 g/L) (reference range, 12.0-16.0 g/dL [SI: 120-160 g/L]). The serum follicle-stimulating hormone (FSH) concentration…
