Reverse shoulder arthroplasty (RSA) is being increasingly employed for complex proximal humeral fractures (PHFs) in older patients, particularly in cases with severe osteoporosis. However, bilateral PHFs and subsequent periprosthetic fractures remain rare and challenging. We report a case of a 60-year-old woman with alcoholic liver disease who sustained bilateral PHFs after a fall. Due to her compromised condition, staged bilateral RSA was performed. The initial postoperative course was uneventful. Eight months after the first surgery, she sustained a displaced periprosthetic fracture on the r

A technique using a cannulated tap in a percutaneous manner allows for easy reduction of the dislocated humeral head with minimal soft-tissue trauma and presents a useful alternative to repeated attempts at open or closed reduction.

Fracture-dislocations of the shoulder can present difficulties in treatment. Two cases are described to illustrate the complications that can occur when an inexperienced examiner attempts a closed reduction of a two-part anterior fracture-dislocation of the shoulder. Careful clinical and radiologic evaluation (A-P and transcapular or axillary lateral X-rays) followed by appropriate management can help to prevent these problems from occurring. If there is evidence that the fracture is displaced by manipulation, immediate open reduction and internal fixation are recommended.

We report the case of a man aged 19 who sustained an anterior dislocation of the shoulder with a fracture of the greater tuberosity, and a fracture of the shaft of the same humerus. Open reduction and internal fixation of the shaft fracture was carried out followed by closed manipulation of the dislocation. He recovered full function of the shoulder and elbow.

The posterior luxation of the shoulder joint is a rarely reported and often not recognized lesion in the clinical workday. The authors present the first case of a posterior luxation of the shoulder joint in combination with an contralateral anterior shoulder joint fracture dislocation. In that case the authors stress the importance of an exact clinical and radiology diagnostic and the right way to reduce a posterior luxation of the shoulder joint. Finally they draw the readers attention to operative steps to prevent a reluxation of the shoulder joint.

BACKGROUND: There are multiple reported injuries associated with cardiopulmonary resuscitation, most of them caused by the force of compressions, like sternal and rib fractures, abdominal organ injuries like splenic rupture, liver lacerations, and injuries to the upper airway and skin. Injuries related to defibrillation and cardioversion are rare, mostly related to skin and muscle injuries on where the defibrillation paddles were placed.
CASE REPORT: A 52-year-old man presented to the Emergency Department with crushing chest pain. The patient was suffering from a myocardial infarction, and du

BACKGROUND: Despite extensive literature dedicated to determining the optimal treatment of isolated greater tuberosity (GT) fractures, there have been few studies to guide the management of GT fracture dislocations. The purpose of this review was to highlight the relevant literature pertaining to all aspects of GT fracture dislocation evaluation and treatment.
METHODS: A narrative review of the literature was performed.
RESULTS: During glenohumeral reduction, an iatrogenic humeral neck fracture may occur due to the presence of an occult neck fracture or forceful reduction attempts with inade

Posterior dislocation of the shoulder is an uncommon injury. Diagnosis is difficult and often missed. Once diagnosed, management must be individualized depending on the amount of the defect of the humeral head and the time from injury. This article presents a case of a 40-year-old man with a 4-month history of bilateral locked posterior fracture-dislocation of the shoulders after a grand mal seizure. Imaging showed loss of the glenohumeral joint lines congruency, reverse Hill-Sachs lesions, and articular defects of 35% and 40% of the humeral heads. A modified McLaughlin technique was performed

INTRODUCTION: Standard rehabilitation regime following hemiarthroplasty for trauma is early mobilisation to prevent the development of a stiff shoulder. However, an aggressive early rehabilitation may lead to non-union of the greater tuberosity. We hypothesise that a delayed rehabilitation will result in a good union rate without undue risk of shoulder stiffness.
MATERIALS AND METHODS: Between December 1996 and June 2003, 40 patients with three or four part fracture of proximal humerus with or without dislocation, not amenable to open reduction and internal fixation underwent hemiarthroplasty

The authors present the case of a displaced fracture of the coracoid process associated with a displaced fracture of the acromion, an undisplaced fracture of the clavicule and fractures of the first to fifth ribs. Open reduction of the coracoid process fracture was performed. At the last follow-up, the coracohumeral distance was restored (10 mm), but the acromiohumeral distance in the sagittal place was decreased (5 mm). The authors recommend open reduction of displaced fractures around the rotator cuff to limit the risk of impingement between the cuff and the coracoid process or the acromion.

Dislocation of the shoulder with ipsilateral shaft fracture is an uncommon, but serious injury. I present one case report and the guidelines used for treatment. The method employed was Hackethal's pins through open reduction. The dislocation was reduced during surgery. The fracture and dislocation healed without any problems, and there was no damage to the radial nerve. After a recent follow-up, the patient was allowed to return to work without restrictions, and he has regained normal mobility.

This is a case of an 11-year old girl with a delayed diagnosis of a shoulder fracture-dislocation. Shoulder dislocations are rare, and proximal humerus fractures are uncommon. A fracture-dislocation in a child is, to our knowledge, as yet unrepresented in the English-language literature. The child's injury resolved without recurrence.

The predictability of outcomes with reverse shoulder arthroplasty (RSA), compared with hemiarthroplasty or open reduction and internal fixation, has led to its increased use in treating displaced three- and four-part proximal humerus fractures (PHFs) in patients older than 65 years. Although RSA was initially designed to restore humeral elevation in the absence of a functional rotator cuff, studies have shown improved patient-reported outcomes and range of motion in patients who achieve tuberosity union following surgery. Despite numerous advancements in implant design over the past decade, op

This study was undertaken to determine the incidence, need for intervention, and time to resolution of pseudosubluxation of the shoulder in pediatric proximal humerus fractures. One hundred and ninety-nine radiographs (199 x-rays) were analyzed for pseudosubluxation of the shoulder following pediatric proximal humeral fractures. Pseudosubluxation occurs when the center of the humeral head aligns with the inferior one-fourth of the glenoid. Fourteen patients met the inclusion criteria for pseudosubluxation. The nonoperative cohort consisted of 100 females and 93 males and the operative cohort c

Although radiologists are likely familiar with basic fracture patterns around the shoulder, certain fracture types in this area have particularly relevant and controversial clinical management issues. These include proximal humeral fractures, in which conservative management is favored but some patients receive surgical treatment; glenoid fractures and Hill-Sachs lesions, in which surgery may be performed depending on the amount of bone loss; and distal clavicular fractures, in which certain displaced fractures have high rates of nonunion, but there is no clearly favored surgical technique.
