Sleepiness in elderly veterans.

PURPOSE: This observational study seeks to describe the distribution of sleepiness among elderly male veterans and to explain the relationship between sleepiness and age, function, mobility, and depression in this population. METHODS: Veterans who were age 60 or older and had two or more functional limitations based on their activities of daily living or instrumental activities of daily living were recruited in outpatient clinics. They were recruited as part of a longitudinal study and completed questionnaires reported here at the 18-month data collection time point. RESULTS: Veterans report…

Behavioral biomarkers of sleepiness.

For those endeavoring to develop better methods of measuring/quantifying sleepiness, the "Holy Grail" is a measure that is maximally objective, completely unobtrusive, exquisitely sensitive, and absolutely specific (i.e., varies only as a function of sleepiness). By these criteria, physiological measures (e.g., based on brain activity such as EEG, fMRI, near-infrared spectroscopy, etc.) would appear to hold the most promise. However, from an operational standpoint, the utility of a sleepiness measure is derived not from its ability to sensitively reflect the brain's extant level of sleepiness …

Sleepiness of occupational drivers.

Drowsiness and sleeping at the wheel are now identified as the reasons behind fatal crashes and highway accidents caused by occupational drivers. For many years, fatigue has been associated to risk of accidents but the causes of this symptom were unclear. Extensive or nocturnal driving was associated to accidents but few reports differentiated fatigue from sleepiness. In the early nineties, epidemiological data started investigating sleepiness and sleep deprivation as cause of accidents. Sleepiness at the wheel, sleep restriction and nocturnal driving have been incriminated in 20% of traffic a…

Sleepiness and human behavior.

The 24-hour properties of sleepiness affect behavior by reducing performance and increasing the likelihood of accidents. This is important to pulmonary physicians who diagnose and treat sleep apnea, because diagnoses of sleep apnea and narcolepsy are associated with as much as a sevenfold increase in the risk of having a motor vehicle accident. Human abilities throughout the 24-hour day have noticeable ups and downs and are probably causally linked to the same control mechanisms that produce the early morning and midafternoon peaks in the tendency to fall asleep. An important characteristic of…

Daytime sleepiness and antihistamines.

A daytime nap procedure was used to evaluate the daytime sleepiness associated with antihistamines, as well as to assess their hypnotic potential. Healthy, normal subjects received diphenhydramine (150 mg), terfenadine (120 mg), and placebo and went to bed at 900, 1100, 2000, and 2200 h with the instruction to try to fall asleep. The remained in bed for 60 min while standard sleep recordings were made. Across all conditions latency to stage 1 sleep increased significantly from nap 1 to nap 4 and the amount of sleep (all nonstage 1 sleep) decreased significantly. Over the four naps the mean lat…

Sleepiness in Children: An Update.

Sleep is essential for children's learning, memory processes, school performance, and general well-being. The prevalence of sleepiness in children is approximately 4%. Reductions of sleep duration have daytime consequences, including sleepiness, behavior problems, cognitive deficits, poor school performance, inflammation, and metabolic dysfunction. Chronic pain, movement disorders, and sleep-disordered breathing also may lead to daytime somnolence, inattention, hyperactivity, oppositional behaviors, and mood dysregulation. Parent-report questionnaires are useful tools to assess subjective slee…

Measurements of sleepiness and fatigue.

Sleepiness and fatigue are terms commonly used in clinical practice and research. At times sleepiness and fatigue are used interchangeably; however, each of them has distinct implications for diagnosis and treatment. The objective of this article is to review the psychometric properties of the measurements of sleepiness and fatigue. Although there are objective and subject measures to evaluate sleepiness, only rating scales are available to assess fatigue. Further research should be directed toward exploring the potential mechanisms underlying the measurements of sleepiness and fatigue. Establ…

Sleepiness and performance in narcolepsy.

Previous attempts to investigate the relationship between sleepiness and performance for subjects with narcolepsy have been limited by both the ability of narcoleptic subjects to contain their sleepiness for brief testing periods and the potential lack of sensitivity of routine performance tasks to sleepiness induced changes. The present study developed a research protocol which allowed subjects with narcolepsy to express states of sleepiness and non sleepiness and to then compare the performance of subjects with narcolepsy to age, gender and IQ matched controls on tasks evaluating automatic, …

Evaluation of the sleepy patient.

BACKGROUND: Inefficient sleep leading to excessive daytime sleepiness is a common complaint encountered by GPs and sleep physicians. Common causes of excessive daytime sleepiness include circadian rhythm disorder/shiftwork, sleep apnoea syndrome, psychiatric disorders, restless leg syndrome, medication effect, narcolepsy and idiopathic hypersomnia. OBJECTIVE: This short review discusses the available objective and subjective testing measures in office evaluation of sleepy patients, predominantly in the primary care setting. DISCUSSION: Beyond affecting patients' quality of life, mood and fun…

Approach to a sleepy child: Diagnosis and treatment of excessive daytime sleepiness in children and adolescents.

The aim of this review is to give updated information to pediatric neurologists on the correct diagnostic approach and treatment of excessive daytime sleepiness (EDS) in children and adolescents. Due to the change in the society habits, EDS is becoming an emerging problem for the health system. At the present there are few articles specifically devoted to the evaluation of EDS. EDS is often reported in several manuscripts as a side effect of other sleep disorders (obstructive sleep apnea, circadian disorders, etc.) or of the use of drugs or of the substance abuse or as a consequence of bad sle…

On treatment but still sleepy: cause and management of residual sleepiness in obstructive sleep apnea.

PURPOSE OF REVIEW: Although continuous positive airway pressure (CPAP) treatment effectively reduces sleepiness in obstructive sleep apnea (OSA) patients, some patients remain sleepy in spite of proper treatment. After exclusion or treatment of known causes of sleepiness, residual sleepiness may be diagnosed. Recent changes in approval for currently available wakefulness stimulants in Europe, development of new stimulants and questions about the reality of residual sleepiness prompted this review. RECENT FINDINGS: Prevalence of residual sleepiness is approximately 10% and clearly decreases wi…

The Time of Day Sleepiness Scale to assess differential levels of sleepiness across the day.

OBJECTIVE: The study evaluated the Time of Day Sleepiness Scale (ToDSS) to determine subjective estimates of sleepiness in the morning, afternoon, and evening. Scores on the ToDSS were compared to the Epworth Sleepiness Scale (ESS). The ToDSS was evaluated on three cohorts of patients at a sleep medicine clinic. METHOD: The items of the ToDSS are modified from the ESS to enable subjective assessment in the morning (before noon), afternoon (noon to 1800 h), and evening (after 1800 h). The scale takes about 5 min to complete. For each item, patients provide an estimate of their level of sleepin…
