Obesity And The Spleen.

This review describes the relationship of splenomegaly, or "splenic obesity", with metabolic syndrome, obesity and metabolic - associated steatohepatic disease (MAFLD), based on animal and human studies. The spleen is enlarged in obesity. It may play a protective role against obesity-associated inflammation and complications. This protection may be lost after splenomegaly. Persons with splenic disease must be evaluated for obesity, and vice versa and counselled to avoid injury to the organ.

The elusive spleen.

INTRODUCTION: The aim of this study was to review the experience of general surgeons performing splenectomy in a district general hospital. The outcomes are discussed together with potential reasons for the increasing rarity of the procedure. METHODS: A retrospective cohort study was carried out of all patients undergoing splenectomy (as identified by a single trust pathology department on receipt of splenic samples) between 1 January 2000 and 1 May 2017. Case notes and computer systems were interrogated for data on operating surgeon, patient demographics, diagnosis, surgical approach (laparo

Hemangiopericytoma of the spleen.

INTRODUCTION: Hemangiopericytoma of the spleen is a very rare tumor, with 14 isolated reports. It was our aim to review our experience and compare it with all the reported cases in an attempt to standardize surgical treatment, adjuvant treatment and follow-up protocol of this infrequent condition. METHODS: A consecutive case series study, with a mean follow-up of 44 months. Five patients (mean age, 49 years) underwent simple splenectomy for hemangiopericytoma limited to the spleen followed by adriamycin-based chemotherapy in one patient. RESULTS: All the patients are alive and free from dise…

Hidatidosis of the spleen.

UNLABELLED: Hydatidosis of the spleen (SH) is an extremally rare condition. The aim of the study was to present our experience and discuss diagnostic methods and therapeutical options in cases of spleen hydatidosis. MATERIAL AND METHODS: Between 1993 and 2008, 8 patients were diagnosed with SH, 6 of whom were operated on (4 males and 2 females). Their average age was 44.5 years (the age range 30-59 years). Four patients presented isolated SH, one had a spleen and liver hydatidosis and there was also a case of one spleen, liver and peritoneal hydatidosis. RESULTS: The main symptom of the cond…

Hamartoma of the spleen.

Splenic hamartoma is a rare, benign vascular proliferation that is often found incidentally while working up other complaints or at autopsy. … positive for CD8 are a key feature that differentiate hamartoma from other vascular lesions of the spleen. Clinical, radiologic, and histologic correlation is essential to ensure this benign lesion is not mistaken for malignancy.

Abscess of the spleen.

Abscess of the spleen is a very rare lesion. In this study, 4 cases of splenic abscess are presented and discussed along with the literature. The cases were between 16 and 55 years-old and two of them had hematologic malignancy. All of them had been operated on because of acute abdomen, and in two cases splenic rupture was present. Only in one of the cases was salmonellosis detected by microbiological methods. By histological examination, expansion and congestion in splenic sinusoids, and foci of abscess including wide areas of necrosis and inflammatory infiltration by neutrophils were seen in…

Cytology of the spleen.

Fine-needle aspiration of the spleen is a useful method for evaluation of reactive, inflammatory, and neoplastic disorders, particularly those that involve the hematopoietic system. Interpretation of splenic aspirates is enhanced by concurrent evaluation of blood and other hemic tissues.

[Hamartoma of the spleen].

Hamartoma is a rare benign lesion of the spleen. Between 140 and 150 cases seem to have been described so far. Hamartoma of the spleen may appear as a single or multiple lesions which may tend to converge. It appears in all ages, mainly in elderly persons. About 20% of patients were described in paediatric subjects. Half of the patients have no symptoms, so that hamartomas were discovered by chance at autopsy. Other 50% of patients had pain, splenomegaly, haematologic abnormalities (most frequently thrombocytopenia or pancytopenia) and spontaneous rupture with intra-abdominal bleeding. In chil…

[A wandering spleen].

A mobile asymptomatic abdominal mass was accidentally found in a 81-year-old man. Further studies revealed a wandering spleen. Wandering spleen is rare, the diagnosis can easily be made with ultrasonography. The patient presented was asymptomatic, but severe complications may occur. Congenital anomalies of ligaments, splenomegaly and pregnancy may be of aetiological importance. In young patients and in case of complaints splenopexia is the treatment of choice; the patient presented was treated conservatively.

[Autotransplantation of the spleen].

The authors present a report on the autotransplantation of the damaged spleen by successful use of a tissue fibrin glue to fix thin lienal slices to the greater omentum. The new method was used in 8 men and 2 women, with the exception of one man in all patients after multiple injuries. From the total number two women died from sequelae of the injuries. Complications caused by replantation of the tissue were not observed. All the other patients survive and have no complaints associated with the loss of the spleen. The function of the transplant can be tested only by scintigraphy and laboratory …

The wandering spleen.

Wandering spleen is an unusual entity, occurring in both sexes and at any age, but is more frequent in women of reproductive age and in children. Wandering spleen is probably most often a result of congenital anomalies of development of the dorsal mesogastrium, but acquired factors may have a role in certain instances. Patients present most commonly with an asymptomatic mass, mass and subacute abdominal or gastrointestinal complaints or with acute abdominal findings. Clinical diagnosis can be difficult, but noninvasive imaging procedures, such as sonography, nuclear scintigraphy, computed tomo…

Preservation of the spleen.

Because the spleen can no longer be considered readily dispensable, a variety of alternatives to total splenectomy (splenic repair, partial resection, autotransplantation, ischemic therapy, reticuloendothelial blockade and irradiation) have been developed to preserve functioning remnants in trauma and hypersplenism. It remains unclear how much spleen adequately protects against OPSI or under persistent workload stimulus leads to recurrence of hypersplenism.
