Upper lumbar spondylolysis is rare and there are few reports which describe the condition. Three patients with upper lumbar spondyloysis, two of whom underwent surgical treatment, are reviewed. A radiological review of 205 patients with back pain attending a Back Research Clinic revealed three additional patients with symptomatic upper lumbar spondylolysis. clinical diagnosis of this condition is often difficult and the results of operation are unsatisfactory. It is more common among men and trauma appears to play a major role in the production of symptoms.

Repetitive stress on the lumbosacral spine during sporting activity places the athletic patient at risk of developing symptomatic pars defect. Clinical history, physical examination, and diagnostic imaging are important to distinguish spondylolysis from other causes of lower back pain. Early pars stress reaction can be identified with advanced imaging, before the development of cortical fracture or vertebral slip progression to spondylolisthesis. Conservative management is first-line for low-grade injury with surgical intervention indicated for refractory symptoms, severe spondylolisthesis, or…

PURPOSE OF REVIEW: Spondylolysis is a common cause for back pain in the adolescent athlete. Increased awareness of the presentation of this subset of patients can aid in optimal outcomes. This paper aims to review the typical presentation of spondylolysis in the adolescent with specific focus on the adolescent athlete. We review current controversies in diagnosis and management and aim to provide a thorough review to aid the pediatrician in making clinical decisions for this subset of patients.
RECENT FINDINGS: The optimal algorithm for diagnostic imaging is controversial. Single positron emi…

STUDY DESIGN: A descriptive study (based on skeletal material) was designed to measure sacral anatomic orientation (SAO) in individuals with and without spondylolysis.
OBJECTIVE: To test whether a relationship between SAO and spondylolysis exists.
SUMMARY OF BACKGROUND DATA: Spondylolysis is a stress fracture in the pars interarticularis (mainly of L5). The natural history of the phenomenon has been debated for years with opinions divided, i.e., is it a developmental condition or a stress fracture phenomenon. There is some evidence to suggest that sacral orientation can be a "key player" in …

Though athletes may be able to return to their sport after treatment of spondylolysis and spondylolisthesis, prevention of back injury in at-risk patient athletes should be the treating physician's primary goal.

Isthmic spondylolisthesis, or spondylolisthesis due to a lesion of the pars interarticularis, is a common source of pain and disability in both the pediatric and adult population. This review examines the current diagnostic and treatment options for patients with this condition. It also reviews the results of the various interventions to facilitate the surgeon in choosing the appropriate treatment option for any given patient.

Spondylolysis and spondylolisthesis are common causes of back and leg pain in the athletic population. Spondylolysis originates most often in children between 5 and 10 years of age. Many types of athletic activities requiring lumbar extension and rotation place the immature spine at risk for developing these lesions.

Spondylolysis and spondylolisthesis are not uncommon causes of low back pain and sciatica among the general population. Symptoms from these complaints also appear to be increasing in frequency among those who participate in competitive sports, especially those resulting in heavy pressures on the lumbar spine. Neural arch dysplasia is often a predisposing factor and there is evidence that genetic factors may play a role. Isthmic spondylolysis and mild spondylolisthesis not exceeding 10 mm can be satisfactorily cured by a simple operation involving screwing of the defect. Younger patients have b…

Increased lumbar lordosis places increased stress on the pars interarticularis. Fatigue fractures of the pars can result in spondylolysis. There was a 50% incidence of asymptomatic spondylolysis in 18 patients who had Scheuermann's kyphosis and an increased lumbar lordosis. This significant increase further confirms the pathogenesis of spondylolysis. Patients with Scheuermann's disease with low-back pain should be evaluated with oblique radiographs of the lumbar spine to rule out spondylolysis.

Spondylolysis is a common problem found in 5% of the general population. The etiology is a combination of two factors: (1) hereditary predisposition resulting from a congenital deficiency of the sacrum and posterior structures and (2) developmental factors, such as trauma, posture, or certain repetitive activities, that may precipitate a stress fracture of the pars interarticularis in susceptible individuals. Although the lesion occurs during the growth years, few individuals develop symptoms during childhood and adolescence. For the occasional child who develops symptoms, the onset usually co…
