INTRODUCTION: Tropical sprue is a postinfective malabsorption syndrome that occurs in some tropical endemic areas.
CASE REPORT: A 65-year-old Caucasian patient, with no significant past medical history, living in Cambodia for 10 years, presented with a 23 kg weight loss and chronic diarrhea. Clinical examination was unremarkable. Laboratory tests showed a moderate nutritional deficiency syndrome. The upper gastrointestinal endoscopy showed duodenal villous atrophy and histological analysis confirmed subtotal villous atrophy with important intraepithelial lymphocytosis. The diagnosis of tropic

A diagnosis of tropical sprue, an infrequent affliction of inhabitants and travelers in tropical regions, should be considered in patients with a compatible history, malabsorption, and chronic diarrhea. It can occur in either endemic or epidemic form and can be preceded by acute gastroenteritis. The cause of tropical sprue is still unknown, although most data support an infectious etiology. Therapeutic experience is greatest with folic acid and tetracycline. Most patients can be expected to recover with proper nutritional support, although relapses and slow responses occur.

Immunological measurements in 85 southern Indian patients with tropical sprue were compared with two groups of controls, namely, 150 patients with gastrointestinal diseases not associated with malabsorption, and 57 normal asymptomatic subjects. A significant increase in intraepithelial lymphocytes in jejunal surface and crypt epithelium, related to the severity of the mucosal morphological abnormality, was the most significant finding in tropical sprue. Measurements such as increased serum concentrations of IgG, IgE, complement component C4 and orosomucoid, gastric parietal cell antibodies and

Previous studies indicated that many patients with tropical sprue in southern India have triglyceride accumulation within the cells of the intestinal mucosa. This could be due to essential fatty acid deficiency as a result of steatorrhea in subjects on a diet normally low in linoleic acid. Plasma lipids have, therefore, been studied in patients with tropical sprue and the results compared to values observed in healthy controls. The low intake of linoleic acid was found to be adequate to maintain normal levels of arachidonic acid and to repress the formation of 5,8,11-eicosatrienoic acid in rur

Twenty-four patients have been found who appear to have endemic tropical sprue, with malabsorption on biochemical, radiological and histological grounds. Patients with the tropical sprue syndrome responded equally well clinically and biochemically to folate and tetracycline. Jejunal histology responded least well to treatment. It is suggested that as more Gastrointestinal Units are opened, especially in humid sea level areas in Africa, more cases of endemic tropical sprue will be found.
