Resuscitating fresh stillbirths.

Guidelines for duration of resuscitation of freshly-stillborn term infants and their long term outcome are unclear. The predictive factors of both cerebral palsy and early neonatal demise include Apgar scores of 3 or less at 10 minutes, perinatal acidaemia and neonatal seizures. We describe the case of severe perinatal asphyxia (umbilical artery pH at birth of 6.75, Apgar scores of 0.1 and 5 0.1 5 at 1.5 and 10 minutes and neonatal hypertonia) in a term pregnancy, where the infant made an uneventful recovery and was discharged home well. Guidelines for neonatal resuscitation are discussed.

Meconium peritonitis in stillbirths.

Meconium peritonitis is a sterile, chemical peritonitis resulting from perforation of the bowel in perinatal life. In stillbirths meconium peritonitis is extremely rare. We report the autopsy findings in three fetuses ranging from 21-39 weeks gestation in which meconium peritonitis was identified. Maternal history in two cases was suggestive of possible fetal hypoxia. No family history of cystic fibrosis was discerned. One fetus was hydropic and abdominal calcifications were noted on postmortem radiograph. Gross evidence of meconium in the peritoneal cavity, visceral adhesions, and serosal nod…

Stillbirth: the heart of the matter.

We evaluated 2,083 cases within the Wisconsin Stillbirth Service Program (WiSSP) that had autopsy reports or ultrasound data relevant to the heart. Of these, 167/1,782 (9.4%) stillbirths after 20 weeks and 11/301 (3.7%) miscarriages <20 weeks had congenital heart disease (CHD). Cases were classified by type of heart defect and whether it related to cause of death. Among cardiac anomalies that contributed significantly to fetal death, 125/151 (83%) were associated with underlying conditions or syndromes, nearly half of which were chromosomal. The most common forms of CHD in stillborns were seve…

Discovering the cause of stillbirth.

PURPOSE OF REVIEW: Stillbirth remains a major problem worldwide, with disparities both between and within nations. Evaluation and classification is essential for development and evaluation of preventive strategies. RECENT FINDINGS: Recently, several organizations have developed standardized protocols for stillbirth evaluation on the basis of maternal history, foetal examination/autopsy and placental pathology. Evaluation is moving from a search for discrete causes to recognition of contributing factors. Comparison of classification guidelines identifies several with the potential for effectiv…

Determinants of stillbirth in Zambia.

OBJECTIVE: The objective of this study was to estimate the rates and determinants of stillbirth in an urban African obstetric population. METHODS: In this retrospective cohort study, we reviewed vital outcomes of newborns whose mothers received antenatal care, delivery care, or both antenatal and delivery care in the Lusaka, Zambia, public sector between February 2006 and March 2009. We excluded newborns weighing less than 1,000 g, those whose mothers died before delivery, and those born outside Lusaka. RESULTS: There were 100,454 deliveries that met criteria for inclusion. The median matern…

Etiology and prevention of stillbirth.

OBJECTIVE: This is a systematic review of the literature on the causes of stillbirth and clinical opinion regarding strategies for its prevention. STUDY DESIGN: We reviewed the causes of stillbirth by performing a Medline search limited to articles in English published in core clinical journals from January 1, 1995, to January 1, 2005. Articles before this date were included if they added historical information relevant to the topic. A total of 1445 articles obtained, 113 were the basis of this review and chosen based on the criterion that stillbirth or fetal death was central to the article.…

The infectious origins of stillbirth.

OBJECTIVE: Our objective was to determine the relationship between various types of perinatal infections and stillbirths. STUDY DESIGN: By use of various textbooks on perinatal infections, multiple MEDLINE searches, and the reference list of all appropriate manuscripts, the appropriate English language literature was reviewed to define the relationship between various perinatal infections and stillbirths. RESULTS: Infection may cause stillbirth by a number of mechanisms, including direct infection, placental damage, and severe maternal illness. A large variety of organisms have been associat…

A study on profile of stillbirths.

The incidence of stillbirths in "one year period" was studied by record analysis in a teaching hospital. A stillbirth rate of 38.4 per 1000 live births was observed. On admission 96.3% cases had some adverse foetal conditions detected readily. Leading factor detected was intra-uterine growth retardation (45.4%). Other major conditions observed were intra-uterine hypoxia (18.4%), malpresentation (11.9%), multiple pregnancy (8.3%). Birth weight was below 2000 g in 53.1% cases. Records showed 78.5% mothers had not received antenatal care.

Stillbirth in Low-Resource Settings.

Globally, 1.9 million late-gestation stillbirths occurred in 2023, 87% in low-income and middle-income countries, with sub-Saharan Africa bearing almost half the burden. Despite a global decline, progress is uneven and stalling in high-burden regions. Most are preventable but persist due to complex factors: biomedical causes (eg, hypertension and infection), societal determinants (eg, poverty, inequity, and low empowerment), and critical health system gaps limiting access to timely, high-quality antenatal, and intrapartum care. Ending preventable stillbirths requires urgent, intersectoral acti…

Stillbirth: we can do better.

Stillbirth is far too common, occurring in millions of pregnancies per year globally. The rate of stillbirth (defined as death of a fetus prior to birth at 20 weeks' gestation or more) in the United States is 5.73 per 1000. This is approximately 1 in 175 pregnancies accounting for about 21,000 stillbirths per year. Although rates are much higher in low-income countries, the stillbirth rate in the United States is much higher than most high resource countries. Moreover, there are substantial disparities in stillbirth, with rates twice as high for non-Hispanic Black and Native Hawaiian or Other …

Late stillbirth due to listeriosis.

Listeriosis is a sporadic infectious disease, which affects high-risk populations, such as the elderly, pregnant women, newborns, and immunocompromised patients. During pregnancy, listeriosis usually presents like a mild non-specific infection, but it may be responsible for fetal loss, preterm labor, early onset neonatal sepsis, and neonatal death. We report the case of a late stillbirth secondary to maternal chorioamnionitis. Listeria monocytogenes was isolated from the amniotic fluid and the fetal pleural fluid. The fetal autopsy revealed a disseminated inflammatory response with multi-organ…

Novel strategies to prevent stillbirth.

This article reviews three new and emerging risk factors for stillbirth that may be modifiable or might identify a compromised fetus. We focus on fetal movements, maternal sleep, and maternal diet. Recent studies have suggested than a sudden increase in vigorous fetal activity may be associated with increased risk of stillbirth. We review the papers that have reported this finding and discuss the implications as well as potential future directions for research. There is emerging literature to suggest that maternal sleep position may be a risk for stillbirth, especially if the woman settles to …

Stillbirth and fetal growth restriction.

OBJECTIVE: To confirm the role of fetal growth restriction (FGR) as a cause of stillbirth, and to compare diagnostic accuracy of customized fetal growth and population-based standards in identifying FGR within a pathological population of early and late stillbirths. METHODS: Retrospective study on a cohort of 189 stillbirths occurred in single pregnancy between January 2006 and September 2011. Unexplained stillbirths, defined by Aberdeen-Wigglesworth and ReCoDe classifications, were evaluated on the basis of fetal birthweight with both Tuscany population and Gardosi customized standards. Unex…

[Care in stillbirth; mothers' opinion].

Semi-structured interviews were conducted with 49 women who had experienced stillbirth in 1987-1989. Only 55% were satisfied with the manner in which the first suspicion had been communicated to them, while 85% were satisfied with the communication at the time of the diagnosis. Opinions about care in the labour ward, while largely positive, indicate that lack of tact from an individual may cloud the entire perception of care. Although only 40% had expressed a desire to see the baby, the others were happy to have been persuaded to do so and all were positive about the contact with the dead baby…

Care in pregnancy after stillbirth.

Pregnancy after stillbirth is associated with increased risk of stillbirth and other adverse pregnancy outcomes including fetal growth restriction, preeclampsia, and preterm birth in subsequent pregnancies. In addition, pregnancy after stillbirth is associated with emotional and psychological challenges for women and their families. This manuscript summarizes information available to guide clinicians for how to manage a pregnancy after stillbirth by appreciating the nature of the increased risk in future pregnancies, and that these are not affected by interpregnancy interval. Qualitative studi…

Temperature, placental abruption and stillbirth.

BACKGROUND: Pregnant women may be vulnerable to changes in ambient temperature and warming climates. Recent evidence suggests that temperature increases are associated with placental abruption, a risk factor for stillbirth. OBJECTIVES: We investigated the effect of acute exposures to apparent temperature on stillbirths in Harris County, Texas, 2008-2013. METHODS: We conducted a case-crossover study to investigate the association between temperature and stillbirth among 708 women. We used data from the National Climatic Data Center to estimate maternal exposure to daily average apparent tempe…
