Aspects of the fine anatomy of aphthous stomatitis.

Samples of human aphthous stomatitis were studied under the electron microscope. The epithelial cells bordering the ulcer present multivesicular bodies, pinocytotic vesicles, primary lysosomes, digestive vacuoles, and autophagic vacuoles. The golgicomplex is noticeably increased; free ribosomes and rough endoplasmic reticulum are abundant. Crystalline structures are close to the nucleus. One or two paranuclear bodies are in most of the nuclei. A frank cell injury is evident, causing a major activity of the digestive cell system; the origin and formation of lysosomes through r.e.r. and the Golg

Hematological study of patients with aphthous stomatitis.

OBJECTIVES: A study was made of the association between recurrent aphthosis (RAS) and iron, ferritin, vitamin B12, and folic acid deficiency. METHODS: A total of 186 adults were consecutively recruited: 92 subjects with RAS (28 males and 64 females, with a mean age of 41 years) and 94 controls (20 men and 74 women, with a mean age of 44 years), and subjected to clinical and hematological evaluation. RESULTS: The overall frequency of hematinic deficiencies was 14.14% in the RAS group versus 6.39% in the control group (P = 0.086). Patient age and a family history of aphthosis were associated t

Etiology and Management of Recurrent Aphthous Stomatitis.

Recurrent aphthous stomatitis (RAS) is an ulcerative condition that affects the oral mucosa without evidence of an underlying medical disorder. RAS is characterized by the appearance of round, shallow ulcerations surrounded by inflammation that chiefly involves the nonkeratinized mucosa. The etiology of RAS is unknown, but has a strong hereditary component and appears to be related to an immune reaction against the oral mucosa. RAS must be distinguished from other diseases that cause recurring oral ulcers such as Behçet's syndrome, systemic lupus erythematosus, and Crohn's disease. This paper

Observations on the diagnosis of recurrent aphthous stomatitis.

The case histories of four patients--one with recurrent aphthous stomatitis who had a "correctable" deficiency, one with pemphigus vulgaris, one with cicatricial pemphigoid, and one with lichen planus--are reviewed. All had oral ulcerations resembling recurrent aphthous stomatitis. These cases demonstrate the heterogeneity of lesions that might be diagnosed as "canker sores." The necessity for clinical correlation and specific laboratory, histopathologic, and immunopathologic diagnostic tests is delineated.

Azithromycin-induced Aphthous Stomatitis: A Case Series.

BACKGROUND: Azithromycin is one of the most popular antibiotics in current clinical practice. This medication generally considered to be safe and well-tolerated in different demographic populations. Like any other drug, azithromycin use is not without risk and adverse effects. In recent years, cardiovascular accidents have been announced as its major and most important side effect. But azithromycin use can be accompanied with less recognized complications which are significantly discomforting. In this article, we presented a neglected adverse effect of azithromycin in medical literature which

Laryngeal manifestations in atypical recurrent aphthous stomatitis.

UNLABELLED: Recurrent aphthoid stomatitis is characteristically observed in children and adolescents in the form of painful relapsing ulcers in the oral mucosa unaccompanied by evidences of systemic disease. The ulcers appear every one or two weeks for at least one entire year. Some patients suspected for recurrent aphthoid stomatitis develop lesions in atypical sites - mainly in the larynx - concurrently to the ones found in the oral mucosa. AIM: this study aims to describe a series of recurrent aphthoid stomatitis patients with atypical laryngeal injuries. STUDY DESIGN: this is a case seri

Recurrent aphthous stomatitis: etiology, diagnosis, and treatment.

Recurrent aphthous stomatitis is a very common condition seen frequently by dentists in practice. Although the majority of cases are benign and resolve in less than two weeks, these ulcerations may be indicative of underlying systemic diseases ranging from vitamin deficiency to autoimmunity. Awareness of these correlations can help the dentist make or aid the diagnosis of potentially serious conditions. There are treatments of which the practitioner should be aware that may ameliorate the pain of the ulcerations or shorten their duration. The diagnosis and management of this important conditio

Recurrent aphthous stomatitis: genetic aspects of etiology.

Recurrent aphthous stomatitis (RAS; recurrent aphthous ulcers - RAU; canker sores) is a chronic inflammatory, ulcerative condition of the oral mucosa. Its prevalence in the general population ranges between 5% and 20%, depending on the method and group studied. The etiopathogenesis of the disease is considered to be multifactorial, but remains still not fully understood. In patients with RAS, an enhanced immunologic response occurs to some trigger factors that may include: mechanical injury, stress or bacterial and viral antigens. Higher prevalence of aphthae in relatives may also indicate the

Thyroid autoimmunity associated with recurrent aphthous stomatitis.

BACKGROUND: Recurrent aphthous stomatitis (RAS) is an autoimmune disorder characterized by the periodic appearance of aphthous lesions on the oral mucosa. TH1 cytokines plays a key role in the aetiopathogenesis. Autoimmune thyroid disease (ATD) is the most common autoimmune disease and is frequently accompanied by various other autoimmune diseases. OBJECTIVE: To investigate the frequency of ATD which has not been studied in the patients with RAS. METHODS: Ninety patients and 30 healthy volunteers were included into the study. The serum samples were assayed for thyroid stimulant hormone (TSH)

Polymorphonuclear neutrophil function in recurrent aphthous stomatitis.

Polymorphonuclear neutrophils (PMN) are the predominant cell type in the established lesion of recurrent aphthous stomatitis (RAS). The function of these cells was studied in 15 patients with RAS and 15 age and sex matched controls. Production of oxygen radicals was assessed using cytochrome-c and nitroblue tetrazolium dye (NBT) reduction assays. Oxidative metabolism was found to be similar for experimental and control groups in both stimulated and non-stimulated assays. Adherence to glass surfaces by PMN was also measured. Adherence of PMN, both from patients and controls, was significantly r

Anamnestic findings from patients with recurrent aphthous stomatitis.

Recurrent aphthous stomatitis (RAS) is a common oral disorder with a prevalence varying between 5% and 66%. RAS appears in three forms; minor, major and herpetiform. The aetiology is unknown. The aim of this study was to evaluate associations between specific anamnestic information and different types of recurrent aphthous stomatitis (RAS). A group of 177 patients (mean age = 42.8 years; SD = 14.3; range 17-79 years) participated. Data were collected from a structured interview, consisting of 22 questions. Information about i) health status and medication, ii) predisposing factors, iii) RAS exp

Mucosal microbiome in patients with recurrent aphthous stomatitis.

Recurrent aphthous stomatitis (RAS) is the most common disease affecting oral mucosae. Etiology is unknown, but several factors have been implicated, all of which influence the composition of microbiota residing on oral mucosae, which in turn modulates immunity and thereby affects disease progression. Although no individual pathogens have been conclusively shown to be causative agents of RAS, imbalanced composition of the oral microbiota may play a key role. In this study, we sought to determine composition profiles of bacterial microbiota in the oral mucosa associated with RAS. Using high-thr

Systemic interventions for recurrent aphthous stomatitis (mouth ulcers).

BACKGROUND: Recurrent aphthous stomatitis (RAS) is the most frequent form of oral ulceration, characterised by recurrent oral mucosal ulceration in an otherwise healthy individual. At its worst RAS can cause significant difficulties in eating and drinking. Treatment is primarily aimed at pain relief and the promotion of healing to reduce the duration of the disease or reduce the rate of recurrence. A variety of topical and systemic therapies have been utilised. OBJECTIVES: To determine the clinical effect of systemic interventions in the reduction of pain associated with RAS, a reduction in e

[Periodic fever, aphthous stomatitis, pharyngitis and adenitis syndrome].

Periodic fever, aphthous stomatitis, pharyngitis and adenitis (PFAPA) syndrome is a non-hereditary autoinflammatory disease, characterized by relatively regular recurrence of febrile episodes of 3-6 days duration, accompanied by aphthous stomatitis, pharyngitis/tonsillitis, and/or cervical adenititis. It is considered to be the most common periodic fever syndrome in Japan. Although no responsible gene is identified, some genetic factors may confer the predisposition toward this disorder. Important differential diagnosis includes hereditary periodic syndromes and cyclic neutropenia. Although it

Mucosal disease series. Number VI. Recurrent aphthous stomatitis.

Recurrent aphthous stomatitis (RAS; aphthae; canker sores) is a common condition which is characterized by multiple recurrent small, round or ovoid ulcers with circumscribed margins, erythematous haloes, and yellow or grey floors typically presenting first in childhood or adolescence. RAS occurs worldwide although it appears most common in the developed world. The aetiology of RAS is not entirely clear. Despite many studies trying to identify a causal microorganism, RAS does not appear to be infectious. A genetic predisposition is present, as shown by strong associations with genotypes of IL-1

PFAPA syndrome (Periodic Fever, Aphthous stomatitis, Pharyngitis, Adenitis).

This paper aims to remind paediatric clinicians to suspect and confirm 'PFAPA' syndrome (Periodic Fever, Aphthous stomatitis, Pharyngitis and cervical Adenitis syndrome). We report two cases of PFAPA syndrome: a 3-year-old healthy boy with atopic rhinitis and a boy aged 8 years 5 months who simultaneously had lymphocytic vasculitis syndrome treated with immunosuppressive drugs. Both met Marshall's criteria. The literature regarding PFAPA syndrome was complied using a Medline search for articles published between 1963 and 1998 and we then reviewed the reference lists of the articles. The Medlin

Topical and systemic therapy for recurrent aphthous stomatitis.

The cause of recurrent aphthous ulcers (RAU), the lesions of recurrent aphthous stomatitis, is incompletely understood but appears to involve immune system dysfunction. Treatment options include no treatment, treatment of associated systemic diseases or conditions (eg, celiac sprue, vitamin deficiencies), systemic medications, topical medications, conversion of the aphthous ulcer to a wound, and palliative treatments. The most effective treatments (systemic or topical corticosteroids, thalidomide) involve agents that suppress or modulate immune system function. In general, topical agents are p
