The threshold for the detection of strabismus.

PURPOSE: To identify the threshold at which horizontal or vertical strabismus becomes reliably detectable by observers and to determine the effects of interpupillary distance, age, gender, and observer experience. METHODS: Six models of different gender, age, and interpupillary distance were digitally photographed in several predetermined gaze positions off-axis in the horizontal and vertical planes. Standardized distance, zoom factor, and lighting were used. The images were digitally altered to exactly superimpose one eye deviated and one eye aligned with the axis of the camera. This simulat…

Development of refraction and strabismus.

In the past year, as in recent years, most of the research on the development of refraction has focused on the following: 1) mechanisms wher … cerbate myopia. Analyses of risk factors and numerous new screening procedures detect patients with strabismus for referral at a variety of sensitivity and specificity levels. Hyperopia and high AC/A ratios are most clearly associated as causal agents for esotropia and intermittent exotropia. However, the action of even these simple mechanisms is confounded …

Development of refraction and strabismus.

The goal of current research on the development of refraction is not to establish whether refraction is either genetically programmed or environmentally influenced, but to assess how heredity and environment interact in the genesis of refractive error. The continuing problem in strabismus research is one of cause and effect. Do binocular deficiencies induce strabismus, or is it the other way around? The papers highlighted in this article address these critical issues. The new findings have the potential to influence the treatment and clinical care of patients with refractive errors and strabis…

Strabismus associated with meningomyelocele.

INTRODUCTION: Strabismus occurs frequently in patients with spina bifida or meningomyelocele. PATIENTS AND METHODS: I performed 1544 eye examinations in 298 patients with spina bifida over a period of 15 years. The patient records were retrospectively reviewed for visual acuity, binocular function, and strabismus. Select patients with A-pattern strabismus were recalled for neuroimaging. RESULTS: Visual acuity was correctable to 20/40 in 85% of the patients. One hundred and eighty-three patients (61%) had strabismus at some time over the study period. Sixty-three percent had an esodeviation; …

Paralytic strabismus and syndromes.

In the past year, there have been interesting communications regarding third nerve palsies in children and acquired fourth nerve palsy. Several interesting articles have confirmed the benign natural history of untreated Brown's syndrome and addressed its management. Two major reviews, one on vertical diplopia and one on Duane's retraction syndrome, have provided a wealth of information and useful bibliographies.

Strabismus in the Marfan syndrome.

We studied 573 patients with the Marfan syndrome for the presence of ocular misalignment, refractive errors, and amblyopia. A total of 110 patients (19.2%) had strabismus. Of 573 patients, exotropia occurred in 67 (11.7%), esotropia in 12 (2.1%), vertical deviations in eight (1.4%), and primary inferior oblique muscle overaction in three (0.5%). Of 67 patients with exotropia, 18 had anisometropia and 39 had amblyopia. Of the 12 patients with esotropia, four had amblyopia and none had anisometropia. Exotropia and esotropia are more common in patients with the Marfan syndrome than in the general…

[Vertical strabismus in plagiocephaly].

Plagiocephaly is cranial synostosis caused by premature closure of one half of the coronal suture. It is manifested by asymmetry of the face of a varying degree with vertical strabismus on the affected side and tilting of the head to the sound side. The vertical deviation imitates ocular torticollis, there are, however, no signs of paresis nor a positive Bielschowski test. Treatment involves early opening of the coronal synostosis or later at least treatment of vertical strabismus. The authors operated successfully a 14-year-old girl with medium grade plagiocephaly. They describe the preoperat…

Strabismus in craniofacial dysostosis.

Ten infants and children who presented with craniofacial dysostosis are discussed; four had Apert's syndrome, four had Crouzon's syndrome, one had Pfeiffer's syndrome, and one had hypertelorism. The follow-up of the patients ranged from 3 months to 7 years, with an average of 19 months. Patients had bifrontal and biparietal craniectomies to correct frontal and temporal orbital retrusion, while two had left unilateral procedures only. One patient (T.S.) had had three similar procedures before he was 3 years old and patient B.B. had two before he was 11 months old due to the complete failure of

[History of strabismus surgery].

The history of strabismus surgery starts from the end of the eighteenth century. The first surgical trials consisted of performing myotomies of the medial rectus. Although Taylor from Great Britain could be one of the first to be mentioned, it was Dieffenbach from Germany who accomplished the first official myotomy in 1839. He is followed by many authors as Roux, Velpeau in Paris, and Bonnet in Lyon, the latter performing tenotomy instead of myotomy. In 1849 Guerin performed muscular advancement. In 1883, de Wecker described the muscular pleating, and Blascowiczs the muscular resection. Thus, …

Restrictive factors in strabismus.

Either muscle weakness (paresis) or mechanical restrictions can account for diminished ocular rotation. In practice, restrictions are more commonly seen. The forced duction test, differential intraocular pressure measurement and saccadic velocity studies can all assist in documenting the presence of restriction. Restrictions frequently occur with orbital floor fracture, endocrine ophthalmopathy and Brown's syndrome, and following multiple stabismus procedures, orbital or retinal detachment surgery, or muscle transposition surgery. They also occur as a result of antagonist muscle contracture af
