Association of a Disease Site Specific Chart Rounds Format With Peer Review Feedback: A Prospective Study.

PURPOSE: Peer review of radiation treatment plans (chart rounds) is recommended by professional societies and is widely used, although variation exists in implementation. The aim of this study was to investigate the association of a disease site-specific chart rounds format with the frequency of peer review feedback. METHODS AND MATERIALS: At a single academic radiation oncology department with multiple sites of clinical practice, weekly review of new treatment plans transitioned from clinical practice site to disease site-specific chart rounds. Peer review participants were blinded to prospe

How Would You Resuscitate This Patient With Septic Shock? : Grand Rounds Discussion From Beth Israel Deaconess Medical Center.

Sepsis is a potentially life-threatening systemic dysregulatory response to infection, and septic shock occurs when sepsis leads to systemic vasodilation and subsequent tissue hypoperfusion. The Surviving Sepsis Campaign published updated guidelines in 2021 on the management of sepsis and septic shock. Here, in the context of a patient with septic shock, 2 critical care specialists discuss and debate conditional guideline recommendations on using lactate to guide resuscitation, the use of balanced crystalloids versus normal saline, and the use of corticosteroids.

An Unusual Case of Low Vancomycin Exposure Despite Extremely High Vancomycin Doses Accompanied by Renal Toxicity: A Grand Round.

This grand round describes the case of a patient who received 10 grams (143.5 mg/kg) of vancomycin every 24 hours via continuous infusion, in whom the highest observed level was only 15.4 mg/L. Despite subtherapeutic levels, renal impairment was encountered, which resolved after the discontinuation of vancomycin. Glomerular hyperfiltration was found through nuclear glomerular filtration rate measurement, which likely explains the need for high doses (>6 grams per 24 hours continuous infusion) without reaching therapeutic serum levels.

A Perfect Storm Averted: Flawed Systems, a Dropped Ball, and Cognitive Biases Delay a Critical Diagnosis.

This is the first case of Cancer Morbidity, Mortality, and Improvement Rounds, a series of articles intended to explore the unique safety risks experienced by oncology patients through the lens of quality improvement, systems and human factors engineering, and cognitive psychology. This case highlights how multiple overlapping factors contributed to a delay in diagnosing disseminated tuberculosis in a patient with lung cancer. The discussion focuses on the ways that cognitive biases contributed to the delayed diagnosis in a patient who, with the benefit of hindsight, exhibited several signs an

Facilitating Academic Success for Practicing Clinicians.

The Psychiatric Consultation Service at Massachusetts General Hospital sees medical and surgical inpatients with comorbid psychiatric symptoms and conditions. During their twice-weekly rounds, Dr Stern and other members of the Consultation Service discuss diagnosis and management of hospitalized patients with complex medical or surgical problems who also demonstrate psychiatric symptoms or conditions. Enhancing Interdisciplinary Team Rounds: A Novel Intervention To Improve Interdepartmental Communication for Complex, Hospitalized Patients.

BACKGROUND: Limited opportunities exist for multiple clinical teams to engage in real-time, in-depth communication. This study aimed to improve multiclinician participation in interteam/interdisciplinary discussions. MEASURES: This study measured the number of disciplines and individual clinicians represented in discussions, the proportion of involved teams participating, and clinician satisfaction. INTERVENTION: A new format was created, inviting all clinicians caring for a highly complex patient to participate in an in-depth, interteam, interdisciplinary discussion. OUTCOMES: An average o
