PURPOSE: HIV prevalence and outcome disparities among sexual and gender minorities are profound in the United States. Tennessee HIV surveillance practices have not been uniform for transgender status, although data collection capabilities exist. We, therefore, describe current reporting of data on transgender individuals in Tennessee to identify targets for improvement.
METHODS: Data for all HIV-diagnosed individuals living in Tennessee as of December 31, 2013, were extracted from the Enhanced HIV/AIDS Reporting System (eHARS). The birth_sex ("Male" or "Female") and current_gender ("Male," "F…

Eight samples of desiccated human feces collected from Big Bone Cave (40VB103), Van Buren County, Tennessee, were analyzed to determine the presence of ecto- and endoparasitic infection among the prehistoric population using the cave. Radiocarbon-dated torch material from the cave indicated that it was a locus of human activity 2,177 +/- 145 yr ago. Parasitic species identified were: Ascaris lumbricoides, Enterobius vermicularis, fleas of the tribe Phalacropsyllini, and protozoan cysts. The cysts were identified as Giardia using an indirect immunofluorescent antibody test. The only report of G…

Rabies virus (RABV) infects multiple bat species in the Americas, and enzootic foci perpetuate in bats principally via intraspecific transmission. In recent years, bats have been implicated in over 90% of human rabies cases in the US. In Tennessee, two human cases of rabies have occurred since 1960: one case in 1994 associated with a tricolored bat (Perimyotis subflavus) RABV variant and another in 2002 associated with the tricolored/silver-haired bat (P. subflavus/Lasionycteris noctivagans) RABV variant. From 1996 to 2010, 2,039 bats were submitted for rabies testing in Tennessee. Among 1,943…

Q fever is a zoonotic disease caused by the bacterium Coxiella burnetii. The most common reservoirs are domesticated ruminants, primarily cattle, sheep, and goats. Humans acquire Q fever typically by inhaling aerosols or contaminated dusts derived from infected animals or animal products. Its highly infectious nature and aerosol route of transmission make C. burnetii a possible agent of bioterrorism. Although up to 60% of initial infections are asymptomatic, acute disease can manifest as a relatively mild, self-limited febrile illness, or more moderately severe disease characterized by hepatit

Opioid use during pregnancy is on the rise in the United States. Neonatal abstinence syndrome (NAS), also known as newborn drug withdrawal, is a public health epidemic. Between 2004 and 2014, Tennessee experienced a fivefold increase in NAS hospitalizations, from 1.5 to 8.0 per 1,000 live births. Soaring increases in the number of newborns with NAS nationwide have caught the attention of many federal and state lawmakers, especially given the unknown burdens associated with medical and social services needed by those affected over time. Tennessee opioid-related regulations and laws enacted betw…

Lyme disease in the United States is caused by the bacterial spirochete Borrelia burgdorferi s.s. (Johnson, Schmid, Hyde, Steigerwalt, and Brenner), which is transmitted by tick vectors Ixodes scapularis (Say) and I. pacificus (Cooley and Kohls). Borrelia lonestari, transmitted by the tick Amblyomma americanum L., may be associated with a related syndrome, southern tick-associated rash illness (STARI). Borrelia lonestari sequences, reported primarily in the southeastern states, have also been detected in ticks in northern states. It has been suggested that migratory birds may have a role in th

We estimated the Leptospira seroprevalence in dogs, cats, and horses from Tennessee, USA, using the microscopic agglutination test (MAT) against 12 Leptospira serovars. We observed Leptospira seropositivity in 110 of 374 (29.4%) dogs, 21 of 170 (12.4%) cats, and 42 of 88 (47.7%) horses. The highest seroprevalence was observed for serovars Autumnalis (74.6%) in dogs, and Bratislava in cats (42.9%) and horses (95.2%). We found a significant level of potential cross-reactivity between multiple Leptospira serovars tested, with highest cross-reactivity to serovar Autumnalis in dogs. Leptospira sero…

In summary, several errors occurred with this patient. One, the patient should have been treated prophylactically for P. multocida, as most cat bites become infected. In the patient not allergic to penicillin, augmentin is the drug of choice, not erythromycin. Two, rabies postexposure prophylaxis should have been advised immediately after assessing the significance of the exposure. A feral cat must be assumed to be rabid if it cannot be quarantined for 10 days. Therefore, the bite or scratch from such an animal constitutes a significant rabies exposure. Three, initial postexposure rabies proph

Tennessee's ecological diversity, spanning forests, farmland, and urban areas, provides an ideal foundation for applying the One Health approach, which integrates human, animal, and environmental health. This review examines Tennessee's current One Health landscape, highlighting active initiatives, ongoing challenges, and future directions. Key efforts involve workforce development, disease surveillance, outbreak response, environmental conservation, and public education, led by a coalition of state agencies, universities, and the Tennessee One Health Committee. These programs promote cross-se…

OBJECTIVES: To understand immediate postpartum long-acting reversible contraception (IPP LARC) desire and utilization trends among publicly insured patients delivering at one academic hospital in a state with health care barriers and high short-interval birth rates.
STUDY DESIGN: We conducted a retrospective cohort study of electronic delivery records between March 2018 and June 2023 for publicly insured patients. Patient demographics, IPP LARC desire and utilization trends were compared using χ2 or Fisher exact tests. Binary logistic regression explored the relationship between IPP LARC util

INTRODUCTION: Phentermine, one of the most-commonly prescribed pharmaceuticals for weight-loss in the United States (US), has appeared on toxicology and listed as a cause of death in fatal drug overdoses in the state of Tennessee. This study aims to evaluate phentermine's involvement in fatal drug overdoses in the state of Tennessee.
METHODS: We used Tennessee State Unintentional Drug Overdose Reporting System (SUDORS) data and controlled substances monitoring program data (CSMD) to evaluate demographics, prescription history and co-occurring substances on toxicology in phentermine-positive c…

We reviewed genetics charts of 2235 patients seen from 1985 to 1990 at Vanderbilt University Medical Center, Nashville, Tennessee, and summarized the 20 most common reasons for referral (occurring in 1138 of the patients) and the diagnoses or conditions among patients receiving genetic services in one of four clinical settings (prenatal counseling clinics, general genetics clinics, outreach genetics clinics, and ward consultations). The five most common reasons for referral were advanced maternal age (> or = 35 years) (203/1138, or 18% of patients), followed by dysmorphic features/multiple con…
